Marie Curie Cancer Care Job Description Job Title: Lead Nurse Community Palliative Care Services Job Ref: SC2022 Department Caring Services Location: Marie Curie Hospice, Edinburgh Reports to: Hospice Manager Accountable to: Assistant Director, Hospices Job Summary The post holder will provide direct clinical care for patients and manage the Community Nurse Specialist Teams, Day Therapy Services, a Fast Track Palliative Care Discharge service and a Volunteer Visitor Programme in Lothian, working in close collaboration with the Hospice Multi-Disciplinary Team, GPs, hospital and social care providers and local Partners. In line with Hospice and NHS Lothian Palliative Care Strategy, the post holder will work alongside Hospice Clinical teams to lead and inspire the development of systems for implementing palliative care tools and frameworks within community settings (GP Practices and Care Homes and Community Hospitals). The post holder as a member of the Hospice Senior Management Team will lead the integration of nursing practice across Hospice Inpatient and Community settings, promoting a patient centred approach to care across all settings, integrating clinical governance systems, working in close partnership with Line Managers and clinical teams. Deliver complex care by working in partnership with patients and their families within a therapeutic relationship to enable them to make informed choices. Demonstrate nursing expertise including advanced assessment skills, clinical judgement , decision making and the facilitation of appropriate intervention Plan and organise complex programmes of care for patients/ carers and groups of patients or carers. Plan, organise and deliver complex specialist palliative care to patients at home who are or are becoming medically unstable and ensure appropriate diagnosis, treatment plan, monitoring and evaluation in undertaken to ensure their well being, stabilisation and recovery as appropriate. Be able to communicate sensitive information directly to patients and carers and staff even when extremely difficult to convey. Rapidly interpret and synthesise multiple data sources to ensure the full range of care options are considered and implemented for patients. Be able to prescribe independently and maintain competence in this area. Be able to back up clinical decision making with advanced theoretical and specialist knowledge and communicate this effectively to patients, carers, staff across the MDT and with clinical partners. Be able to make and communicate to all parties independent authoritative clinical judgements that ensure the highest standard of clinical care. Reconcile views that differ on the best course of care around individual or groups of patients and communicate effectively all clinical information. Integrate generalist palliative care services effectively with community health and social care services and look for creative ways to improve this integration wherever possible. Management Participate in local NHS palliative care strategy groups and networks to ensure awareness and recognition of the contribution of the activities within the Hospice and to identify future development opportunities. Produce reports, statistics and information as requested and use information to review and develop services. Manage the performance and availability of staff and volunteers throughout the year, including undertaking PPRD’s, managing sickness, absence and annual leave levels, dealing with complaints and participating in the disciplinary process. Recruit and induct staff and volunteers, ensuring that probationary reviews are undertaken and mandatory/core training and education is provided and completed. Monitor and audit standards to secure and evaluate quality improvements in care. In association with the Hospice Manager and other senior colleagues, to be responsible for project management in specified areas to improve the delivery of care. Professional Leadership and Consultancy Provide transformational leadership in the development of palliative care nursing, at clinical and organisational levels, and contribute to the local and regional agenda through the auspices of the Nurse Consultant Network in Scotland and in Marie Curie. Demonstrate strategic decision-making and Influencing Skills, in both forming and translating policy which is of relevance to palliative care. Engage with key stakeholders in the identification of need and in the development, implementation and evaluation of clinical protocols, guidelines and clinical competencies Service Development Work with and provide clinical support to CNS and Day Services Staff, providing appropriate case load management and clinical supervision support. Explore and implement ways of preventing unscheduled admission to acute hospitals by working collaboratively with services across Lothian. Provide leadership in specific service developments to meet identified need GP Practices, Care Homes and Community Hospitals. Lead and participate in clinical governance systems to ensure continuous improvement in quality at both an individual, team and service level. Be able to effect change and coordinate service re –design as required with line managers. Assist and advise with the setting up of new services for patients with non malignant disease for patients and carers. Work with line managers the Marie Curie Nursing Service and the Hospice Management Team to develop and execute service development in line with the Marie Curie and NHS Lothian Strategy. Develop an annual audit plan to monitor clinical outcomes across services and across the patient pathway in order to make service decisions based on up to date knowledge. Trouble shoot any problems in establishing new models of care by liaising with GP’s, IT providers and other organisations. The post holder will be expected to represent the Hospice and Marie Curie Cancer Care externally at regional national and international audiences and have an impact when engaging with large audiences. The post holder will be a highly credible nurse and become widely respected for their clinical and academic and research knowledge and skills Education Within clinical settings, through active facilitation, contribute to the development of a work-based learning culture that actively supports nursing staff to commit to the principle of life long learning. Work with other Palliative Care Nurse Consultants in NHS Lothian in Scotland and in the UK to lead educational interventions which will ensure good palliative care for patients in community settings in NHS Lothian. Provide a consultative educational role in palliative care thus providing an expert resource of specialist knowledge and skills. Identify own personal and professional learning and development needs in response to the development of the service and nurse consultant role and actively addresses these. Research Contribute to the development of a culture that values evidence from patients, from professional expertise and from research, and seeks to use that evidence in the development of person centred practice across all settings. Take responsibility for developing an ethos of critical inquiry among specialist palliative care nursing teams, and support such teams in the critique of literature, and in the implementation of research findings into practice. Be responsible for actively contributing to the palliative care nursing research agenda relevant to service strategy by working in conjunction with the Hospice Research Facilitator and contributing to the Hospice Research Steering Group and the Marie Curie Research Strategy. Engage in nursing research, and in the dissemination of findings through publication in peer reviewed journals and presentations at regional and national conferences, in close collaboration with academic colleagues at Edinburgh and Napier University and specialist palliative care teams. Utilise research findings in clinical work and augment clinical decisions and service decisions with up to date knowledge.
