All Rights Reserved 6 Secretion of Estradiol ⢠The concentration of estradiol in the plasma during the menstrual cycle is shown. ⢠Almost all of this estrogen comes from the ovary, and two peaks of secretion occur: one just before ovulation and one during the midluteal phase. ⢠Estradiol secretion rate is 36 mcg/d (133 nmol/d) in the early follicular phase, 380 mcg/d just before ovulation and 250 mcg/d during midluteal phase. All Rights Reserved 14 Estrogen Receptor Sites ⢠Estrogen has two main receptor sites that it binds to in the body â Estrogen receptor alpha ⢠Increases cell growth â Estrogen receptor beta ⢠Decreases cell growth ⢠Helps prevent breast cancer development Estrogen Receptor Sites (contâd) ⢠E2 equally activates estrogenâreceptors alpha and beta ⢠E1 activates estrogenâreceptor alpha selectively in a ratio of 5:1 which increases cell proliferation ⢠E3 binds preferentially to estrogenâreceptor beta in a 3:1 ratio which may be the reason that E3 may help prevent breast cancer Estrogen Metabolism ⢠After menopause, the metabolism of estrogen can change. Consequently a women may respond differently to estrogen replacement. © 2015. All Rights Reserved 15 Estrogen Metabolism ⢠Two major competing pathways â 2âOH estrone â 16âOH estrone Estrogen Metabolism ⢠One minor pathway â 4âOH estrone 2âOH Estrone/Methylation ⢠Good estrogen. It does not stimulate the cell growth. ⢠Blocks action of stronger estrogen products that may be carcinogenic. ⢠2âOH estrone is protective against cancer when methylated by catecholâOâmethyltransferase (COMT) into 2âmethoxyâestrone. The ratio of 2âmethoxyestrone to 2âhydroxyestrone can be measured in the urine and is a good gauge of the bodyâs ability to methylate. © 2015. All Rights Reserved 16 2âOH Estrone/Methylation (contâd) ⢠Another way of evaluating the bodyâs ability to methylate is by measuring the homocysteine level. ⢠Low ratios of 2/16 hydroxy estrogen are also associated with an increased rate of developing lupus. Factors that Support Methylation SAMe Methionine B2, B6, B12 Folic acid (also as folinic acid, 5âformyl THF, or 5â methyltetrahydrofolateââMTHF) ⢠TMG (betaine) ⢠Reducing catecholamine production by decreasing stress ⢠⢠⢠⢠16âOH Estrone ⢠Has significant strong estrogenic activity and studies show it may be associated with an increased risk of breast cancer. ⢠High levels are associated with obesity, hypothyroidism, pesticide toxicity (organochlorines), omegaâ6âfatty acid excess, and inflammatory cytokines. © 2015. All Rights Reserved 17 4âOH Estrone ⢠Studies show it may directly damage DNA and cause mutations. Therefore, it is proposed to enhance cancer development. ⢠Equine estrogens, such as conjugated equine estrogen, increase metabolism into 4âOH estrones. ⢠Is present in greater quantities there is a deficiency of methionine and folic acid ⢠People who have uterine fibroids also may have increased levels of 4âOH estrone. How Can You Raise 2âOH Estrone? ⢠⢠⢠⢠⢠⢠Moderate exercise Cruciferous vegetables Flax Soy Kudzu Broccoli derivatives: indoleâ3âcarbinol taken as a supplement. Daily dose is 200 to 300 mg. Other derivatives of broccoli that have been shown to be effective are DIM (diindolymethane, a breakdown product of Iâ3âC) and sulforaphane glucosinolate How Can You Raise 2âOH Estrone? All Rights Reserved 19 Alcohol ⢠Alcohol interferes with the bodyâs ability to detoxify estrogen and increases E2 levels and the risk of breast cancer Antibiotics ⢠Antibiotics found in food may be associated with an elevated risk of breast cancer by changing the gut flora involved in the enterohepatic circulation of estrogens Estrogen and the Brain ⢠Common comments I hear from patients are the following: â âI think that I am losing my mind.â â âI feel like my body is divorcing itself.â â âI have lost the ability to spell.â â âI am always losing my keys.â â âI may be getting Alzheimerâs disease.â © 2015. All Rights Reserved 23 SERMS ⢠Tamoxafin Citrate and Raloxifene â Do not have the same positive affects on the brain that natural hormone replacement does. â They decrease total cholesterol by 5% and LDL by 10% but unlike natural hormone replacement are not very effective in lowering triglycerides and do not increase HDL. Progesterone Biosynthesis of Progesterone ⢠Progesterone is a C21 steroid secreted by the corpus luteum, the placenta, and in small amounts by the follicle ⢠It is an important intermediate in steroid biosynthesis in all tissues that secrete steroid hormones, and small amounts enter the circulation from the testes and adrenal cortex © 2015. All Rights Reserved 24 Biosynthesis of Progesterone (contâd) ⢠2% of circulating progesterone is free whereas 80% is bound to albumin and 18% bound to corticosteroidâbinding globulin ⢠Progesterone has a short halfâlife and is converted in the liver to pregnanediol, which is conjugated to glucuronic acid and excreted in the urine Secretion of Progesterone ⢠In men, plasma progesterone level is approximately 0.3 ng/ml (1 nmol/L). ⢠In women, the level is approximately 0.9 ng/ml (3 nmol/L) during the follicular phase of the menstrual cycle. â Difference is due to the small amounts of progesterone by cells in the ovarian follicles; theca cells provide pregnenolone to the granulosa cells, which convert it to progesterone. All Rights Reserved 25 Effects of Progesterone ⢠The principal target organs of progesterone are the uterus, the breasts, and the brain. ⢠Progesterone has an antiâestrogenic effect on the myometrial cells, decreasing their excitability, their sensitivity to oxytocin, and their spontaneous electrical activity while increasing their membrane potential. ⢠Progesterone downregulates the number of estrogen receptors in the endometrium and increases the rate of conversion of 17βâ estradiol to less active estrogens.
