Request for Proposals (RFP)
Noah’s Animal House @ The Shade Tree is seeking discounted veterinary hospital care
and support for medical requirements needed by the animals under the shelter and care of
our 501c3 non-profit organization. This would include animals that require medical
attention that cannot be performed by the on-site Operations Manager & Licensed Vet
Tech or Licensed Veterinary Independent Contractor.
I. BUDGET & COST ESTIMATES
Estimated Annualized Cost: $8000-$10,000
Annualized outside veterinary expense based on historical medical needs while serving
approximately 130 animals per year.
II. TERMS AND CONDITIONS
a) Seeking Veterinary Hospital discounted services and partnership effective February 7, 2015
b) Parties submitting separate proposals may not discuss pricing information or they will be
ineligible to bid on the project.
d) Bidder status: Bidder must disclose any relevant conflicts of interest and/or pending lawsuits.
e) Noah’s Animal House will reserve the right to obtain veterinary services elsewhere when there
is an emergency or when it is more cost effective.
III. BACKGROUND OF COMPANY
a) The Shade Tree has a staff comprised of 44 full-time 5 on call and 13 part-time
b) Noah’s Animal House is staffed with one Licensed Veterinary Technician and 3 part time
kennel assistants.
IV. Scope of Services, but not limited to:
a) Provide basic veterinary care on site that cannot be performed by the on-site Operations
Manager and Licensed Veterinary Technician or Licensed Veterinary independent contractor.
b) With prior authorization from Noah’s Animal House, perform advanced veterinary care off site.
Examples blood work, emergency surgery, routine surgery, hospitalization.
c) Provide veterinary recommendations based on physical exam, including medications,
procedures, treatment.
d) Vaccination of animals on site.
e) Provide humane euthanasia for Noah’s Animal House for animals suffering as determined by a
veterinarian.
f) Provide monthly billing for services provided.
g) Supply a veterinary license number for Noah’s Animal House to order supplies as needed.
h) Be available via email, text or phone for medical advice.
i) Weekly visits to Noah’s Animal House.
V. Fees
Provide a flat monthly fee for any and all services outlined in the Scope of Services.
VI. Qualifications
Minimum Qualifications: Provide evidence of Veterinary License through the Nevada Medical
Board.
Desired Qualifications: experience with shelter medicine and operations.
VII. Proposal Timetable
Request for Proposal (RFP) may be obtained beginning January 11, 2015 from George Cotton at

VIII. Proposal Submission
In order to be considered by the Selection Committee, a respondent’s proposal must be received
by Noah’s Animal House 1 West Owens, North Las Vegas, NV 89030 or via email at
[email protected] no later than 5pm February 1, 2016.
A signed original and three (3) copies of the signed original proposal must be submitted. The
original must be signed by the respondent. Confirmation of receipt of Proposal to RFP will be
made at the time of receipt upon delivery in person, or if received via U.S mail, a confirmation of
receipt will be sent to the bidder via facsimile.
Proposals must be labeled on the outside of the package to clearly indicate that they are in
response to the RFP for Noah’s Animal House.
IX. FORMAT FOR PROPOSALS
Each proposal must contain at minimum, the following:
a) A one page transmittal letter expressing interest in the services
b) A document(s) that provide the following information:
1) Evidence of licensure that the facility and veterinarians are licensed through the
Nevada Veterinary Medical Board
2) The address, phone number and facsimile number of the Respondent
3) Any additional information relevant to Respondent’s qualifications for the position or
pertinent to the selection criteria identified in Section XI below
4) Professional references
X. Selection Committee
The evaluation of the proposal will be performed by the Selection Committee composted of the
Director of the Shade Tree or her designee, Staci Alonzo and Noah’s Animal Houses Manager
and one Director from The Shade Tree.
XI. Selection Criteria
The Selection Committee shall consider, at minimum, the following:





Specialized experience, qualification and competence of the hospital or respondent, its
principals and key staff
The capacity and capability of the hospital/respondent to perform the work with required
time limitations.
Past record and performance of the hospital/respondent with respect to scheduled
compliance, cost control and quality of work
Commitment, connection and proximity of the hospital/respondent to Noah’s Animal
House
Other items that arise as the result of the proposal or interview
XII. Miscellaneous
a) Noah’s Animal House is seeking to enter into a one (1) year contract, with the reserving
the right to terminate on thirty (30) days’ notice, with or without cause, by submitting
written notice. Noah’s Animal House reserves the right to renew the contract for
additional annual one year terms for up to three years.
b) It is understood and agreed that the contract is subject to appropriation of funds
c) All materials submitted in accordance with this RFP will become and remain the property
of Noah’s Animal House and The Shade Tree and will not be returned. All proposals shall
be considered public records, but may be deemed and treated as closed or exempt by
Noah’s Animal House or The Shade Tree. Noah’s Animal House and The Shade Tree
cannot guarantee confidentiality of any materials during the evaluation process or at any
other time. Thus, proposals and communications exchanged in response to this RFP
should be assumed to subject to public disclosure.
d) Noah’s Animal House reserves the right to reject any and all bids or seek additional bids.
e) Noah’s Animal House reserves the right to interview one or more of respondents.
f) Noah’s Animal House reserves the right to negotiate a contract with one or more
respondents.
g) Noah’s Animal House reserves the right to waive any and/or all non-material irregularities
pertaining to the submission of the proposal.
