CHCWM – Pharmacist IDS START MIDWEST
General Summary: The investigational pharmacist is responsible for all facets of the
medication order verification, dispensing and documentation process for investigational
drugs, conventional chemotherapy and supportive care medications provided by the
Investigational Drug Section. The pharmacist will be actively involved in the provision of
pharmaceutical care including: assisting with patient interviews, maintenance of current
medication lists, providing patient medication education, providing drug information to
the healthcare team and patients. The pharmacist will practice with a high degree of
autonomy, guided by IDS policy and procedures and quality assurance verification
guidelines. Responsible for daily supervision of staff assigned to medication preparation,
dispensing, and operation of the IDS in accordance with all applicable rules and
regulations. . The pharmacist will provide symptom management consultations to
START physicians and patients as needed. Develop and manage clinical pathways for
staff pharmacist(s).
Job Requirements:
Education: Graduate of an ACPE accredited School of Pharmacy. Intravenous sterile
product training preferred. Speciality residency in oncology and or board certification
in oncology pharmacy is a plus.
Licenses: Current State of Michigan Pharmacy license
Experience: Two years of pharmacy experience, knowledge of IV and chemotherapy
medications preferred, experience with investigational protocols a plus. Familiarity
with biological safety cabinets, telephone, copier, facsimile machine, computer
equipment and software (Microsoft Word and Excel).
Preferred Knowledge/Skills/Abilities/Education/Experience:
Primary Duties and Responsibilities (Essential Functions):
1. Responsible for verification of medication orders including accuracy of dosages and
calculations thereof, review of basic patient eligibility, including but not limited to
accurately signed / dated study consent form(s), laboratory values within treatment
criteria, appropriate tests have been completed, resolution of known toxicity from
previous chemotherapy, and an absence of serious known drug interactions.
2. Responsible for directly overseeing the preparation and dispensing of all medications
from the IDS, including supervision of certified pharmacy technicians during
preparation of IV admixtures and oral medication dosages, insuring compliance with
all steps of the double check verification process prior to releasing dosages from the
IDS.
3. Provide pharmaceutical care including patient medication counseling, entering home
medications in the electronic medical record (EMR), and drug information to the
healthcare team and patients.
4. Responsible for facilitating departmental communication by demonstrating a
professional, positive and helpful attitude in interactions with co-workers both
internal and external to the IDS.
5. Oversee drug inventories including documentation of current inventory, projection
of expected usage in concert with ordering, receiving, storage and dispensation
tracking of Phase and conventional medications.
6. Assist pharmacy technicians in maintaining adequate investigational documentation,
records, weekly inventory, and interactions with study personnel.

Develop and/or check study-specific orders and other forms necessary for
investigational protocols and amendments.
Review new and amended protocol(s) and associated forms.
Attend the Phase I and Protocol Review weekly meetings as required.
Maintain and generate patient reports and records using the current pharmacy
computer system.
Contribute to the effective operation of the pharmacy department which requires
daily accurate documentation.
Maintain education, skill assessment testing and competency requirements for
current job title and position, Requirements include providing a true and correct
copy of current pharmacy license and receipts from required continuing education
(ACPE accredited courses) when requested.
Other duties as assigned.
Function as a team member by expanding his/her responsibilities as directed by
management to facilitate optimal workflow within the cancer center.
Communicates and interacts with peers and other departments in a manner that is viewed
as having a positive effect on patient services and CHCWM staff morale. Interacts
effectively with patients/family members and demonstrates sensitivity in regards to
cultural diversity, age specific needs and confidentiality.

This employee has access to medical records daily. The parts of the medical record s/he
needs to references to perform his/her job are as follows: Lab Information System,
Pharmacy Information System, Entire Chart/Electronic Medical Record (EMR),
Electronic Billing System (EBS).
Work Environment:
Able to see clearly in an environment which is equipped with typical lighting.
Able to perform manual tasks that require the use of fine muscle motor skills. Able to
freely bend/stretch. 50% of time sitting at computer, 50% walking/moving, able to
transport items weighing less than 30 pounds 5% of the time. Able to access, input and
retrieve information from computer. Able to tolerate frequent handwashing. Very fastpaced and ever changing office environment. Demanding regarding deadlines and time
