Vivo Health Consumer Rights and Responsibilities If you are a Vivo Health Pharmacy consumer, you have certain rights and responsibilities. Patient’s Rights: 1. The right to considerate and respectful care. 2. The right to know about the philosophy and receive information about the patient management program. 3. The right to and is encouraged to obtain from pharmacists and other direct caregivers relevant, current, and understandable information concerning their medication therapy and treatment. 4. Is entitled to the opportunity to discuss and request information related to their specific drug therapy, the possible adverse side effects and drug interactions. 5. The right to make decisions about the plan of care prior to and during the course of treatment and to refuse a recommended treatment or plan of care. 6. The right to competent counseling from the pharmacist to help them understand their medications and use them correctly. 7. The right to identify the program’s staff members, including job title, and to speak with a staff member’s supervisor if requested. 8. The right to have personal health information shared with the patient management program only in accordance with state and federal law. 9. The right to receive administrative information regarding changes in, or termination of the patient management program. 10. The right to decline participation, revoke consent, or disenroll at any point in time. The collaborative nature of health care requires that patients, or their families, participate in their care. The effectiveness of patient care and patient satisfaction with the course of drug therapy will depend, in part, on the patient fulfilling certain responsibilities. Patient’s Responsibilities: 1. To submit any forms that are necessary to participate in the program, to the extent required by law. 2. To provide accurate clinical (history of medications, drug and food allergies, current medications, etc.) information and to notify the patient management program of any changes. 3. To notify their treating provider of their participation in the patient management program, if applicable. 4. To participate effectively in decision making, patients must be encouraged to take responsibility for requesting information or clarification about the drugs they are taking when they do not fully understand information and instructions. 5. Patients are responsible for following medication regimen as instructed by your physician. 6. Notify your provider and pharmacist if you choose to end your medication therapy. 7. To notify the provider and pharmacist of any changes in residence or insurance information. 8. To accept any financial responsibility not covered by insurance.
