Pharmacist to login and navigate to Pharmacy care organiser (PCO). Pharmacist Use the unverified orders column to identify medication which requires a clinical check. 2. Review the appropriate medication using the drop down list – this will require confirmation with the Drug Chart in Powerchart Pharmacist Select ACCEPT to denote this medication is clinically suitable for this patient. Use INTERVENE to complete an intervention form- (refer to Pharmacy Intervention QRG) Do not use the Other options in the menu ie Enquire,History, Product Assign or Verify NOTE – both reject and intervene can be applied to the drug if needed, one at a time. NOTE - If all orders are to be accepted at once can choose “Accept All” from the Task Menu at the top of the screen Click “Apply” 4. Once accepted the pestle and mortar symbol will be removed from the drug chart indicating that this is now clinically checked and the capsule icon should appear Pharmacist . Click “Submit” 5. To order the medication return to Powerchart by clicking on the Powerchart Icon on the top of the screen Pharmacist / Pharmacy Technician / Preregistration pharmacist Using requests/ care plans or the drug chart, review the medication and identify which medications need ordering. 6. Open ascribe and navigate to eMM Pharmacist/ eMM technician. 7. Search for patient using the search button, select the correct encounter. o Pharmacist / Pharmacy Technician / Preregistration pharmacist Pharmacist / Pharmacy Technician / Check patient demographics (ward, consultant, speciality, patient Preregistration status) – edit if incorrect or missing information pharmacist CHB_Prescribe_012_ EPR clinical checking and supply requests via eMM or paper V0.3 09/02/2017 o If no current episode use the lifetime episode - edit ward, consultant, speciality and patient status – if not the supply request will be lost 8. Check if the medication has previously been ordered and had “clinical check” on eMM (if so skip to step 10) Pharmacist / Pharmacy Technician / Preregistration pharmacist 9. If medication from other episodes yet to be reconciled: - Stop medication and copy to current PMR (quickest if a number of items to reconcile) o Highlight the drug/s – can do multiple lines o Select `STOP’. o Select a reason for stopping the drug (or ‘editing) o Select `save’ o The medication will now go into the patients `History’ PMR o Enter the ‘history’ tab and expand the episodes o Highlight the appropriate drug/s and select ‘COPY’ - Amending items – can also be used to reconcile medication from other episodes as alternative to stopping drugs if only one or two items (Amend an item if the drug is the same but the dose/ formulation has changed) o Highlight the drug o Select `AMEND’ Pharmacist / Pharmacy Technician / Preregistration pharmacist Add a prescription note on eMM to an item : - - 12. Search for the most appropriate prescription as prescribed on EPR, e.g. asp tab 75 Select the most appropriate prescription Highlight and select the template to add it to `orders selected’ Multiple items can be added to this list by continuing to add each new drug in the same way complete all the order information in eMM and click send. select `add note’. This is a yellow note pad and will follow the item through the system, and will be available to view later if the `history’ button is selected. A yellow note should only be used when all dispensing details cannot be communicated via the template or the supply request details. eg complicated reducing doses of Prednisolone or eye drops. Check the information is correct on the PMR against EPR then: CHB_Prescribe_012_ EPR clinical checking and supply requests via eMM or paper Pharmacist / V0.3 09/02/2017 Create a supply request - highlight each item on the PMR and select `New Supply Request’. o Additional information - which the dispenser needs to know but is not appropriate to add at the template stage e.g Patient going home at 2pm o Additional Details - unless indicated otherwise 28 day supply OR an ‘original pack’ (whichever is least quantity) will be dispensed. If a different amount is required, indicate here by using the drop down menu, or in the additional information box. o Priority - the item(s) will appear at the top of all work-lists Use this option when ordering items for TTO, critical medicine e.t.c Only use this option if really necessary, to help prioritise work appropriately. Pharmacy Technician / Preregistration pharmacist Endorse the drug chart on EPR by right clicking on the drug and “create admin note”. Process now ends on the ward. TRANSCRIPTION CHECK (must be undertaken by a different individual form the person who ordered the medication this can be a pharmacist or accuracy checking technician only) 13. The transcription check will need to take place for all medication which have been Pharmacist / ordered on eMM and not checked before. Accuracy checking technician 14. Open up the “clinical check required” section and select the appropriate ward / group you are working through. Pharmacist / Accuracy checking technician 15. Click on the medication in order of priority. And open up the patients PMR Pharmacist / Accuracy checking technician 16. Search for the patient on EPR using the NHS or MRN number. Pharmacist / Accuracy checking technician 17. Go to the requests and care plan section or drug chart and right click on the drug. Pharmacist / Select order information. Accuracy checking NOTE – each individual user can set up on requests and care plans to just technician straight to order information by right clicking on the drug and untick Disable order information hyperlink. Ensure the details match the order information on ascribe then , go to validation tab and ensure a pharmacist has clinically verified the drug. 19. If the medication order on eMM matches the prescribed medication on EPR and Pharmacist / has had a clinical check click on the “clinical check” button on eMM to allow this to Accuracy be dispensed. checking technician 20. If EMM is not available the supply request should be made on paper using Trust Approved Stationary- Transcribe the details across to the supply sheet, double check all the details and sign.
