Narrative Report of Research, Teaching and Clinical Contributions I am a Board Certified Pediatrician and Anesthesiologist with subspecialty boards in Critical Care, who has had a career long interest in promoting safety in the care of children. This research focus has been in the form of: 1) clinical trials so as to develop pediatric drug labelling, 2) research on equipment specific to infants and children, 3) safe sedation practices by nonanesthesiologists who sedate children for procedures, and 4) pediatric sedation outcomes. I have taught residents and fellows since 1977, I continue to provide direct patient care, and I have served nationally and internationally on multiple committees for the American Society of Anesthesiologists, the American Academy of Pediatrics, as well as the World Federation for the Societies of Anaesthesiologists. I co-authored the first Pediatric Sedation Guidelines published by the American Academy of Pediatrics in 1985 and 4 further revisions. I am also serving as a member of the Task Force for the Sedation Guidelines by Non-Anesthesiologists of the American Society of Anesthesiologists. I am best known nationally and internationally for my pediatric anesthesia textbook, my work on sedation guidelines and sedation outcomes, and for the development of international pediatric anesthesia fellowships for anesthesiologists who desire to become pediatric anesthesia specialists but who come from a medically disadvantaged country. These fellowships developed for the World Federation for the Societies of Anaesthesiologists were designed to allow hands on training in a country that spoke their native language. Additional fellowships are in process in Singapore, Hong Kong, and Nairobi, Kenya. I was the main driver in fundraising and expanding the number of these programs with contributions obtained from national and international societies as well as from industry. Research. The majority of my academic career has focused on transfusion therapy, clinical pharmacology, monitoring, and patient safety. I published two papers that have been pivotal in terms of describing adverse events associated with sedation administered to children by nonanesthesiologists. I am the lead author of the fifth revision of the sedation guidelines for children, a joint task force from the American Academy of Pediatrics and the American Academy of Pediatric Dentistry; this most recent pediatric sedation guideline of the American Academy of Pediatrics will be published in 2016. In 1993, I was recruited to the Children’s Memorial Hospital at Northwestern University as Vice Chairman and Director of Research. I was a mentor to all the junior attendings as well as the 2 to 5 fellows per year. I helped them through the process of study design, the IRB submission, carrying out the study, analyzing the data, writing the papers and, finally, getting them accepted for publication. This resulted in a significant increase in clinical research output, all of which was funded from projects that I was able to garner from various pharmaceutical and equipment manufacturers. From 1979 to the present, I have had approximately 31 industry-funded studies and 38 internally funded studies. All of these studies involved pediatric specific pharmaceutical or equipment studies often leading to improved prescribing information for children. Teaching. I have been actively involved in teaching residents, fellows, and fully trained anesthesiologists, pediatricians and dental specialists my entire academic career. My primary role as Director of Research while at Northwestern was to teach and mentor the junior attendings so that they would develop research skills to gain their promotion. I am proud to say that as a direct result of my mentoring several of my junior attendings gained promotion: one was recently promoted to Professor and is the new Chair at the Laurie Children’s Hospital at Northwestern University Medical School in Chicago, three were promoted to Associate Professor, and several others to Assistant Professor. I am the primary editor of a textbook on Pediatric Anesthesiology with 53 chapters contributed by 110 authors from 6 continents. This textbook has in the past been translated into Spanish, Italian, and Japanese and it is the lead pediatric anesthesia textbook used by the American Society of Anesthesiologists as a resource. We are in the process of editing the 6th edition which should be published in 2017. I was an editor of a handbook on Pediatric Pain Management and Sedation. I also developed a Reference Guide for Principles of Pediatric Sedation under the sponsorship of Tufts University Medical School Department of Continuing Medical Education with an associated educational video. I also co-edited a Manual on Pediatric Anesthesia (sixth edition) with Dr. Jerry Lerman and Dr. David Steward which was recently translated into Japanese and the 7th edition will be available in late 2016. I also was a consultant editor of a handbook ―Understanding Paediatric Anaesthesia, 2nd Edition, Editor Rebecca Jacob MD, Vellore India. This handbook has also been translated into French, Spanish, Chinese and is being translated into Arabic. All versions are being distributed at no cost throughout the world including French speaking Africa, English speaking sub-Saharan Africa, India, and Asia by the World Federation of Societies of Anesthesiologists. I also have authored 79 published book chapters with at least 6 others in process. Clinical Contributions. At the Massachusetts General Hospital, The Shriner’s Burns Institute, and the Massachusetts Eye and Ear Infirmary I provide direct patient care as well as supervisory care for neurosurgery, general pediatrics, ophthalmology, ENT, orthopedics and urologic surgery, as well as organ transplantation. These activities also involve anesthesiology services for invasive radiology, magnetic resonance imaging, computerized axial tomography and sedation for gastroenterology and oncology procedures. B. Report of Clinical Activities My current clinical practice is based in the main operating rooms of the Massachusetts General Hospital, the Shriner’s Burn Institute, as well as off-site locations within the hospital, such as invasive radiology, proton beam therapy, and magnetic resonance imaging. I provide anesthesia directly or in a supervisory role for the full array of pediatric anesthesiology cases. The caseload on the days that I am in the operating room may vary from 1 to as many as 10 procedures, with complexity varying from simple procedures such as dressing changes to the more complex congenital neonatal surgical problems (emergent or elective). As a clinician/educator/researcher, much of my research has been on the clinical level and this research has involved monitoring of patients. My research has examined pulse oximetry, capnography, critical events that occurred during the sedation process, the function of pediatric ventilators, the comparison of different generations of pulse oximeters and how they reflect what is truly happening in the patient. I have conducted many pharmacokinetic and pharmacodynamic studies, both industry-sponsored and internally sponsored, and I have participated for a number of years as a member of the Anesthesiology Expert Committee for the United States Pharmacopeia as their first pediatric anesthesiology representative. As part of that process, I had an extreme interest in developing drug studies for pediatric patients so that an appropriate label for the age and size patient may be developed. This activity was intertwined with my activities with the Committee on Drugs for the American Academy of Pediatrics for which I was the anesthesiology liaison for 10 years. This committee met with the Food and Drug Administration 3 times a year and we, as a committee, helped craft much of the language that was eventually incorporated into the Food and Drug Administration Modernization Act (FDAMA) that was passed in 1998, renewed in 1999, and revised in 2001 (The Best Pharmaceuticals Act and the Pediatric Drug Equity Act and the Pediatric Research Equity Act). This series of legislation Charles Coté 35 Charles J Coté MD curriculum vitae – May 16, 2016 resulted in the Food and Drug Administration being empowered to require pediatric drug trials for all new drug applications (NDA’s) and to request studies of all previously approved and still patent protected medications. This essentially resulted in an explosion of pediatric pharmacology studies from a low of about 30 between 1991 and 1999, to now well over 700 since FDAMA was instituted in 1998. This legislation was renewed in November 2007. I have been the beneficiary of a number of these drugs trials, and as an expert have been consulted by a variety of companies to develop pediatric clinical trials for drugs that are commonly used in pediatric anesthesia. My expertise in medical devices, in terms of monitoring, has led to my being appointed to an FDA Advisory Committee (currently resigned). My interest in sedation-related adverse outcomes and sedation monitoring guidelines for children has been my greatest accomplishment and this has resulted in my recognition as a national and international expert. I have participated in a number of sedation guideline committees and I was chair of a Joint Task Force on Sedation for the American Academy of Pediatrics and the American Academy of Pediatric Dentistry. From this followed many invited national lectures on this topic as well as international invitations in Switzerland, Chile, Japan, England, Canada, Scotland, Argentina, Italy, and Africa.
