YMCA OF MONTCLAIR EMPLOYEE BENEFIT SUMMARY Eligible Employees: Regular Full-Time Employees New full-time employees or staff members who become eligible due to their employment classification must complete 30 days of continuous employment as a full time employee to be enrolled in the group health and life insurance and long term disability plan. Coverage is effective the first of the month following 30 days of continuous employment as a full time employee. GROUP HEALTH COVERAGE Regular Full-Time employees and their eligible dependents are eligible to enroll, however, employees may elect to waive their rights to healthcare coverage. Eligible dependents include spouse, except in the event of divorce, legal separation or annulment and children from birth through age 25 (includes dependent stepchildren, legal guardianship, and legally adopted children). Medical and Dental coverage may also be continued for children past age 25 if they are incapacitated/handicapped, subject to approval by the Medical coverage administrator. To qualify for continued coverage, the child must become incapacitated/handicapped while enrolled in Medical/Dental coverage as an eligible dependent. Child includes the following: A stepchild or foster child, provided they are dependent upon the Employee for support and maintenance. A legally adopted child (A child is considered legally adopted on the earlier of the date of placement or the date the legal adoption proceeding have been started). Placement for adoption means the Employee has assumed a legal obligation for partial or total support of the child. Any other child related to an eligible Employee, mainly dependent on the eligible Employee for maintenance and support. Dependents must reside in the United States. Newborn Children Eligibility: Medical enrollment required during 30-day period following the birth of a child. Coverage is effective retroactive to the date of birth. Otherwise, there will be no coverage. Coverage: Group Health Insurance (Medical / Prescription Drugs /Dental / Vision) There are four categories options offered to employees. (Please review the Annual Group Rate Sheet). Employee payroll deductions for health and dental insurance are automatically taken on a pre-tax basis, unless an employee notifies us otherwise. Insurance Carriers: Medical: United HealthCare Prescription Drugs: This benefit remains with United HealthCare, however, the plan is administered by Optum Rx. Employees continue to use their HealthCare medical ID card. Dental: CIGNA Vision Care: EyeMed (Discount Program) Page 1 of 5 GROUP LIFE COVERAGE Group Life Insurance - Life / Accidental Death & Dismemberment (AD&D): Group Term Life Insurance is payable at a benefit valued at $10,000. The YMCA of Montclair covers all eligible employees and pays this benefit at 100%. Employees will have the option of increasing their coverage amount to 1 times their annual gross salary rounded up to the nearest thousand at their own expense. Dependent Life Insurance: Employees may cover their eligible dependents in the event of their death, for $5,000 at the rate of $2.00 per month per family unit. Premium must be paid by the covered employee. Enrollment is optional. Dependent Life covers Spouse (except in the event of divorce, legal separation or annulment), children from birth through age 25 (includes dependent stepchildren, legal guardianship, and legally adopted children), and registered domestic partners between same or opposite gender individuals. Children under the age of 14 days are excluded from Dependent Life coverage. Insurance Carrier: MetLife LONG TERM DISABILITY The YMCA of Montclair covers all eligible employees and pays this benefit at 100%. Insurance Carrier: Liberty Mutual RETIREMENT YMCA of Montclair participates in the National YMCA Retirement Fund for the benefit of all eligible staff members. As an employee of the YMCA of Montclair, upon completion of eligibility requirements, employees must participate in the YMCA Retirement Plan as a mandatory condition of employment. The Plan provides retirement and other benefits for YMCA employees and their designated beneficiaries. All of the staff at our YMCA becomes eligible to participate in the Plan when they are age 21 or older and have completed 1,000 hours of service within each of any 2 years, beginning with their date of hire or anniversary date at the YMCA organization. These two years are not required to be consecutive. All eligible employees must enroll in the Fund and it is a "condition of employment" at the YMCA of Montclair. There are only a few exceptions to this rule. These include employees who: Are hired at age 60 or over Are non-resident aliens with no US-source income Work at a YMCA newly joining the Fund Belong to a bargaining unit that has negotiated out of the plan The YMCA of Montclair pays the entire cost of each staff member’s participation in the Retirement Fund, an amount currently equal to ten percent (10%) of each individual staff member’s gross salary. Employees are immediately vested once enrolled. What is Vesting? Vesting is your right to receive the benefits the fund provides. MAKING VOLUNTARY ADDITIONAL CONTRIBUTIONS All participants can open Tax-deferred Account in addition to their Basic Account. Tax-deferred Account: Page 2 of 5 Contributions to this account reduce the employees’ federal income taxes the year they make them, giving the employees a current tax savings. Contributions are through payroll deduction. Since our YMCA participates in the YMCA Retirement Plan, any employee of our YMCA may immediately participate in the YMCA Retirement Fund Tax-Deferred Savings Plan by opening a 403(b) Smart Account regardless of their age, length of service or hours worked. They may also roll in money from an eligible employer plan or IRA. A more detailed description of the Eligibility and Enrollment rules can be found on the YMCA Retirement Fund’s website: www.yretirement.org. The YMCA reserves the right to amend its participation in the Plan at any time within the terms and conditions set by the YMCA, the Plan and the YMCA Retirement Fund. TIME OFF PROGAM VACATION DAYS Full Time Staff Vacation Full-time employees who have worked less than one year of continuous service are entitled to accrued time earned per month up to a maximum of ten days per calendar year for non-exempt staff and fifteen days per calendar year for exempt staff. A vacation “week” is the equivalent to five working days. The amount of vacation days earned increases with subsequent years of employment as follows. Vacation must be taken within the calendar year, between January 1st and December 31st and may not be carried over into the next calendar year. Years of service from another Y are credited. Full Time Exempt Staff Vacation Per Calendar Year: For employees with less than two (2) years of continuous service, three (3) weeks of vacation leave; After two (2) years of continuous service, four (4) weeks of vacation leave; After fifteen (15) years of continuous service, five (5) weeks of vacation leave. Full Time Non-Exempt Staff Vacation Per Calendar Year: For employment less than five (5) years of continuous service, two (2) weeks of vacation leave; After five (5) years of continuous service, three (3) weeks of vacation leave; After ten (10) years of continuous service, four (4) weeks of vacation leave; After twenty five (25) years of continuous service, five (5) weeks of vacation leave. Part Time Staff Vacation Per Calendar Year: Part Time employees who work 1,000 hours per calendar year continuously, become eligible for vacation hours after January 1st. The amount of vacation hours earned increases with subsequent years of employment as follows: Less than 5 years 20 HOURS 5-10 years 40 HOURS 11-24 years 60 HOURS 25 PLUS years 80 HOURS HOLIDAYS Eligible Employees: Regular Full-Time Employees Full-time employees of the YMCA of Montclair are eligible for the following paid holidays. Employees may be required to work on certain holidays that the facility is open but will be entitled to take another day in its place, at the approval of your supervisor. Page 3 of 5 New Year’s Day Martin Luther King Day President’s Day Good Friday Memorial Day Independence Day (July 4th) Labor Day Thanksgiving Day Christmas Day One Floater Day SICK DAYS Eligible Employees: Regular Full-Time Employees Full-time employees are entitled to six sick days per calendar year. Employees are eligible for sick leave only after the first six months of employment. Beginning of the seventh month of employment, and until December 31, employees are entitled to the entire six days. Sick time is not cumulative, sick time cannot be carried over into the next calendar year. Eligible Employees: Regular Part-Time Employees Refer to the attached Montclair Sick Leave Memo & Ordinance Effective as of March 4, 2015. Bereavement Full-time Exempt and Full-time Non-Exempt employees will receive up to three paid days, if necessary, for the death of an immediate family member. Members of the immediate family include spouses, parents, siblings, children, grandchildren and grandparents. AFLAC YMCA of Montclair offers benefits through AFLAC to all regular full-time employees and regular part time employees. Employees are responsible to pay 100% of the premium. Full –time employees may elect to enroll in the following AFLAC benefits: Insurance Plans Accident Protection Plan Short-Term Disability Cancer Protection / Critical Illness Dental Insurance Life Insurance Flexible Spending Accounts Unreimbursed Medical Dependent Day Care Please note, that for Full-time employee’s, payments for the plans with AFLAC will be deducted directly from payroll. CREDIT UNION MEMBERSHIP Employees of the YMCA of Montclair are eligible to utilize the many services provided at the Proponent Federal Credit Union. EMPLOYEE ASSISTANCE PROGRAM (EAP) YMCA of Montclair offers Employee Assistance Program (EAP) through a third party company called CONCERN to all full-time employees and employees who work 1000 hours in a calendar year. This confidential service can assist employees and their family members who may be experiencing personal or work related dilemmas. From time to time, many of us face financial, legal or personal issues that can lead to emotional distress, alcohol and substance abuse and martial or family stress. It is not unusual for these types of crises affect job performance. Finding solutions to challenges is the purpose of CONCERN EAP. The one to ten EAP sessions are provided at no cost to the employee or their family member. Experienced counselors will assist you in identifying and resolving the challenges. Services can be at six other convenient locations in the New Jersey. ALL CONTACT and THE CONCERN EAP RECORDS ARE COMPLETELY CONFIDENTIAL. No identifying information will be shared with YMCA of Montclair. Employees may obtain CONCERN brochures located at the employee information or Human Resources office.
