PATIENT’S NAME:
MEDICATION: levofloxacin (Brand names are Iquix, Levaquin, and Quixin.)
WHAT IT’S USED FOR: Levofloxacin is used to treat various infections.
HOW TO TAKE IT
§ Take drug with or without food, but 2 hours before or after consuming milk, yogurt, multivitamins
containing zinc or iron, or antacids containing aluminum or magnesium.
§ Take drug exactly as prescribed.
§ Don’t exceed the recommended dosage and don’t share this drug with other people.
§ Compete the entire course of drug therapy as ordered, even after your symptoms improve.
§ Avoid touching the applicator tip to your eye, finger, or any other object when using the eyedrops.
WARNINGS & PRECAUTIONS!
§ Before taking this drug, tell your prescriber if you have heart, liver, or kidney disease.
§ Stop taking this drug and notify your prescriber immediately if you develop signs or symptoms of
hypersensitivity reaction (such as rash, hives, or other skin reactions) or severe diarrhea.
§ Avoid excessive exposure to sunlight or ultraviolet light.
§ Avoid driving and other potentially hazardous activities until you know how the drug affects your
concentration and alertness.
§ Tell your prescriber if you are pregnant or breastfeeding.
SIDE EFFECTS
This drug may cause headache, dizziness, insomnia, seizures, chest pain, low blood pressure, heart
palpitations, increased eye sensitivity to light, sinusitis, sore throat, nausea, vomiting, diarrhea (possibly
serious), constipation, stomach pain, heartburn, passing of gas, vaginal infection, serious blood
disorder, back pain, tendon rupture or inflammation, high or low blood sugar, increased skin sensitivity
to sunlight and ultraviolet light, change in taste, and allergic reactions (including serious skin
conditions). Notify your prescriber of all serious or bothersome symptoms.
INTERACTIONS
§ Levofloxacin may interact with other drugs, including antacids, anti-inflammatory drugs, iron, and
zinc. Tell all prescribers that you are taking it.
§ Don’t use herbs without consulting your prescriber.
STORAGE
§ Store drug at 59° to 86° F in a tightly closed container.
