Upon receipt of this form and full payment WLIC will ship items selected below with all forms/instruction sheets to use for DNA samples. Please specify the type of livestock tag you are ordering, be specific on species. Livestock must be identified in the exhibitor’s name or in the immediate family member names. If animals are identified in the immediate family name only one form needs to be submitted along with the correct fees, however all siblings’ names MUST appear on the top of the form. Cousins are not considered immediate family and may not co-identify the same animal. There is no limit to the number of animals which may be identified by an exhibitor. The RFID tag listed on the identification forms MUST be the RFID ear tag in the animal’s ear at the fair’s check-in. DNA Request Forms MUST include payment and complete information or they will be placed on Hold. Checks returned to WLIC from a financial institution for any reason will be charged a $20 service fee or fee the bank charges for overdraft fee. Return this completed request form with payment (check, money order or cashier’s check) to WLIC. All DNA hair sample envelopes with the white DNA forms must be returned to WLIC by your fairs deadline date. Please contact WLIC with questions at [email protected] or 888.808.1910. Contact your Fair Office for deadline dates. All DNA Hair sample submits must be sent/postmarked by the deadline dates set by WI State Fair to WLIC.
