PORT OF VIRGINIA EMPLOYEE BENEFITS
The Local Choice Health Benefits Program
Health benefits are administered by four Plan Administrators. Anthem Blue Cross and Blue Shield is the administrator
for covered Medical Services; ValueOptions, Inc. for covered Behavioral Health Services and the Employee
Assistance Program (EAP); Medco Health Solutions, Inc. for prescription drugs; and Delta Dental Plan of Virginia for
routine dental services. The following gives a brief overview of plan services administered by these companies under
The Local Choice Health Benefits Program.
Medical, Anthem Blue Cross and Blue Shield
The VPA provides employees a choice of 2 health insurance plans administered by Anthem Blue Cross and Blue
Shield. Both the VPA and the employee contribute to the premium cost of the plan.
Key Advantage Expanded Plan Care is provided by primary care physicians, specialty care providers and facilities.
Referrals are not needed. Higher copayments apply for specialist and facility visits. Networks are the Anthem PPO
network in Virginia and the BlueCard® PPO and BlueCard Worldwide® networks outside Virginia. Care may be
received outside these networks, however, a separate plan year out-of-pocket network deductible and out-of-pocket
expense limit applies. Covered services include doctor visits, hospital, surgical, dental, orthodontia, chiropractic,
prescription drugs, vision, employee assistance, mental health and substance abuse.
High Deductible Health Plan (HDHP) Health Savings Account (HSA) compatible plan. Employer contribution into
the HSA is $1,000 for single and $2,250 for family. Care is provided by medical health care providers and facilities
and behavioral health care providers and facilities. Networks are the Anthem PPO network in Virginia and the
BlueCard® PPO and BlueCard Worldwide® networks outside Virginia. There is no coverage for care outside these
networks except in an emergency.
Dental, Delta Dental Plan of Virginia
Dental insurance is administered by Delta Dental Plan of Virginia. When employees receive their dental care from a
DeltaPremier dentist, they will not be responsible for any charges that exceed Delta’s allowance for the covered
services received. Therefore, out-of-pocket expenses are limited to coinsurance and any amounts that exceed annual
or lifetime maximums. Should an employee decide to receive dental care for a dentist who is not a member of the
DeltaPremier network, they will still receive benefits from the plan, but the employee share of the cost will likely be
higher than if they receive care from a network dentist.
Employee Assistance, Mental Health and Substance Abuse, ValueOptions, Inc.
Benefits are provided through ValueOptions, Inc., a company that specializes in managing the treatment for mental
health and substance abuse conditions. The ValueOptions’ confidential help line is staffed 24 hours a day, seven
days a week. As another benefit to employees and immediate family members, ValueOptions provides an Employee
Assistance Program (EAP). The EAP offers help with relationship problems, work-related stress and financial
problems among others. This free service is available 24 hours a day, 365 days a year.
Prescription Drugs, Medco Health Solutions, Inc.
Retail Pharmacy – This is a mandatory generic outpatient prescription drug program. If a generic equivalent exists
for a brand name drug, you have two choices. You may request the generic and pay only the copayment. Or you or
your physician may request a brand name drug and you will be responsible for the applicable copayment plus the
difference between the allowable charge for the generic equivalent and the brand name drug when you use a
participating pharmacy. At a non-participating pharmacy you pay total retail price for the drug and then file a
Prescription Drug Direct Reimbursement Claim Form. Reimbursement is limited to the allowable charge for the
generic drug minus your copayment.
Home Delivery Pharmacy – Medco By Mail home delivery service is a convenient, cost-effective way to obtain up to
a 90-day supply of medications you take routinely.

Disability
Short Term The VPA provides Short Term Disability (STD) coverage administered by Lincoln Financial Group (LFG).
The plan provides 60% of base pay up to 180 days.
Long Term After STD of 180 days, the VPA provides Long Term Disability (LTD) coverage administered by Lincoln
Financial Group (LFG). The plan provides 60% of base pay and may continue to age 65.

Life Insurance
The VPA provides basic employee life insurance coverage administered by Minnesota Life equal to two times the
employee’s annual salary rounded up to the nearest thousand. VPA employees also have the option to purchase
supplemental life for themselves, their spouse and their children.

Deferred Compensation Plan (457b/401a)
The VPA offers automatic enrollment in the Deferred Compensation Plan which allows the employee to defer a
portion of their income until their retirement years, providing supplemental retirement income. The amounts deferred
are pre-tax dollars and, therefore, reduce current taxable income. Employee is vested immediately in their own
contributions to the 457b Plan. The VPA matches 50c on every dollar that employee contributes up to 6% of their
compensation. Employee is vested in VPA match (401a Plan) after 5 years of service. Detailed plan and account
information and the flexibility to make changes are available online or through an automated voice response system.
Retirement
The VPA has an Employee Pension Plan, paid by the VPA, to provide eligible employees with a monthly pension
benefit upon retirement based on a formula that takes into account pay and years of service. Employees are vested
in the plan after completing 5 years of service with the VPA.
Flexible Spending Account
The VPA offers employees the opportunity to contribute, through payroll deduction, to a flexible spending account
administered by Flexible Benefit Administrators, Inc. Employees may choose a Health Care Reimbursement account
or a Dependant Care Reimbursement account or both. Plan contributions are pre-tax.
Supplemental Insurance (employee paid)
The following plans are available through AFLAC:
Personal Cancer Protection Plan (a cancer expense insurance policy)
Personal Accident Expense Plan (accident expense insurance policy)
Personal Long-Term Care Plan (a long-term care insurance policy)
Life Assurance (a choice of whole life & 10 year renewable term life policies including associated riders)
Additionally, supplemental life insurance policies are available for employee, spouse and children through Minnesota
Life.
Employee Wellness Program
The VPA recognizes the importance of employee health and well-being by encouraging employees to engage in
physical fitness activities that are offered by various health facilities. Full-time and part-time employees of the VPA
are eligible to receive a $25.00 reimbursement, minus taxes, for their monthly membership fee to a health club
provided the employee makes at least 6 visits to the health club each month. Proof of health club membership and
proof of attendance is required.
Vacation
VPA employees earn Paid Time Off (PTO) each year based on years of service. PTO is allocated each January 1 st,
based on the table below. For new employees, PTO is accrued based on hire date after completion of the first three
months of the probationary period.
