JOB DESCRIPTION
SPR Level Trust Grade post in Oncology (Breast)
This post is a new position created as part of a reconfiguration between the NHS and
Imperial College departments of haematology. The creation of this new NHS post has
been enabled by the provision of new funding from both parties but is itself an NHS
funded post.
SECTION 1 TRUST INFORMATION
Imperial College Healthcare NHS Trust is the UK's first Academic Health Science Centre and
the largest trust in the country. The creation of the AHSC is a major advance for patient care,
clinical teaching and scientific invention and innovation.
The Trust, formed on October 1 2007, incorporates Hammersmith, Charing Cross, Queen
Charlotte's & Chelsea, St Mary's and Western Eye hospitals.

Hammersmith Hospital - A centre for respiratory medicine, rheumatology, cardiac
sciences, gastro - intestinal disease, cancer, particularly gastro - intestinal and HPB, lung
and solid organ cancers, and renal medicine.

Charing Cross Hospital - A centre for major trauma and serious illness, emergency
surgery, neuroscience, vascular services and musculo - skeletal.

Queen Charlotte’s & Chelsea Maternity Hospital - An advanced obstetrics and fertility
centre, women’s health and ambulatory paediatrics

St Mary’s Hospital - Comprising St Mary’s Hospital in Paddington and the Western Eye
Hospital in Marylebone - is world renowned, with a rich history and vibrant future. The Trust

IMPERIAL COLLEGE, LONDON

Study, Teaching and Training at Hammersmith Hospitals NHS Trust
The Trust encourages all consultants to participate fully in continuing professional development
to ensure that skills are updated in line with relevant medical developments. Consultants may
apply to the study leave budget for help with associated expenses.
The Clinical Skills Laboratory at Charing Cross Hospital provides an opportunity for skills
training in a controlled environment.
An advanced program of multi professional management and leadership training is available for
consultants and runs as short modules provided by external trainers on site.
The Trust has excellent library facilities on both sites, offering a full range of electronic and

SECTION 2 – ABOUT THE DIRECTORATE
The Directorate is managed jointly by the General Manager and Clinical Director for Cancer
Services and Clinical Haematology. Four Chiefs of Service report to the Clinical Director and are
responsible for the Divisions of Clinical Oncology, Haematology and Medical Oncology. The fourth
Chief of Service was appointed following the formation of the West London Gynaecological Cancer
Centre in 1994. The service is managed on two hospital sites with core staff providing services to
eight cancer units within the West London Cancer Network.

THE WORK OF THE DEPARTMENT
Oncology forms part of the Directorate of Cancer Services. Each Division within the Directorate
(Radiotherapy, Medical Oncology and Clinical Haematology) has an appointed Chief of Service.
The Chief of Service for Medical Oncology is Professor Jonathan Waxman and Prof Mark Glaser
for Clinical Oncology. The Directorate is led by Prof Robert Leonard as the Clinical Director for
Cancer Services and Clinical Haematology
The Directorate is managed by a General Management team with three Chiefs of Service reporting
to the Clinical Director and Director of Services. The service is managed on two hospital sites with
core staff providing services to scanner units within the West London Cancer Network.
In May 1999, the West London Cancer Network was formed with Hammersmith Hospitals NHS
Trust (HHT) as the main Cancer Centre for the network and the Mount Vernon Cancer Centre and
The Royal Marsden Hospital on the boundaries of the West London cancer Network. The Trust has
organised services in a tumour-site specialisation model for lung, colorectal, gynaecological, head
and neck, urology, CNS, skin and breast cancer, with clinical and research lead clinicians, and
multidisciplinary clinics. The aim of this approach is to improve the ‘patients journey’ from primary
care to the Trust and to provide specialist treatment and palliative care services. This will enable
standard operating procedures to be used and audit of agreed quality standards.
The Hammersmith Hospitals Trust has a large chemo-radiotherapy service which is provided on
both the Hammersmith and Charing Cross sites that treats the whole spectrum of malignancy, with
the exception of early childhood cancers. The chemo-radiotherapy service forms part of an
integrated cancer treatment, centre, which has all the appropriate support, services essential for
high quality cancer care. Admitting rights to Inpatient facilities will be provided on the Hammersmith
Hospital site. The service has 72 seven-day oncology beds and 20 five-day oncology beds with
day-care and outpatient facilities across the two hospital sites
The National Centre for Tropho-blastic Disease and the National Centre for germ cell tumours of
the ovary is provided by the Trust and was appointed as the national centre for gynaecological reconstructive surgery by NSCAG.
The Breast Cancer Unit
The Breast Cancer Unit at Charing Cross Hospital comprises 2 Consultant Medical Oncologists,
Professor RC Coombes and Dr J Stebbing. Both consultants alternate for care of in-patients on a
monthly basis. However, both consultants undertake the outpatient clinic work on a continual
basis. There is one SpR attached to the unit at present. This is a new post to provide enhanced
specialist registrar support for the clinical activities of the Breast Cancer Unit, working
predominantly with Dr J Stebbing. There is an SHO attached to the unit whilst the unit also has 2
Macmillan Nurses and 2 Research Nurses. There are 2 dedicated Secretaries.
The Medical Oncology Breast Cancer Unit runs outpatient clinics 4 days per week. The Tuesday
clinic for new patients is a joint clinic with Clinical Oncologists and Surgeons. The remaining clinics
are for patients on follow-up, receiving chemotherapy, on endocrine therapy and for patients in
clinical trials.
The unit has a large breast cancer biology research group of over 50 scientists, clinicians and
technical staff based in the Cyclotron Building at Hammersmith Hospital. Researchers are
studying the molecular mechanisms of oestrogen receptor activation, growth factor signalling in
breast cancer cells, purification and functional analysis of normal breast epithelial cells, and new
methods of silencing oestrogen regulated genes. In addition there is a substantial translational
research programme to develop new targeted therapies for breast cancer such as by inhibiting
histone deacetylases. The Clinical Trials Group based at Charing Cross Hospital is responsible for
a number of early (Phase I-II) clinical trials of novel vaccine therapies, signal transduction
inhibitors, and oncolytic viral therapies for breast cancer. In addition, the group runs a number of
large multicentre Phase 3 studies of cytotoxic and endocrine therapies for breast cancer. Thus,
there are ample research opportunities.

The Trust Doctor works closely with the 2 Clinical Oncologists who treat breast cancer and these
consultants are available in joint clinics on Tuesdays and Wednesdays. The surgeons specialising
in breast surgery are also available during these times (Mr D Sinnett, Mr R Cummins and Mr J
Grogono). In addition there is a weekly formal multidisciplinary team meeting attended by medical
oncologists, radiotherapists, surgeons, histopathologists, cytologists, radiologists, breast care
specialist nurses and Macmillan Nurses where all new breast cancer patients are discussed and a
management plan formulated.
There are opportunities for the SpR to participate in teaching of third and final year medical students
attached to the unit, MRCP teaching, and both clinical and didactic teaching for SHOs as well as
informal teaching sessions for nursing and allied staff.
While this post is not recognised for postgraduate training, it offers equivalent experience to SPR
posts in the Trust, and the post holder will have the opportunity to attend numerous educational
activities:
There is an excellent library (the Imperial College Wolfson library) on site which is open to Trust
staff. In addition, each clinical area has PCs with Internet access (including Medline) and access to
the Trust's Intranet.
During the year there are bi-monthly Audit meetings, weekly Research Seminars, fortnightly Journal
Clubs and fortnightly Work-in-Progress Seminars.
There are Grand Rounds at both the
Hammersmith Hospital and Charing Cross Hospital. There is also a fortnightly Registrar Teaching
Course that the consultants take in rotation.
On call commitments
The on-call requirement of the post is 1 in 6.
Administrative/secretarial support: The Post holder will be supported by a team of medical
secretaries.
Research Opportunities
Excellent research opportunities are available throughout the Trust and with Imperial College. A
number of funding opportunities are available from the Hammersmith Hospitals Trust Research
Committee (HHTRC) and the Trust has experience in obtaining funds from among others the
Department of Health, Medical Research Council, BBSRC, and the Wellcome Trust.

Job Plan
A formal job plan will be agreed between the appointee and Clinical Director, on behalf of the
Medical Director, this is not designed to be exhaustive.
The post holder and Clinical Director/Chief of Service will review the Job Plan annually in line with
the provisions in Schedule 3 of the Terms and Conditions. Either may propose amendment of the
job plan.
SECTION 3 - MAIN CONDITIONS OF SERVICE
The full-time hours of work are 40 hours.
Salary scale:
£29,580 - £44,581 per annum
London Weighting: £2,162 pa (non-resident)

Trust arrangements for adherence to the EU Working Time Directive are in place.
Appraisal
The post holder is expected to participate in the annual appraisal system in line with the Trust’s
appraisal guidelines. Individual appraisal interviews will be held annually and reports submitted to
the Medical Director and Medical Personnel.
Governance
The Trust has extensive arrangements for Clinical Governance. In particular, there is a focus on
clinical risk management with clinical incident reporting; clinical effectiveness including an extensive
audit program and processes to monitor clinical performance; clinical service development, including
the development of IT based protocols delivered to wards via the Intranet; and medical staff
development, including appraisal and continuing professional development. There is an extensive
network of Trust committees and structures to promote clinical governance activity.
Information Quality Assurance
As an employee of the Trust it is expected that you will take due diligence, care and follow Trust
requirements and instructions with regard to any information collected, recorded, processed or
handled by you during the course of your work.
All Clinical staff making entries into patient health records are required to follow the Trust standards
of record keeping
Intellectual Property Rights
The Trust will assume joint or outright ownership of all intellectual property arising from your work
unless it can be proven that the invention demonstrably arises outside your area of duty. The Trust
operates a reward to invention scheme, details of are available from the Human Resources
Department.
Freedom of Information
The postholder should be aware of the responsibility placed on employees under the Freedom of
Information Act 2000 and is responsible for helping to ensure that the Trust complies with the Act

Management of a Violent Crime

All employees have a responsibility to ensure that those persons using the Trust and its services are
as secure as possible.
Pension Scheme
The post is subject to deduction of superannuation contributions unless a member of staff opts out
of the Scheme. Deductions are 6% before pay is assessed for income tax. It is possible in certain
circumstances to pay additional contributions to enhance pensionable service; there is also an
Additional Voluntary Contribution (AVC) scheme linked to the NHS Pension Scheme.
SECTION 4 - CONDITIONS OF EMPLOYMENT

Statutory Medical Examination
All appointments are conditional upon prior health clearance by the Trust's Occupational Health
Service. Failure to provide continuing satisfactory evidence will be regarded as a breach of contract.
Criminal Records Bureau
Applicants for posts in the NHS are exempt from the Rehabilitation of Offenders Act 1974. All
