calculations in the study of cardiac diseases. (CCC 1-6; PGC 1, 3, 4, 5)
6.
Explain the prosthetic heart valve function to include evaluation of the need for cardiac
valve replacement, recognition of replacements and evaluation of structural and
hemodynamic effects of the prosthesis. (CCC 1, 2, 5-6; PGC 1, 3, 4, 5)
7.
Explain cardiac pathology, pathophysiology, and hemodynamics in the evaluation of
valvular (atrioventricular and semilunar valves) heart disease. (CCC 1-6; PGC 1, 3, 4, 5)
8.
Explain cardiac pathology, pathophysiology, and hemodynamics in the evaluation of
coronary heart disease, ischemic heart disease, left ventricular function, hypertensive
heart disease, and pulmonary hypertension. (CCC 1-6; PGC 1, 3, 4, 5)
Summative Evaluations
Please note: All courses must have a minimum of four summative evaluation measures, and those measures should
include a variety evaluation methods (e.g., test, oral presentation, group project). Please list all summative
evaluation measures. In addition to these summative measures, a variety of formative exercises/quizzes/other
assignments should be used to guide instruction and learning but do not need to be included on this template.
For each measure, please include a scope of the assignment: for example, if requiring a research paper, include the
range of required number of words and numbers and types of sources; for a test, include the types and numbers of
questions; for a presentation, include the minimum and maximum time, and so on.

LIST RELEVANT COURSE POLICIES (e.g., makeup policies for evaluation measures
and learning experiences agreed upon collegewide)
RETEST AND MAKE-UP EXAM POLICY
RETEST POLICY
The Program Retest Policy will comply with the procedures outlined in the Allied
Health/Science Department Student Policy Manual.
In order to provide students with reasonable opportunities for successful completion of the
Program, the following retest policy applies to students who have received an “F” in a technical
course or a laboratory competency:
A. At the end of a program major course, when the student obtained a grade of “F” for the
course or a lab competency, the student may be permitted to retest. This is at the discretion of
the instructor and will only occur if doing so gives the student the potential to pass the
course. If the student passes the retest, a grade of 75 will be awarded for the retest. It will be
weighted according to the course evaluation mechanism described in the Instructor course
outline.
B. Instructors will notify the Program Coordinator of any retests administered. This information
is maintained by the Program Coordinator in a secured location.
C. The following six restrictions apply:
1. Only two opportunities to retest will be permitted during a student’s tenure in the program
curriculum. Instructors will notify the Program Coordinator prior to administering a retest
to verify eligibility. All information pertaining to the student’s retest history will be in the
Program Coordinator’s files, and available for student review.
2. Only one retest in one course component will be permitted per semester regardless of the
number of courses taken that semester.
3. This policy is in place to assist a student with one difficult day of testing, therefore no
long-term and/or scheduled semester projects, group or take-home activities are included.
4. Only program major courses are included in this policy.
5. No opportunities to retest will be permitted when academic dishonesty occurs.
6. No opportunities to retest will be permitted when an “F” is the result of final grade point
deductions due to absences, tardiness and/or other behaviors that cannot be evaluated
using the retest format.
MAKE-UP EXAM POLICY
In the event that a student misses a written test/exam, or a lab competency, a make-up exam will
be given with the following restrictions:
1. Only one make-up will be granted per semester. Second occurrence will fall under the
Re-Test Policy.
2. Make-up exam will not be the same exam as those given during regularly scheduled class
time.
3. The student must contact the instructor immediately upon return from absence prior to
next scheduled class time, in order to schedule the make-up exam.
4. If illness should occur, a doctor’s note must be provided.
Pre-arranged test/exam or laboratory competency will need to be approved by the course
instructor and program coordinator. An Academic Honesty Statement form must be signed by
the student.
