RESOLVE: The National Infertility Association 2010 Ask the Expert: A Family Building Expo November 7, 2010 General Giveaway Participation Rules • Registrants must check in at registration table at the conference to receive their giveaway tickets. • Each attendee will be given a ticket and can choose their door prize "category" (treatment or adoption consultation or others). Giveaway Rules for the IVF Cycle courtesy of South Florida Institute for Reproductive Medicine: • All registrants for the 2010 Family Building Conference have a chance to win a free IVF cycle, courtesy of South Florida Institute for Reproductive Medicine. Some restrictions apply: • Good for one free Cycle of IVF or $9,500 toward Donor Egg IVF Cycle • Patient must contact South Florida Institute for Reproductive Medicine within 30 days of award date (11/7/2010) • Certificate is non-transferable • IVF retrieval and transfer will be performed at the South Miami or Palm Beach office. • Initial consultation is required to confirm IVF is needed • If IVF is not required and other treatment options are utilized the $10,000 services value will be utilized for services rendered on site. All outside services must be paid by patient. IVF Door giveaway does NOT include: • Medications • The initial visit, ultrasounds or procedures needed before the actual IVF cycle. • Any testing performed outside the South Florida Institute for Reproductive Medicine • ·Any treatment/surgery costs of side affects or complications • ·Hospital charges • ·Pesa/Tesa/Tese/Testicular Mapping Services and associated facility fees • Urologist Charges • PGD Biopsy • Cryopreservation & Storage fees • Pregnancy blood work, ultrasounds and office visits • ·Donor Fees, Donor Prescreening or Donor Medications • ·Gestational Carrier fees *This is a comprehensive list of exclusions at this time, there maybe additional tests/programs that may be considered exclusions.
