Tracey Phillips RN Manager, Clinical and Quality Systems Department of Quality & Safety Safety Reporting Overview • What is rL Online Safety Reporting? – A web based safety reporting product to document incidents, events, near-misses, and hazards Who will using the system to reporting events, incidents, and hazards? – Everyone Safety Reporting Overview • An incident or event – Is an unexpected, unintended occurrence that results in injury or has the potential to cause harm • An error – omission/ commission – Is the failure of a planned action to be completed as intended or use of a wrong plan to achieve an aim • A near-miss – an opportunity for learning – Is an intercepted error, a situation that could have resulted in an accident, injury, or illness but did not either by chance or timely intervention • A hazard is – A source or situation with the potential for harm or damage Goal of Safety Reporting • Identify, study, and document events and vulnerabilities • Determine, through this learning, prevention strategies and risk reductions to avoid adverse event Accessing the System •Click on Start and open Internet Explorer from the computer’s program menus •Click on the General Tab on the CareGroup homepage •Under Employee Resources click on rL Online Incident Reporting •The system can also be accessed via the Quality and Safety department page Accessing the System •From the rL Online Incident Reporting page, click on the link for the Incident Reporting - Live Database Logging In • Click on the User ID field and enter your Meditech username •Click on the Password field and enter your Meditech password. • Click on Login to enter the system Entering a new incident • From the Safety Reporting home screen, click on the button Report Incident to open a new file •Choose Open incomplete file to complete and submit a prior file saved as incomplete •Add a follow up note allows you to add supplemental information to a submitted event Completing a file • The system guides you through a series of screens which contain drop down boxes and text fields to complete •The field description is to the left of the field, fields in red are mandatory and must be completed to submit the file •The field ? Button to the right provides a description of the General Information • Complete the Classification: the type of person that was affected by the event •Choose the General Incident Types from the drop down list •Click on the Next button at the button right of screen to progress Complete Person Information •Complete Person Information. Patients can be looked up on Meditech. Type in Lastname or MR# and click the search button •Reminder: All the Red Fields must be completed to submit the file •Click on the Next button Event Details: Time and Place • Complete the incident date. Click on the Calendar icon and choose the date •Document details: Time, Site, Department and Location fields •Location is the Location where the incident occurred •Click on the Next button Specific Details • Complete Specific Incident Type •Document the Reported Incident Severity •Brief Factual Description requires a brief text narrative of the incident •Click on the Next button Reported Incident/ Event Data Collection/ Impact Assessment Unknown Safety Near Miss Environment (did not reach patient) Hazard Patient Event (reached patient) Harm to patient No harm to patient Level 0 Near Miss/ Potential Harm/ Damage Level 1 No Harm/ Damage Level 2 Temporary Minor Harm/ Damage Level 3 Major Injury/ Functional Impairment Level 4 Permanent Impairment/ Disability/ Death Severity Levels Level 0-Near Miss/Potential Harm/Damage Circumstances had potential to cause harm/damage. Follow-up may be required. Did not reach patient. Level 1-No Harm/Damage Event/error results in no harm/damage. Follow-up may be required. Level 2-Temporary Minor Harm/Damage Event/error results in temporary minor harm/damage. Additional monitoring or follow-up required. Level 3-Major Injury/ Functional Impairment Event/error results in major injury and/or functional impairment. Additional monitoring, prolonged stay and extensive follow-up required. Level 4-Permanent Impairment/ Disability/ Death Event/error results in permanent functional impairment, disability and or death. Extensive follow-up and investigation required. Notifications •Click the Add button to document who was notified at the time of the event • Complete the details of the notification. You may add multiple notifications •Notifications are for documentation and do not replace notifying the relevant parties at the time of an incident •Click on the Next button Event Summary and Submission •The Event Summary allows you to view the file prior to submission • Use the Modify button to change any details •Click on the Submit Incident to complete the process Training Resources Training Page: The Safety reporting Training Page contains: • Training materials (including printable step-by-step guides) • Online training tutorial on submitting an incident • Training version of the Online Reporting system • Help and support information
