Gypsy Vanner Horse Society DNA Analysis Form Registered Name of Horse Registration Number Gender Stallion Mare Gelding Color DOB Name of Sire Sires DNA Accession number Sires Reg # Name of Dam Dams DNA Accession number Dams Reg # I certify the identity of the horse listed above as pending registration with the GVHS, and that the attached hair sample was obtained from this same horse. Owner name Address Telephone --------------------------------------------------------------------------------------------------------------------------------------Signature of Owner Date The purpose of the DNA sampling is to verify parentage and to identify the DNA type for the individual horse. The information received from the test will be held by the Gypsy Vanner Horse Society and the University of Kentucky. The University may use the samples in its research. The GVHS does authorize the University of Kentucky to share this DNA information with certain other registries as needed to verify parentage. Hair samples are collected by pulling (not cutting) either mane or tail hairs so that the root bulb is left intact. Attach with tape approximately 50 hairs with root bulbs to the bottom of this form. Mail this form with the hair sample to: University of Kentucky Animal Genetic Testing & Research Lab Department of Veterinary Science 108 Gluck Equine Research Center Lexington, KY 40546-0099 TAPE HAIR SAMPLE HERE
