bleeding, took enzymes for 18 months, then hemicolectomy, now widespread disease to chest
and ascites. Daughter reading book on Tort Law. Cachectic.Taking “effusion pills” and heavy
enzymes but has a large one.
Patricia Strehlow 52 year landscaper from Seattle who has 96% ownership of her gardens

primary (we found it in pancreas of PET), progressed on treatment. Says “I know I’ll die but
I don’t know when.” Bone lesion produces foot drop- he says its from not enough exercise.

herbals, refused chemotherapy, radiation, now with bilateral breast disease and bony mets, EPR
positive never too hormone blockers..
Robert Bazil 78 year old mantle cell lymphoma post chemotherapy with progression brought by
Son from Huntington Beach, where they bowl together. Son;s average is over 200; pt has very
strong grip.Well dressed, but pt is emaciated and covered with bruises.

transferase inhibitor, stable, with wife MD, 3 yo boy; reads the bible to his son, takes it kiterally,
not “what someone says it says” Feels that if he goes for transplant there is no road back.
Not unstable enough to demonstrates failure in terms of fever, transfusions, though not in
cytogenetic remission any more

with lung and liver lesions 1 year later, treated with 5FU and leucovorin without result, here for

Robert Bees- 78 year old man with metastatic colon to liver, drowsy, but wife very motivated and
Business-like. Proves to have Her2-neu positive, but has risiing BUN and received Herpceptiin.
Prognosis very poor.

relapsed with positive cytology but –ve imaging, received Carbotax and BCG without benefit, now
here to find where residual disease is. –ve cystos.

explained to her that Rituxan alone (she is CD20 +ve) would not offer cure like Rituxan plus
CHOP. Has heart murmur, needs echo.

twice, wioth no diagnosis “probably desmoid” sent to Dr Rudolph, who refused to biopysy due to
Size, then to Dr Gard, plastic surgeon, who also felt too much danger, then referred to MDA
which refused to take him as he had no cancer diagnosis.
Dorsi Crowe 81 yo Canadian ladt who received CHOP, should start on Rituxan at home. Afraid
of more chemo but only chance for cure with high grade lymphoma They did not offer it. ?Cost?
Protacio Nicolas Phillipine man with with hormone refractory metastatic prostate cancer,
cachectic , with family. Will need Nizoral and Taxotere. Severe bony pain on Duragesic

incontinence. Was told at VA it was malnutition, but chart shows severe multilevel lumbar
spondylosis Without evidence of cancer; started on decadron for actute cord compression, will
need platinum plus Taxotere for his disease.

brainmets and CVA. On hospice care. Two handsome sons flew in, one from Boston and One

Patient with bone marrow lymphoma ifrom wisconsin, has pancytopenia, refused standard
chemotherapy, and received laetrile iv from a clinic in Mexico. Quite stable, will need PB, plus
Rituxan, VP. Wanted to avoid “killer” therapy.

Jane Iwashita 61 screen mammo gram shows spiculated lesions, 3/3 biopsy positive,
“contaminated” Lymph nodes positive. How could it be contaminated when its so far away??

prostatectomy. Remission for 2 years, then boney mets and rising PSA, lung lesions, now on
hormonal blockers. Local MDS want to start him on Taxotere, but not clear he has failed to
respond to blockers.
David Vick 61 yo physician (Internist) from Missouri diagnosed in 1999 with Ca prostate
Gleason 8 withg bladder involvement, never had surgery, just biopsies, had progressed on
hormonal blockade, hepatotoxic reaction to Nizoral, not presents fur further treatment. No bone
involvement.
Needs PET scan for restaging.

Madeline Pocorba 54 yo psychotherpist with inflammtory breast cancer, inflammarotyr, EPR –ve,
Her2-neu negative, + nodes per CT scan, presents with recurrent disease after treatment with low
dose Gemzar and “Genoxal” (Cytoxan) in Mexico. Needs restaging and AC-T. Has blurred
vision. ? CNS mets.
Milan Reznik 41 Yo Swedish/Czeck man with adenoid cystic Ca of the lung. Cannot get a PET
scan at home! Brought own Oxalaplatin. No local MD will treat with Cisplatin/VP16 or XRT. We
will Make arrangements here. Post 3 thoracotomies. Needs an opinon that he would benefit
From Radiation with Platinol because he cannot get that provided in Sweden without such a
statement.

adenopathy, but PET scan shows no disease outside bladder. For PB alone, told needs repeat
imaging, probably will need XRT/chemo/Surgery if progresses on CT. Basically refusing

mass in the left lung- biopsy showed large cell CL. Surgery- mediastinal disease despite –ve PET
scan there. Severe reactions to Taxol and Iressa. Recurrent malignant pleural effusion.
Prob Taxotere or Navelbine. Chemically sensitive. IIIB

synchronous ca's of the colon (sigmoid and cecal). Complaints about HMOs and Kaiser.
They would not do PET scan.

Has stem cell, followed by ESHAP, relapsed again, but does not want to go off chemotherapy
To receive ATNP. Knows that eventually things stop working.
Mark Roby 50 year old P A from Michigan , holistic orientation, developed RUQ pain and fever,
found to have liver mass with lung mets, hemangioendothelioma (rare) Offered Interferon and
chemotherapy at MDA, but very apprehensive about only one treatment form, and local MDs in
Detroit say no PBs or ATNP. Prob will try Interferon first. Wants to work with MDs who will "halp
save his life." Attractive, well dressed single man. Tearful.
Peter Phillips 70 yo had radical prostatectomy in 1995, relapsed in 2001 with bone mets, severe
allergic reaction to Zometa, now on Casodex. Psa always low, now from 3 pretreatment to 0.
From England, lives in Las Vegas.

Refused LAD, XRT. Left breast dsstroyed, nodal mets clinically, retyracted. Posiitve bone survey,
eicence of pulmonary mets.
Gergory Gadomski 39 yo man with history of Crohn's disease with synchronous colorectal
cancers to liver, presented with obstruction. Ramdonized to arm of study where he did not receive
Xeloda, but infusional 5-FU. Very pale, double colostomy/ileostomy..
Hanna Bates 15 year old girl from Idaho, developed left foot pain. X ray was compatbile with
fracture, but biopsy showed Ewings sarcoma/neuroectodermal tumor. On Cytoxan+Mesna+VCR.
Staging is negative.
Dennis Pratt 45 yo man from Dallas with history of migraines, preented with headahce
And vomiting, scan showed R frontal GBM. Surgery 6 months ago, raprid recurrence despite
Gliadel, another surgery 1 months ago with no residual disease. Recommended Temodar plus
XRT, but wants to avoid XRT. Refused ANTP due to no measureable disease.

on Gemzar. Metstatic disease to liver. Had such N + V had to leave.

recurrent disease in lungs, brain, and opposite kidney.
Loreatta Buehler 67 year old lady with adenoCa of the lung, post Carbo-Tax, now with brain mets,
No bone mets, for Temodar and whole brain radiation.
Sheila Oldham 38 yo woman had breast cancer 9 years ago, ? EP status, Her 2-neu 2+, got FAC
Pre- and post-op with reconstruction, remission for 4 years, relapsed same breast, got Taxotere
pre and post remission for 3 years, then bone mets. Wants not to lose her hair. FAC caused
some hair loss, but with Taxotere she lost her eyebrows
John E Lang 56 yo man from Lockport, NY seen at Roswell park, diagosed with Ca prostate and
Recommended prostatectomy, found on pre-op to have lung mass- bilateral hilar nodes, +ve
Medistinoscopy, so strated on hormonal blockers and recommended chemotherapy.
May benefit from Taxoere for both malinancies.

jaw after chiropractic adjustment, found to have metastatic nasopharyngeal carcinoma. Had
chemoreadiation with resolution of primary and neck noses, now presents with hot PET showing
cervical spine and RUL uptake for biopsy. Prob PB alone.

to have Metastatic ovarian cancer to liver with large ovarian mass. For Carbo-Tax.

Recurrent laryngeal involvement

To have brain mets/cerebellar and frontal due to NSSSCL, had cranial irradiation, presents for
evaluation for possible VP16 plus PB

Node +ve gastric CA to liver. Postop –ve CT scan, for PET scan, recommend chemotherapy.

mole on his neck; had recurrence after surgery,m then fialed XRT post radical neck dissection,
then failed temodar + thalidomide. And failed vaccines, now tih brain and liver mets. Possible
Interferon.
Sharon Carnes 42 yo lady with metastatic carcinoma of lung primary to the neck
Failed chemotherapy and on XRT to the mediastimum. Suffers from hoarsnesss fue to
Recurrent laryhngeal n involvement. For PB, continue XRT.
Margaret Edmonsdon 66 yo lady from Seattle with olfactory neuroblastoma per biopsy, during
workup found to have adenocarcinoma of the lung, resected with apparent cure. Does not want
chemotherapy or radiation or surgery (radical facial reconstruction, but it is radiosensitive and
chemosensitive). Suggesting oral VP16 plus platinum neoadjuvantly plus pB.

Radioresistant as well.

Then recurrence 1991, another surgery, ? radiation, then neuroogical symptoms 2001
Found to have bony disease, left optic n treated with cpinal and cranial radiation. For Lupron,
Xeloda + Taxotere.

Ben through all available chemotherapy for colon cancer, offered methotrexate. .
Candace Wolf 54 yo woman with EPR positive breast cancer, now wiith leptomeningeal disease
despite hormoanl blockers, does NOT want radiation to cord, Taxol, etc. Offered Temodar plus
Xeloda. Son is attentive, wants her not to have to suffer any damage to the immune system.
Joihn Valentine 42 yo presented with painless jaundice, Ca head of the pancreas, had stent, now
