(Mycoplasma ) › Rest, nutrition, fluids (SIADH), antipyretic, Anticonvulsant › Acute psychosis : haloperidol A text book of Infectious disease I, สมาคมโรคติดเชื้อแห่งประเทศไทย Depends the virulence of the virus and on variables associated with the patient's health status, such as extremes of age, immune status, and preexisting neurologic conditions Rabies, EEE, JE, and untreated HSE have high rates of mortality and severe morbidity, including mental retardation, hemiplegia, and seizures Encephalitis, Emedicine.com The mortality rate in treated HSE averages 20% and is correlated with mental status changes at time of first dose of acyclovir Approximately 40% of survivors have minor-to-major learning disabilities, memory impairment, neuropsychiatric abnormalities, epilepsy, fine-motorcontrol deficits, and dysarthria Encephalitis, Emedicine.com Controlling mosquitoes : Dengue Animal vaccination : Rabies virus Human vaccination : JEV Failure to consider HSE in the diagnosis or to initiate administration of acyclovir in a timely fashion Current practice in common Infectious disease 2005 อาการและอาการแสดงมีหลากหลาย และคล ้ายคลึงกับ viral encephalitis ชนิดอืน่ ๆ จนไม่สามารถนาอาการทางคลินิก มาแยกเชื้อได้ ยกเว้น olfactory หรือ gustatory hallucination ซึง่ มักเกิดช่วงสัน้ ๆ ก่อนทีผ่ ูป้ ่ วยจะมี ความรูส้ กึ ตัวเปลีย่ นแปลงไป Profile : Viral encephalitis › Lymphocyte : 20 – 200/cm3 +/- PMN › Protein 50 -200 mg/dl › Normal CSF sugar (5% low) › In systematic review : RBC or Xanthochromia มีหรือไม่มไี ม่ได้ช่วยในการวินิจฉัยแยกจาก encephalitis ชนิดอืน่ เลย เนื่องจาก มีโอกาสพบได้พอๆ กัน Viral encephalitis › Rabies & Dengue encephalitis : most no pleocytosis or few < 30/ cm3 › JE encephalitis : 30 – 360 Lymphocyte predominant, rarely > 1000 › โดยส่วนใหญ่ LP ซา้ ในวันหลังๆ จะปกติ HSV encephalitis › Biopsy 4/98 no pleocytosis › Most in immunocompromised & LP in first day of illness >>> PCR + › ดังนัน้ ถ้าอาการเหมือน HSVE แต่ไม่พบ cell ไม่ rule out เหมือนกับ PCR negative เช่นกัน ถ้า CT brain หลังจากมีอาการแล ้วหลายวัน > 5 วัน จะคัดค้านการ วินิจฉัย HSVE ถ้า CT brain วันแรกอาจไม่พบได้ ทา MRI จะพบความผิดปกติได้เร็ว กว่า และควรเป็ น imaging of choice ในรายทีเ่ ป็ นมาในวันแรกๆ CSF PCR for HSV รักษา 5-7 วันแล ้ว ยังมีโอกาส PCR ขึ้นเชื้อได้ บางรายได้ acyclovir 10 -12 วันไปแล ้วก็ยงั PCR ขึ้นได้ แต่ สัมพันธ์ กับความพิการทางสมองทีส่ ูงขึ้น โดยเฉพาะถ้ารักษาครบ course แล ้วยัง PCR ขึ้นแนะให้ acyclovir ต่อจน PCR negative หรือ อาการดีข้นึ ถ้าส่งเลยเก็บได้ในอุณหภูมหิ อ้ งได้ > 24 ชม เก็บ -20oC หรือ 4oC ก็ได้ Confirm test ส่งตรวจวินิจฉัยในสัปดาห์ทส่ี องของโรคขึ้นไป เพราะ อาจจะได้ผลลบในกรณีม ี อาการ > 10 วัน หรือในกรณีได้ acyclovir > 5-7 วัน
