Haematology Consultant
This is a replacement post for a 10 PA Consultant Haematologist, due to the retirement of the current post
holder. The duties are principally in general and malignant haematology, though a special interest may be
accommodated. The appointee will be part of a team of six consultants, with a specialist doctor and two
specialist registrars.
The department provides a comprehensive clinical and laboratory haematology service for the acute trust
and primary care, and manages inpatients with a full range of haematology conditions to BCSH Level 2b
(2010). The Trust is a large, modern, district general hospital. It is a Cancer Centre and is part of the Central
South Coast Cancer Network. The department is a Haemophilia Centre, linked to Basingstoke for
Haemophilia Comprehensive Care.

The Trust is proud of its military associations and hosts the largest Ministry of Defence Hospital Unit (MDHU)
in the country. The MDHU Portsmouth also means that the Trust has wider responsibilities than standard
NHS organisations. Military staff account for 3% of the Trust’s total workforce. We have a responsibility to

The main hospital is Queen Alexandra Hospital situated in an elevated position to the North of Portsmouth
City. Services include a wide range of Medical and Surgical Specialities, Emergency Department, Paediatrics,
Neonatal ICU, Intensive Care and the Regional Renal Unit. The Haematology and Oncology Centre has a 47 –
bedded ward, Day Unit, Outpatient facilities and Radiotherapy. The large catchment area means a busy and
wide clinical experience.
The Trust works closely with the Universities of Portsmouth, Southampton and Bournemouth where a
number of staff hold academic appointments. It is proud of its excellent, nationally recognised contribution
to research and development. It encompasses postgraduate teaching facilities and is involved in training of
nurses and undergraduate doctors from the University of Southampton. The Trust is the lead body for the
local Research and Development Support Unit and has close links with the University of Portsmouth and its
new School of Postgraduate Medicine for both education and research.
The area of the Health Authority borders on the Solent and English Channel and includes the City of
Portsmouth and the Boroughs of Gosport, Fareham and Havant, extending from Warsash in the West to
Emsworth on the Sussex border. The Northern boundary extends beyond the South Downs National Park to
encompass Petersfield and Liss.
The new Oasis Wellness Centre is an exclusive fitness centre for staff comprising a gym, indoor heated
swimming pool and a range of health and treatment services. The Tops Day Nursery provides on site flexible
child care for staff members.

Queen Alexandra Hospital has been extensively enlarged with a major PFI build which was completed in June
2009, and unified all acute services on one site.
PHT provides:

 State of the art diagnostic imaging facilities

The South block houses paediatrics, maternity and most
of the new wards, including the haematology oncology wards,
F5, F6 and F7 with views over the Solent and Isle of Wight.
The new West Wing houses the Haematology and Oncology
Centre (CHOC) day unit and outpatients, linear accelerators,
main outpatients and the academic centre. To the North of the site is the Rehabilitation Unit. The New
Pathology Building was opened in August 2007 and is linked to the main hospital campus by an access
bridge. On top of the Pathology building is a helipad for the air ambulance service.

Haematology Services
The Haematology service is based at Queen Alexandra Hospital. There are six Consultants and an experience
Speciality Doctor who takes a full share in many of the consultant duties. There are two Haematology
Speciality Registrars from the Wessex Rotation. The Department prides itself in good and well organised
team working with an equitable share of workload rotating between in-patient cover and laboratory duties.
Consultants have individual clinic lists and are supported in clinic by the registrars, staff grade and specialist
nurses.
At present all Consultants manage the full range of haematology malignancy cases. Lymphoma is run as a
joint clinical service with oncology. There is the opportunity for the post holder to be either a general
haemato-oncologist or have some specific interests, but the post holder would need to cross-cover
colleagues and take a share in duties, rotating through ward cover, outlier and liaison haematology,
laboratory cover and anticoagulation advice. The successful candidate would be expected to adopt a flexible
approach to job planning and to fully engage in the team-based cross-cover of all duties and the provision of
out-patient services to sites outside the main department.

Clinical Haematology
The size and population base of the hospital creates a wide-ranging clinical service. The Service Level is 2B,
with a full range of adult haematology care including acute leukaemia induction and complex chemotherapy,
as well as the full range of other benign and malignant haematology conditions. Bone marrow transplant
referrals are sent to Southampton; the department has an excellent working relationship with the Wessex
Bone Marrow Transplant Centre at Southampton, which performs autologous, sibling and matched
unrelated bone marrow transplants. In future, Portsmouth expects to become a satellite centre for
autologous transplants. The Cancer Centre is part of the Central South Coast Cancer Network.
The regional Renal Service is based in Portsmouth giving additional exposure to myeloma patients in renal
failure and plasma exchange treatment facilities for TTP. Malignant Paediatric haematology cases are
managed at Southampton with shared care at Portsmouth provided by the Paediatricians. The lymphoma
CNS provides specialist support for Teenage and Young Adult Cancer patients.
Inpatients are housed on Haematology & Oncology wards in the new buildings. Six of the beds on F7 form
the acute oncology assessment unit. There are a high proportion of single rooms all with en-suite facilities
including 10 HEPA-filtered rooms for neutropenic patients. There are 12-18 haematology inpatients at any
time, principally acute leukaemia treatment, myeloma, elderly haematology and non-malignant
haematology. Lymphoma inpatients are allocated to haematology or oncology depending on their clinic
consultant.
The consultants rotate for a period of ward cover. The ward consultant manages all the haematology
inpatients, assisted by the ward registrar, SHOs and nursing teams. There is a weekly ward multidisciplinary
meeting, supported by microbiology and palliative care.
There are seven haematology-oncology SHO-grade doctors covering the wards and day unit.
One SHO is allocated to each of F5, F6, F7 wards and to the acute oncology unit. One is on twilights and one
is on planned leave. Night cover is provided by Hospital at night.
Consultants have admitting rights to available beds in the Haematology & Oncology Unit.

Outpatient Facilities
Clinics are held in the Haematology Oncology Centre on Level B. All consultants have two main clinics for
general and malignant haematology on weekday mornings, which include both new and follow-up patients.
There are additional emergency/rapid review clinics on Monday and Friday lunchtimes. Specialist
thrombophilia and haemophilia clinics are held weekly. There are two nurse-led anticoagulation clinics for
new patients each week and one review clinic.
Additional general haematology peripheral clinics are held at least once a month at community hospitals on
Portsea Island, Gosport, Fareham and Petersfield.
Dr Ann O’Callaghan (Consultant Medical Oncologist) and Dr Corser (Haematology) lead the Lymphoma
Service with clinics on Tuesday and Wednesday. Dr James attends the lymphoma follow-up clinics. There
will be an opportunity for Candidates with a particular lymphoma interest to incorporate a share of the
lymphoma practice into their job plan.
Day Unit
The Haematology Oncology Day Unit is adjacent to Outpatients on Level B and administers intravenous
chemotherapy, blood and platelet transfusions, immunoglobulin infusions iron infusions, venesections etc.
Bone Marrow Biopsies are performed in the Laboratory with MLA support.

Haematology MDTs
Haematology has two weekly video-conferenced Cancer MDTs with its MDT partner, Western Sussex
Hospitals Trust, Chichester. There is a Leukaemia/Myeloma MDT and a Lymphoma MDT . The lymphoma
MDT includes a specialist lymphoma histopathologist from Southampton by videoconferencing. MDTs are
supported by an MDT coordinator, in a purpose-built MDT room.

Haemophilia and Thrombophilia
The department is a District Haemophilia Centre. Dr Hirri leads the service and works with the Regional
Haemophilia Network. Two part-time Haemophilia Clinical Nurse Specialists and two A&C are seconded
from North Hampshire NHS Trust at Basingstoke. There are 51 Haemophilia A patients on the database,
including 9 severe haemophiliacs. Dr Hirri runs a monthly joint paediatric haematology clinic. Dr Hirri and Dr
James run a weekly thrombophilia clinic.
Research
The department actively participates in a number of NCRI haematology and lymphoma trials and is
supported by two clinical trials practitioners and a clinical trials assistant. There is excellent administrative
support from the Central South Coast Cancer Research Network.
Palliative Care
The hospital has a specialist palliative care team including a palliative care consultant and specialist nursing
team. There is excellent hospice provision in the district provided at Rowans Hospice, Countess Mountbatten
House and St Wilfred’s Hospice.
Laboratory Haematology
Automated Haematology Track
The Blood Sciences Department has an integrated haematology and biochemistry laboratory. It has CPAaccreditation (June 2013). There is a high level of automation, with an integrated robotic track system.
Automated blood analysis, is performed on four Coulter LH 780 analysers on a track, with two slide stainers.
Anticoagulation utilises two ACL TOPS; platelet studies are performed on a PFA and an aggregometer. Blood
transfusion uses three Diamed gel stations for grouping, with manual Diamed crossmatching. Dr Gwynn
Matthias is the lead clinician for transfusion is the Haematology representative on the Hospital Transfusion
Committee.
The onsite flow cytometry laboratory uses two Beckman Coulter FC500, Navios, FP1000 cell preparation and
a TQ prep. Haematinics are on the automated biochemistry track, utilising a Beckman Coulter DXL 800.
Haemoglobinopathy testing uses two TOSOH G8, and SEBIA HYDRASYS gel electrophoresis. The department
provides a regional service for universal neonatal screening.
Future pathology services may be subject to change, for example a network pathology consortium is under
consideration, with some redistribution of services, which could involve increased Blood Sciences capacity at
Portsmouth. The post holder will be expected to participate in laboratory management, authorisation of
laboratory results, anticoagulation dosing, and advice to hospital and GP staff on abnormal results.

The department operates a postal anticoagulation service for 8000 patients. Community phlebotomy INRs
are analysed in the laboratory and dosed on RAID software by anticoagulation specialist nurses with
assistance from the medical staff.
Management Structure
Clinicians are expected to take an active interest in the management of the department, the clinical service
centre and the hospital. They will be expected to work together with professional managers in the interests
of improving the service provided and in the overall objectives of the Trust. Development of leadership and
management skills will be encouraged.

Qualifications
The appointee will have completed the GMC approved Joint Royal Colleges of Physicians Training Board
(JRCPTB) curriculum in haematology or be able to show (to the GMC or an Advisory Appointments
Committee) evidence of equivalence in knowledge, training and experience in clinical and laboratory
haematology practice.
Applicants must hold the Fellowship of the Royal College of Pathologists and Membership of the Royal
College of Physicians. Candidates must be registered Medical Practitioners and on the Speciality Register of
the GMC or be eligible to be on the Speciality Register within 6 months of the date of the Advisory
Appointments Committee (AAC).

The timetable will be finalised on appointment. The above would be fixed commitments within a 10 PA job
plan. Additional PAs would be subject to negotiation. The successful candidate would also be expected to
adopt a flexible approach to working to allow prioritisation of activity dependant on the clinical
requirements of the department and Trust.
If there is disagreement in between a Consultant and the CD for Haematology then review will be
undertaken with the CSC lead Consultant initially and/or a Deputy Medical Director. In the rare event of
major disagreements then there is a mediation and appeals process. A detailed Trust policy is available upon
request, details this policy, it can also be accessed from the Trust Intranet.
Teaching
There is a commitment to teach junior medical staff at all levels including training of Specialist Registrars on
the Wessex Training Scheme. There is also an opportunity to teach medical students from Southampton.
Audit and CPD
The appointee is expected participate in audit and to undertake appropriate continuous professional
development (CPD) each year.

The New Consultant Contract (2003) terms and conditions will apply together with appropriate Trust Policies
and Procedures. The appointee will be required to take part in an on call rota, currently 1 in 6 prospective.

Accommodation
Office accommodation will be made available within the department together with secretarial and audiotyping support. There will be access to a PC and current books and journals. A modern microscope is
available for the appointee, suitable for the work that he/she will be required to perform.
Management
The post holder will be expected to work with local managers and professional colleagues in the efficient
running of services and will share with consultant colleagues in the medical contribution to management.
Subject to the provisions of the Terms and Conditions of Service the post holder is expected to observe
agreed policies and procedures drawn up on consultation with the profession on clinical matters and follow
the standing orders and financial instructions of the Portsmouth NHS Trust. In particular, managers of
employees of the Portsmouth Hospitals NHS Trust are expected to follow the local and national employment
and personnel policies and procedures.
The post holder will be expected to ensure that there are adequate arrangements for hospital staff involved
in the care of patients to be able to contact him/her when necessary.
Disputes with management (or ‘whistle-blowing’)
Should a disagreement with Trust management occur, the appointee has an obligation to endeavour to
resolve any dispute constructively, through the normal management channels or by informing the Clinical
Director, Medical Director, Chief Executive or Chair of the Trust. Should the problem not be resolved, the
Trust has a list of Designated Officers available on the Internet available to hear complaints. The Trust Policy

If the problem is still unresolved, the appointee should seek advice from an authoritative external medical
source such as the College’s Regional Specialty Advisor, the British Society for Haematology or a mentor
from the Association of Clinical Pathologists.
Appraisal
Annual appraisal according to the GMC’s Good Medical Practice Framework cumulating in successful
revalidation process is expected.
Your annual appraisal will be performed by a fellow consultant who has received specific training in
appraisal, with a change in appraiser after a maximum of three years. Any substantive issues arising from an
appraisal will be communicated directly to the CSC lead for Cancer and Surgery or Medical director. The
outcome of appraisal is routinely communicated to the Medical Director through the trust CRMS – Clinical
Resource Management System.
Job Planning review will occur at yearly intervals.
All medical and dental staff are expected to comply with the Portsmouth Hospitals NHS Trust Health and
Safety Policies.
Study leave
30 days within a three-year period, subject to national and local policies will be allowed.

Status of Post
The post holder will be appointed on the new consultant contract and the sessions have been allocated
accordingly. The job plan is for a 10 PA working week. The job plan is reviewed on an annually.
Any Candidate, who is unable to work full-time, i.e. only able to work part-time, will be eligible to be
considered for the post. If such a person is appointed, modification of his/her job plan will be discussed
between the Consultant and Clinical Director in conjunction with Consultant Colleagues if appropriate.
Similar considerations will be made if the Candidate wishes to Job Share
Residence
Residence within either 10 miles or 30 minutes by road from Queen Alexandra Hospital is usually required
unless alternative arrangements are agreed with the trust management. The post holder must normally have
a current driving licence and his/her private residence must be maintained in contact with the public
telephone service.
Safe Guarding
The appointee must act in such a way that at all times the health and well being of children and vulnerable
adults is safeguarded. Familiarisation with and adherence to the Safeguarding Policies of the Trust is an
essential requirement for all employees. In addition all staff are expected to complete essential/mandatory
training in this subject.
Infection Control
In compliance with the Trust's practices and procedures associated with the control of infection, you are
required to:
Adhere to Trust Infection Control Policies assuring compliance with all defined infection control standards at
all times.
Conduct hand hygiene in accordance with Trust policy, challenging those around you that do not.

Hand Hygiene Policy
The Trust has adopted the "Naked Below the Elbow" strategy which means that when involved in patient
care and direct physical contact with patients, you must wash or decontaminate your hands as per the Hand
Hygiene Policy.
Compliance with the Hand Hygiene Policy is mandatory; you must wear short-sleeved shirts / blouses /
uniform, remove any outer jackets and roll up your sleeves. No jewellery should be worn below the elbow
(except a plain wedding band) and wrist-watches must be removed.
Compliance with this policy will be monitored and any non compliance may be subject to disciplinary action.
Rehabilitation of Offenders
Because of the nature of the work for which you are applying, this post is exempt from the provisions of
Section 4(2) of the Rehabilitation of Offenders Act, 1974 by virtue of the Rehabilitation of Offenders Act,
1974 (exceptions) order 1975. Applicants are therefore not entitled to withhold information about
convictions. This could result in dismissal or disciplinary action from the Trust.

Vacant Post
The post is expected to be available in November 2012, and the appointee will be required to take up the
post no later than four months from the date of the offer of an appointment, unless a special agreement has
been made between the appointee and Portsmouth Hospitals NHS Trust
