PATIENT’S NAME:
MEDICATION: cyclobenzaprine hydrochloride (Brand names include Apo-Cyclobenzaprine, Flexeril,
and Novo-Cycloprine.)
WHAT IT'S USED FOR: Cyclobenzaprine hydrochloride is used as a muscle relaxant.
HOW TO TAKE IT
Take drug exactly as prescribed.
Don’t increase dosage or stop taking drug without consulting prescriber.
Don’t take drug within 14 days of drugs called MAO inhibitors. Otherwise, you may experience
extremely high blood pressure and severe seizures.
Don't take drug for more than 3 weeks.
WARNINGS & PRECAUTIONS!
Nofify prescriber immediately if chest pain occurs.
Avoid driving and other hazardous activities until you know how the drug affects your concentration
and alertness.
Tell your prescriber if you are pregnant.
SIDE EFFECTS
This drug may cause dizziness, drowsiness, confusion, fatigue, headache, nervousness, decreased
mental sharpness, irritability, weakness, disorientation, delusions, nerve problems, Bell’s palsy
(paralysis of one side of the face), abnormal walking, involuntary muscle movements, chest pain, low
blood pressure, dry mouth, blurred vision, bowel obstruction, constipation, indigestion, nausea,
unpleasant taste, tongue discoloration, mouth sores, swollen saliva glands, enlarged or leaking breasts,
decreased sexual ability, problems urinating, swelling of testicles, serious blood disorders, weight gain
or loss, high blood sugar, muscle pain, difficulty breathing, increased sensitivity to sun or sun lamps,
and loss of head hair. Notify prescriber of serious or bothersome symptoms.
INTERACTIONS
Cyclobenzaprine may interact with nervous system depressants, such as alcohol, sedatives, and
pain medications. Tell all prescribers which drugs and substances you're using.
Avoid taking over-the-counter drugs and herbs without consulting your prescriber.
Avoid alcohol use.
