Professor Reyad obtained his medical degree with honor, subsequently his MSc and MD degrees in Pediatrics and Neonatology from Ain Shams University in Cairo, and Benha University, Egypt. He has been practicing Neonatology for more than thirty years; establishing and designing many NICUs, selecting their equipments and training their neonatologists and nursing staff. He is an international member and certified Neonatal Resuscitation Instructor from AAP, as well as EPLS certified. Professor Reyad is a founder and board member of the Egyptian Society for Neonatal and Preterm Care, the first Neonatology Society in Egypt. He is also a member of the Advisory Committee of the Egyptian Neonatal Guidelines. Professor Reyad’s main areas of interest are Pediatrics and Neonatology. He also has a strong background and special interest in CME in asthma, vaccines and antibiotics. He has been conducting many professional presentations and training sessions in Egypt and other Middle East countries. 10 - 13 February 2016 , Hurghada - City , Egypt 7 International Congress of The Egyptian Society for Neonatal and Preterm Care ESNPC Prof. Ahmed Saad ElDin El-Beleidy Cairo University - Dr. Ahmed El-Beleidy was born in October 1955. - Dr. El-Beleidy has got his M.Sc of Pediatrics in May 1982, then his MD of Pediatrics 4 years later, in May 1986. Both degrees were taken from Cairo University. - In 1996, Dr. El-Beleidy was appointed as Professor of Pediatrics, he became the head of the Pediatric ICU of Cairo University New Children’s Hospital in 2011. - In August 2013 Professor El-Beleidy was elected as the head of the Pediatric Department in Cairo University. He was also senior Consultant Neonatologist and chief of Neonatology at St. Helier University Hospital. He was also the Director of Research and Development at Epsom & St Helier University Hospitals Trust. He has been principal regional examiner for the Royal College of Paediatrics and Child Health and visiting professor and examiner to four universities. Prior to returning back to the UK in 1990, professor Haque held the first chair of neonatal medicine in the Middle East as Professor of Neonatal Medicine at the University of Riyadh. He has a total of 40 years of Paediatrics and neonatology experience working in Asia, Arabia, North America, Canada and the UK. He has published 160 peer-reviewed papers, six chapters and has authored two small books. He has done pioneering research work in immuno-modulation and other aspects of neonatal sepsis. He was one of the principal investigators of the International Neonatal Immunoglobulin Study (INIS). As an invited expert on neonatal sepsis he has recently published consensus definitions of neonatal blood stream infections in paediatric critical care medicine. He has regularly supervised Masters and PhD students and is a reviewer for twelve international journals. He is visiting professor to four universities. He is married with three daughters and five grandchildren. None of his daughters have been foolish enough to follow the career path their father has taken. Haque FRCP (Lond, Edin, Ire), FRCPCH, MBA, DCH, DTM&H Professor of Neonatal Medicine Abstracts According to Lectures 54 ESNPC 2016 10 - 13 February 2016 , Hurghada - City , Egypt Despite the development of neonatal intensive care and use of modern generation antibiotics mortality and morbidity from neonatal sepsis remains high, particularly in the very low birth weight infants. In this group it remains around 18 to 20% in the most advanced countries. In fact in the extremely low birth weight infants it has slowly crept upwards. There are many reasons for this continued rate of mortality and morbidity. These include immunological immaturity of the preterm infant, our inability and uncertainty in diagnosing signs and symptoms of sepsis early. Plus the lack of highly sensitive and specific biomarkers and our own inadequate management practices. Added to these difficulties is the increasing incidence of microbial resistance and lack of development and availability of newer antibiotics. Given the above, and our better understanding of the simultaneous activation of pro-inflammatory and anti-inflammatory mediators in sepsis, researchers, clinicians have attempted to develop adjuvant therapies exchange transfusion, use of haemopoeitic stimulating factors but what has been most fruitful has been the use of various immune-modulatory therapies that work by regaining the deranged immunological balance is sepsis. The recent recognition of a new class of immunologic disorders known as autoinflammatory diseases has changed the way we look at pathologic inflammation and allowed for a better understanding of the mechanisms regulating the innate immune system. Although patients with these disorders can present with classic rheumatologic symptoms, including inflammation Involving the joints, skin, muscles, and eyes, the underlying inflammatory mechanisms do not involve autoantibodies or antigen-specific T cells. Some patients also display episodic or precipitated symptoms, including urticaria and conjunctivitis, similar to those seen in allergic patients, but there is no evidence of IgE-mediated inflammation. Recurrent fever is a common feature of both autoinflammatory disorders and infections caused by immune deficiency. Autoinflammatory disorders are primarily driven by the innate immune system and include rare inherited syndromes and more common and genetically complex inflammatory diseases. Understanding of the cause of inherited autoinflammatory syndromes has led to effective targeted therapy with significant clinical effect. However, Colchicine is still the standard of care for FMF and is used widely around the world. IL-1 is one of the central mediators of autoinflammatory diseases and is the target of 3 approved biologic therapies, including Anakinra, Rilonacept, and Canakinumab. This disease category has now expanded from the original rare, inherited, recurrent fever syndromes to include much more common diseases. Translational research concerning the single gene autoinflammatory disorders has not only improved our knowledge of pathophysiology but has rapidly led to improved diagnosis and therapy of the patients with these diseases and holds promise to improve future treatment. This presentation with briefly highlight pathogenesis, clinical picture, diagnosis and management of some of those disorders 56 10 - 13 February 2016 , Hurghada - City , Egypt International Congress of The Egyptian Society for Neonatal and Preterm Care ESNPC RSV prophylaxis in preterm babies Medhet Ezzat Consultant Neonatologist and Honorary senior clinical Lecturer, Aberdeen University Bronchiolitis is the most common acute lower respiratory tract infection of infancy and causes approximately 3% of all infants to be admitted to hospital 1. The hospitalisation rate for bronchiolitis is higher in certain, including those born prematurely 1. Prophylaxis with a humanised monoclonal antibody (Ig) to the virus associated with most cases of bronchiolitis (Respiratory Syncytial Virus, RSV) is licensed in the UK for use in high risk infants. The approval was based on the IMPACT study2 which demonstrated that RSV-Ig reduced the relative risk for a hospitalisation with bronchiolitis by 55% among ‘‘high risk’’ infants and by 78% among infants born prematurely without bronchopulmonary dysplasia (BPD) when compared to placebo. The absolute risk for a bronchiolitis admission associated with Palivizumab treatment is 5%1. Despite this promising intervention against a common and sometimes serious infection, a consensus statement and systematic review recommend that RSV-Ig prophylaxis should be confined to ex-premature infants with BPD 1 3. Additionally, the UK Department of Health states that prophylaxis is indicated in some Infants born up to 34 weeks gestation (https://www.wp.dh.gov.uk/immunisation/files/2012/07/chap-27a-dh_130131.pdf) .The question “should Palivizumab prophylaxis be given to ex-premature infants without BPD?” remains unanswered although in practice treatment is not given in this scenario. As part of an annual audit of bronchiolitis admissions to our unit we have collected details of infants admitted between the winters 2004/5 and 2010/114. Here we determine the number of prematurely born infants who did not develop BPD who would require Palivizumab prophylaxis in order to prevent a single bronchiolitis hospitalisation. The mosquito-borne Zika virus, which has been linked to brain damage in thousands of babies in Brazil, is likely to spread to all countries. WHO director general, Margaret Chan called Zika an “extraordinary event” that needed a coordinated response. She advised pregnant women to consider delaying travel to areas affected by Zika and to seek advice from their physician if they are living in areas affected by Zika, as well as protect themselves against mosquito bites The incubation period (the time from exposure to symptoms) for Zika virus disease is not known, but is likely to be a few days to a week. The most common symptoms of Zika include fever, skin rashes, conjunctivitis, muscle and joint pain, malaise, and headache These symptoms are usually mild and last for 2-7 days. Zika virus usually remains in the blood of an infected person for a few days but it can be found longer in some people. Prevention can be done by using insect repellent-, wearing clothes (preferably light-coloured) that cover as much of the body as possible using physical barriers such as screens, closed doors and windows; and It is also important to empty, clean or cover containers that can hold water such as buckets or flower pots , so that places where mosquitoes can breed are removed. The CDC now is advising pregnant women to consider delaying travel to Barbados, Bolivia, Ecuador, Guadeloupe, Saint Martin, Guyana, Cape Verde and Samoa. Recent work has shown that neonates with septic shock demonstrate reduced expression of genes representing key pathways of innate and adaptive immunity. They demonstrate a predominantly down-regulated transcriptome that may account for why more neonates develop septic shock than any other age group. Whilst, genomic studies have improved our understanding of the pathophysiology of septic shock in the new born, similar advances have not been made in diagnosing septic shock early which is the hallmark of good management leading to better outcome. While all of us know the importance of encouraging breastfeeding since birth, the importance of adequate complementary feeding of infants is increasingly acknowledged. Complementary feeding is defined as the process starting when breast milk alone is no longer sufficient to meet the nutritional requirements of infants, and therefore other foods and liquids are needed, along with breast milk. The transition from exclusive breastfeeding to family foods – referred to as complementary feeding – typically covers the period from 6 - 24 months of age. This quest is of greatest relevance in neonatal medicine as neonates as a group have the highest rate of infection compared to any other group save elderly immune-compromised patients. Finding such biomarkers is also important in neonatology because for every one neonate with sepsis thirty babies are prescribed antibiotics leading to antimicrobial resistance. Conventional biomarkers like white cell count; C-Reactive protein, pro-calcitonin etc, are useful but have either poor sensitivity or specificity. With advancement in biochemical and genomic research, more sophisticated classes of biomarkers are being identified. Researchers are seeking ‘novel’ and or specific biomarkers that not only diagnose an individual disease such as sepsis but also monitor disease activity and give an indication of prognosis. With increasing advancement in technology it is hoped that in the very near future biomarkers will likely be highly specific, universally available and universally clinically applicable.
