Jackson Memorial Hospital Bactrim Intravenous Attainment Procedure Bactrim IV remains on national backorder with no release date available • Teva pharmaceuticals, LLC is the sole manufacturer • Medication may be obtained on a patient specific basis through a compassionate use protocol with Teva Compassionate Use Protocol 1. Upon initial receipt of an IV Bactrim order: a. Check “patient note” section in PharmNet to see if current approval has been granted by ID, a CHP, or ASP b. If there is no approval, please have the PRESCRIBER contact ASP 2. Upon confirmed approval of the medication: a. Please enter the order as written for a duration of 72 hours 3. Teva should be contacted by the prescriber for additional medication supply a. By phone (either number): i. 1.800.729.9991 option #3 ii. 1.800.545.8800 b. By email: i. To: Cynthia Weidman - [email protected] ii. CC: Kathryn James - [email protected] c. The patient case should be presented, identifying the need as critical d. The amount of drug required (total dose needed) should be calculated i. Dosed using the trimethoprim component 1. 15-20 mg/kg IV daily, divided q8-12h ii. Actual body weight should be used iii. Please round doses to the nearest mL iv.
