Welcome
On behalf of the CSDA Board of Governors, Annual Scientific Session Council members,
Continuing Education Council members and CSDA Staff, it is our distinct pleasure to
welcome dentists and dental professionals to the Connecticut State Dental Association’s
151st Annual Charter Oak Dental Meeting. Being held at Mohegan Sun Resort Casino
May 11 – 13, we will proudly continue our tradition of hosting one of the finest dental
meetings in the Northeast.
We’ve got a 3-day event packed with education, networking opportunities and social events. This world-class
meeting was once again developed with the entire dental team in mind and includes 60 educational sessions on
a wide range of topics. Expect some tough choices ahead and be sure to use the Schedule-at-a-Glance to help
you plan your agenda. Our trade show features approximately 120 Exhibitors with lots of activity on the floor
throughout the event. Check out the “Exhibit Hall Happenings” section for more details. After spending your day
learning, we invite you to join us at our exciting evening parties! Our Opening Night party on Wednesday, May 11
will take place at The Lansdowne Pub which has proven to be the perfect venue for this event. Our Thursday night
dinner/dance party will have a western theme this year and traditionally sells out. Don’t miss the fun!
As you begin to make your plans, please keep in mind that early registration is strongly encouraged. This not only
saves you money but secures your spot in the courses you want to participate in – many of which have a limited
number of seats. Hotel rooms also tend to sell out so be sure to make your reservations and take advantage of the
special discounted room rate we’re offering. You’ll find complete details on everything you need to know as you
turn the pages.

Information
If you would like to request an additional copy of this
Program Guide, please call the CSDA office OR for
immediate access, please reference the printer-friendly

RECOMMENDED AUDIENCE CODES:
REGISTER EARLY!
In order to guarantee your spot and avoid higher fees, you are
encouraged to register by the early deadline of Friday, April 1st.
After this date, fees increase and space is more limited. Each year
several courses sell out so don’t risk missing out! Please refer to the
Registration Form in this book for detailed instructions on how to
register as well as important deadlines. You can register online or by
mail (no fax or phone registrations will be accepted).

CONFIRMATION:
Visit the “Annual Meeting” page of CSDA.com. Online registration
is fast, easy and secure and will provide you with an instant
confirmation of your registration. This option will also give you
the benefit of knowing if any of the courses or events you want to
register for are already sold-out allowing you the opportunity to
make alternate selections right then and there.
CANCELLATION POLICY:
All requests for registration cancellations/refunds must be made
in writing and received no later than Wednesday, April 13th. A $25
cancellation fee will be applied to requests received by this date.
After this deadline, no refunds will be granted.
TICKETS AND FEES:
All courses and events require pre-registration and a ticket for
admission. As long as your registration is received by Friday, April
29th, you will receive your badge, tickets, and other pertinent
material in advance via regular mail. This will allow you to bypass
the Registration Desk on-site. Be sure to carefully check your packet
immediately upon receipt and communicate any discrepancies to
ExpoTrac at: (401) 766-4142.
IMPORTANT:
Don’t forget to bring your credentials with you when you come to
the meeting! Badge and/or ticket replacement will result in a $15
charge. If tickets to any of the social functions are lost, they will
need to be re-purchased at the full ticket price. Additional fees are
refundable if original tickets are found/returned to the CSDA after
the meeting.
COURSE CODES:
“Schedule-at-a-Glance” is used to make your selections on the
Registration Form or when you are ready to register online. Please
note: All courses are lecture format unless stated otherwise in the
title and/or description (i.e. workshop or hands-on).
As you flip through the Program Guide, you’ll see that each course
includes a “Recommended Audience” notation. Here are the codes
to help you make choices that are best suited for you:

H = Hygienists
A = Assistants
O = Office Staff
L = Lab Technicians
ALL = All attendees
COURSE HANDOUTS:
Handout materials that correspond with 2016 Annual Meeting
courses will be available as downloads. You will receive instructions
on how and when you can begin to access 2016 Annual Meeting
handouts in your registration confirmation e-mail and in your
registration packet. If you wish to have them with you on-site, be
sure to print them and bring them with you.
COURSE LOCATIONS:
All Annual Meeting courses and events will be held in Mohegan
Sun’s Convention Center with the exception of the Opening Night
party at The Lansdowne Pub (casino restaurant). Attendee programs
will be available on-site at the Information Kiosks indicating meeting
room locations and times and each room will have the appropriate
signage just outside the entryways. This information is also available
in the mobile app!
CONTINUING EDUCATION CREDITS:
CE credit hours are noted with each course description throughout
this book. In order to obtain proof of attendance, you must write
the Verification Code given out at the end of the course on your
ticket and turn it in to a course Moderator as you exit. Please be
advised that course codes will NOT be given out at any other time.
CE CERTIFICATES - New This Year!
Approximately 30 days after the 2016 meeting, your CE certificate
will be accessible online via the CSDA Annual Meeting website.
Simply enter your last name and zip code in the appropriate fields.
This exciting new feature will allow you to have immediate access to
your CSDA Annual Meeting CE proof of completion record! (Please
note: This will be effective in 2016 and will not be retroactive.)

ADA CERP PROVIDER.
ADA CERP is a service of the American Dental Association to assist
dental professionals in identifying quality providers of continuing
dental education. ADA CERP does not approve or endorse
individual courses or instructors, nor does it imply acceptance
of credit hours by boards of dentistry. The CSDA’s Continuing
Education Programs are also accepted by the Academy of General
Dentistry (AGD) for Fellowship and Mastership credit. The current
term for both is 11/1/15 through 12/31/19.

FREE EXHIBIT ONLY PASS:
A special “Exhibit Only” pass will be available to CSDA member
dentists and their staff on Friday, May 13th to access the Exhibit Hall.
There will be approximately 120 Exhibitors showcasing products
and services designed to enhance both your professional and
personal life. Many of them offer “show-only” specials allowing you
to take advantage of additional savings on top of the convenience
of a “one-stop shopping” experience! No pre-registrations are
necessary. Please go to the on-site Registration Desk to take
advantage of this special offer.

required to secure your reservation. A limited number of suites
are available for only $50 more per night!
Hotel Cancellation Policy: You can cancel your reservation without
penalty until May 4, 2016. Cancellations made after May 4, 2016
will result in a loss of the one night deposit. There is also an early
departure fee of $75 so be sure to communicate any change in plans
to avoid extra fees.

Allied Membership in the CSDA!
What is Allied membership?
The CSDA has created a special membership category for
hygienists, assistants, administrative staff, lab owners and
technicians so they can join our association.
What are the special discounts available to
Allied Members?
Allied members can save up to $95 when attending CSDA
events:
• A $10 discount off the registration fee for individual
CSDA package Program Series courses or $55 off the
special three course package.
• A free registration for the CSDA Annual Meeting
(a $40 savings) if they sign up prior to April 1st or $20 off
their registration after the early bird deadline.
What are the other benets of Allied
Membership?
Allied members receive a complimentary subscription to
The Communicator, our quarterly news magazine. The CSDA
also offers more timely updates through our bi-weekly
e-newsletter.
How much are Allied membership dues?
At $35 the annual dues for Allied membership is incredibly inexpensive.

• Non-dentist staff employees of a licensed Connecticut dentist,
• Non-dentist staff persons employed in the dental field by a clinic, hospital,
educational institution, or insurance company based or situated in Connecticut, or
• Non-dentist laboratory owners or technicians based or situated in Connecticut that
provides laboratory services to a CSDA member
Allied membership terminates when the staff person ceases to be an active employee of a
licensed Connecticut dentist, or when they are no longer affiliated or employed by the entity
that qualifies them for membership.

Mandatory Continuing
Education For CT Dentists
The State of Connecticut mandates that licensed dentists must earn a minimum of 25 hours of CE
credit every two years commencing on the first date of license renewal. Of those 25 total hours,
one must cover the topic of “prescribing controlled substances and pain management” (effective
10/1/15), with four additional hours selected from the allowable mandatory topics. The remaining
20 hours can be any combination of elective credits offered by approved providers in areas related
to the licensee’s practice and reflect the professional needs of the licensee in order to meet the
health care needs of the licensee’s practice.

Certification in Dental Radiology is mandatory under Connecticut State Law (CT State Law HB7097). This is the minimum
level of competency expected of dental assistants in the State of Connecticut. As of January 1, 1995, only registered
Dental Hygienists and Certified Dental Assistants who have passed the Radiation Health and Safety Examination (RHS)
portion of the Dental Assisting National Boards can expose radiographs under the supervision of licensed dentists.
The RHS is a free-standing examination designed to determine competency in dental radiation health and safety for
personnel performing dental radiography procedures.

Joe Garzi, BA has a career in technology
that spans over twenty five years. He is
a Certified HIPAA Security Professional
and has been supporting the Technology
and Education of Dental Practices for over
15 years. Southridge Technology is the
Endorsed Information Technology Services
Provider for the CSDA and the Ninth District
Dental Association.
Matthew Gordon, JD is a founding
partner of Matthew Dallas Gordon, LLC.
Attorney Gordon is admitted to practice in
all Connecticut state and federal courts and
the U. S. Supreme Court. Attorney Gordon
is a board member of the CT Hispanic Bar
Association, CT Trial Lawyers Association and
he has tried numerous jury cases to verdict
and argued numerous cases on appeal.
Richard Hirschinger, DDS serves
as a Dental Advisor for Officite, the official
website and online marketing provider of
the Connecticut State Dental Association.
He actively practices in Beverly Hills, treating
patients with orofacial pain and obstructive
sleep apnea. He also serves as Clinic Director
at UCLA in the Orofacial Pain and Dental
Sleep Medicine Program.
Leslie Icenogle has been a Practice
Manager in the dental field for over three
decades. She is certified in medical/
dental coding and is a dental radiographer
specializing in CBCT imaging. Leslie offers
coaching, virtual training and lectures. She
is a member of NSA, SCN, ADMC, AAOMS,
ADOM, and ADIA.

Noel Brandon-Kelsch, RDHAP is
an international speaker, writer and RDH in
Alternative Practice. She is passionate about
oral health and has the uncanny ability to
motivate and enlighten through her unique
humor and cutting edge information. She is
known for taking tough subject matter and
presenting it in an interesting and thought
provoking way.
John Nadeau is the VP of Sleep Group
Solutions and resides in Gorham, Maine with
his wife and three children. A renowned
expert on acoustic airway imaging and
sleep testing, his lectures focus on the latest
regulations, protocols and strategies for
the successful dental management of sleep
related breathing disorders.
Uche Odiatu, DMD is a certified
personal trainer, Certified Lifestyle Coach,
and co-author of the books: “The Miracle
of Health” and “Fit for the LOVE of It!”. This
lifelong athlete, licensed ZUMBA instructor
and father of four has been an invited guest
on 400 radio and TV shows including ABC
20/20 and Canada AM.
Caitlin O’Rourke is a Regional Business
Development Officer and Vice President with
Bank of America Practice Solutions. Caitlin
specializes in working with dentists seeking
financing and banking products to acquire
existing dental practices throughout New
England. She enjoys working with both first
time and existing practice owners to provide
creative financing solutions.
Michael Ragan, DMD, JD, LLM is an
attorney who represents physicians, dentists,
and nurses in civil and administrative
litigation. Dr. Ragan practiced dentistry for
many years and also spent more than ten
years managing healthcare professional
liability claims in the insurance industry. Dr.
Ragan is a Director of Fortress Insurance
Company and Chairs the Claims Committee.

Kannan Rengasamy, DDS completed
his dental degree in Dental Science from
the Annamalai University, India. He is a
Diplomate of the American Board of Oral
and Maxillofacial Radiology. He is currently
the Clinical Director of the OMFR Clinic in
the University of Connecticut School of
Dental Medicine. His teaching activities are
directed to the residency and undergraduate
programs.
Debra Z. Sabatini, RDH is the founder
of the OralED Institute and President of the
non-profit; NationalCancerNetwork.org.
Debra is a tireless Pioneer Health Crusader,
educator and author dedicated to total
health initiatives and making oral cancer
history. Debra’s passion for continued
education has led her to deliver programs
to medical, dental and health organizations
around the world.
Mary Ellen Sholes, AAS graduated
from Boston University School of Dental
Medicine’s Dental Assistant Program. She
has been working at Boston University since
1974 and for the past 17 years has worked in
the Department of General Dentistry in the
Oral Diagnosis Radiology Division where she
instructs dental students in clinical radiology.
John Sobel, DDS graduated from SUNY
at Buffalo in 1988 and became Branemark
certified that same year. Since that time, Dr.
Sobel has restored well over 2,000 implants.
He has lectured at the Greater New York
Dental Meeting, Nassau and Suffolk Dental
Societies, The first District Dental Society and
numerous study clubs.
Aditya Tadinada, DDS, MDS is
a board certified Oral and Maxillofacial
Radiologist, currently Assistant professor in
Oral Health and Diagnostic Sciences and
Director of Student Research at the UCONN
School of Dental Medicine. He is the author
of several published papers in peer reviewed
journals, Chair of the Radiology section
in ADEA and currently president of AADR,
Connecticut Chapter.
Lori Trost, DMD is a sought-after dental
educator who brings everyday experience
and shares an approach to patient care that
is practical, innovative, and efficient. She
regularly contributes a variety of articles and
webinars, is a clinical evaluator, and has been
spotlighted as one of the “Top 25 Women in
Dentistry”.
Thomas Viola, R.Ph., C.C.P. serves
the dental professions as a clinical educator,
published writer and professional speaker.
He is a member of the faculty of nine
colleges as well as several national board
exam review courses. Tom is a contributor
to several dental professional journals with
pharmacology columns appearing regularly
in General Dentistry, AGD Impact, Access
Magazine and RDH Magazine.
Arthur Volker, DDS, MSEd is the
Chairman of the New Dentist Committee
and a member of the Continuing Education
Committee for the New York State Academy
of General Dentistry. Dr. Volker holds
diplomate status with the World Congress
of Minimally Invasive Dentistry as well
as Fellowships in the American College
of Dentists and the Academy of General
Dentistry.
Greg Whitmer is the Regional Business
Development Officer for the Practice
Solutions division of Bank of America,
covering all of New England. He works
specifically with dentists in the region to
assist with their financing needs including
anything from equipment and construction
financing to commercial real estate
purchases.
Martin Zase, DDS is an Accredited
Member, Accreditation Examiner and PastPresident of the AACD, a Master in the AGD,
and President of the New England Academy
of Cosmetic Dentistry. He is well-published
and is a reviewer for many dental magazines.
He has been honored with both the AGD’s
Lifelong Learning and Service Recognition
award and the AACD’s Lifetime Achievement
Award.
Robert Zavoski, MD, MPH is the
Medical Director in the Division of Health
Care Administration of Connecticut’s
Department of Social Services. Previously,
he served as the Medical Director for
Community Health Services, Inc., a federally
qualified community health center serving
Connecticut’s poorest communities, and
as the Medical Director of the Primary Care
Center at CT Children’s Medical Center.
