Strain Review for Donating Strains to the CGC (submitting this form does NOT guarantee your strain(s) will be accepted into the collection) Please complete this form and email it to [email protected] BEFORE sending the strains! Strain Name: The lab designation used to name the strain must have been approved by the CGC. Complete Genotype: You must include a complete genotype with gene and allele names. If you are sending a transgenic strain, a complete genotype and the transgene is required, using standard C. elegans nomenclature. For example, if the strain contains an extrachromosomal or integrated array, the name and full genotype of the array is required, including any transformation marker genes. Any mutations included in the background but rescued by the array must also be specified (i.e. – unc-119). Please indicate the chromosome/LG on which each mutation, transgene insertion, etc., resides. Species: Name of person and who made the strain: Code of lab submitting the strain: Phenotype: A complete phenotypic description of the strain. If the strain is heterozygous, include the segregating phenotypes. Mutagen used (if applicable): # of times outcrossed: If the strain has not been outcrossed, indicate that with a “0.” Culture conditions: e.g. growth temperature if temperature-sensitive of nutritional requirements if necessary. Reference: Full citation of the first description of the strain in the literature. Subsequent publications that contain additional significant information about the strain should also be included. Please justify why this strain should be added to the CGC permanent collection: ___ First allele of a gene ___ First or second null allele of a gene ___ Unusual allele of a gene already in collection Specify unusual property: ___ New deficiency Specify endpoints: ___ New balancer chromosome Specify region balanced: ___ Other Please specify and indicate justification:
