SUBJECT: KtO/ Striving Readers Assessment Data Reporting Dear School Representative, The Pennsylvania Department of Education will be contracting with SAS, Inc. to provide enhanced reporting to determine progress for districts and students participating in the Keystones to Opportunity (KtO)/ Striving Readers grant. This enhanced reporting will enable districts to determine whether or not the targeted students are making progress. SAS, Inc. provides the annual statistical analysis for the Pennsylvania Value-Added Assessment System reporting for Pennsylvania’s 500 school districts. All districts participating in the KtO/ Striving Readers grant will be required to submit assessment data on all targeted students to SAS, Inc. Districts that are administering DIBELS Next, and are utilizing the web-based member center (University of Oregon) provided by the assessment vendor, may complete the permission form below for the Department identified entities to extract the data. Districts that are administering a required assessment other than DIBELS Next will submit data via the approved data submission Excel files. The Department appreciates your cooperation as we move forward with this exciting opportunity to provide KtO/ Striving Readers grantee districts with additional data to help them make informed-decisions. Please complete the following and return NO LATER THAN September 23, 2013 to: Barbara Smith, Lancaster-Lebanon Intermediate Unit 13, 1020 New Holland Avenue, Lancaster, PA 17601 School District:__________________________________, _______ hereby grants permission to the Department identified entities to have access to data stored in this district’s account on the DIBELS Data System (http://dibels.uoregon.edu/). Access and reports will be provided to the Department identified agencies on a project level, district level, school level, and individual student level. Access encompasses district, including all schools, information stored in the Data System, including student names and ID numbers, demographic information, scores, and outcome measure scores. However, access will be password-protected and confidential. ______ does not grant permission to the Department identified entities. I understand that by not providing consent, my district will be responsible for submitting assessment data to SAS, Inc., as per the KtO Data Submission Requirements.
