RE: Open Enrollment for Benefits Elections
Open enrollment for the Tuscarora Intermediate Unit Health, Vision, and Dental Insurances is May 16
through June 16, 2016. This is your opportunity to make changes to your level of coverage. You will
not be able to make changes outside of this time frame unless there are qualifying events as defined by
IRS regulations.
There will be two medical plans offered effective July 1, 2016 through June 30, 2017.
Plan #1 – PPO Plan – there will be no changes in the plan design from the 2015-16 plan.
Plan #2 – Qualified High Deductible Health Plan with a Health Savings Account (HSA) with a
$1,300 annual deductible for individual coverage and a $2,600 annual deductible for family tier
coverage. No coinsurance or copays will be applicable upon meeting the deductible. TIU will
contribute $325 for individual and $650 for family tier coverage to an HSA account.
Prescription Drug Coverage – The Prescription Drug Coverage will be “carved out” from
Highmark and will be directly with Express Scripts administered through Rx Benefits. You will
receive a new ID card specifically for Prescription Drug Coverage under either medical plan selected.
Your ID card and packet of information should be received by June 24, 2016.
A Summary of Benefits and Coverage for each plan has been included for your review.
Premium Share Amounts will remain the same for both Plans being offered. The Premium Share Rate
schedule has been included as an attachment to this memo. Premium Share Amounts for Head
Start/Early Head Start Program will be provided to staff by May 23, 2016 pending budget allocations.
ID Cards: Any member making medical plan changes will receive a new ID card from Highmark. Cards
will be mailed to your home address. You can download a paper copy of your ID card at
www.highmarkbcbs.com. If you do not make any plan changes, keep your current Highmark ID card.
ID cards will only be issued to new members and those making an enrollment change.
Get to know the Highmark member portal! Log on at www.highmarkbcbs.com and click in the green
box. Here you’ll find a wealth of information, including your personal claims and cost history,
estimation tools for costs of services, including MyCare Cost Estimator. See also the Preventive
Schedule, a reference tool for planning your family’s preventive care.

Dental Coverage – No changes to the level of coverage and rates will remain the same.
Vision Coverage - There will be no changes to the vision coverage. The rates for vision will remain the
same.
As a reminder, children to age 26 can be covered under a parent’s health insurance plan regardless of
their status. However this change was not required for vision or dental insurance and has not been
incorporated into the plans.
If you wish to make changes at this time, please forward a TIU Form 45A - Employee Request for
Change in Insurance Coverage form to Cindy Felisberto no later than the close of business on June 17,
2016. All changes will be effective July 1, 2016. If you are not planning on making any changes, you
will not need to submit a change form.
