I, the parent or guardian of the above registered person, do hereby give permission to the CBM staff and attending medical personnel to give emergency
care to the above person which might include first aid, drugs, X-ray, surgery, or other medical procedures deemed necessary should I not be able to be
reached in a safe amount of time. I hereby give permission by my signature below for the dates indicated. I release Children’s Bible Ministries and Camp TaPa-Win-Go from any liability in connection with these activities. I also promise to hold Children’s Bible Ministries and Camp Ta-Pa-Win-Go harmless and
indemnify the camp in case of a lawsuit. Registration constitutes permission to use your student’s picture if it appears on camp video or still photographs.
