Refer to the map on the back cover for counties served. We have selected a variety of resources that can be utilized by middle and high school students, their teachers, and students enrolled in professional health care programs. The exchange of materials requires a signed agreement between the school and the MidMO AHEC. One signed agreement is valid throughout the current school year. Our Mission: Mid-MO serves as a catalyst for recruitment, retention, and local support of healthcare providers in the rural and medically underserved counties of central Missouri resulting in healthier, stronger communities. If you are interested in the use of any of the material listed in this catalog, please complete the agreement and reservation forms and send to us at: Mid-Missouri AHEC 1110 West 11th Street Rolla, MO 65401 For additional information, please call or e-mail 573 364-4797 573 364-8972 (fax) [email protected] The catalog and request for material is also available online at www.midmoahec.org. No materials will be loaned without a signed agreement between the school and the Mid-MO AHEC. One signed agreement is valid throughout the current school year. We appreciate a seven-day advance reservation. Items are reserved on a first-come basis. A few of the resources can be sent through the Post Office, however, most will require the teacher or the teacher’s designee to pick up and return the items. We recommend calling ahead and setting a specific time for pick-up and return. Mid-MO AHEC will work with one person at each school. That person does not have to pick up or return the materials, but all reservations for materials to be borrowed will funnel through one person. The cost to replace damaged or missing materials will be charged to the person who requested them. If items are not returned as scheduled, a late charge of $5.00 per day will be invoiced to the person requesting the item.
