Familial Cancer Service
The Familial Cancer Service provides genetic counselling and risk assessments for individuals
with a personal and/or family history of cancer. We provide surveillance recommendations and
may offer genetic testing where appropriate.

Eligibility Criteria












Blood relative of a known carrier of a high risk cancer predisposition gene (e.g. BRCA1,
BRCA2)
An individual or a close relative diagnosed with breast and/or ovarian cancer before 40
years of age
Triple negative breast cancer (TNBC) under 50 years of age
Male breast cancer
Two or more family members (on the same side of the family) diagnosed with breast or
ovarian cancer at any age
Personal or family history of breast or ovarian cancer and Ashkenazi Jewish ethnicity
Serous ovarian cancer at any age
An individual or a close relative diagnosed with bowel cancer before 50 years of age
Abnormal Mismatch Repair Immunohistochemistry (MMR-IHC) on tumour
Two or more family members (on the same side of the family) diagnosed with bowel,
uterine or ovarian cancer at any age
An individual with multiple bowel polyps (10 or more)
Several family members having developed the same type of cancer or diagnosed with
cancers of different types
Exclusions


A solitary case of breast cancer over 50 years of age or bowel cancer over 55 years.
Moderate risk breast and bowel cancer families may be managed by GP. Please refer to
guidelines (breast cancer: FRA-BOC, bowel cancer: cancervic.org.au).
Clinic Information

1 x Friday afternoon per month (Frankston Hospital)
Level 3 Consulting Suites Frankston Hospital (enter via main entrance)
• Additional appointments are available at Monash Medical Centre (MMC) Clayton and
Moorabbin clinics (both held weekly), as well as an outreach clinic in Moe held every 3
months

Minimum Referral Information Required
(Please note, cannot be processed if minimum information is missing)
Referral must be addressed to Dr Marion Harris, Monash Health Familial Cancer Centre
Referring practitioner name, provider number and signature.
Date of referral
Period for which referral is valid (if different to standard referral validity)
Patient’s name, address, date of birth, Medicare number and phone number.
Clinical details and reason for referral.
Relevant medical history including cancer histopathology
Relevant family history
Medications
Results of all recent and relevant investigations
