Clinical Pharmacist Job Description Under limited supervision, provides clinical pharmacy services including primary care services, consultative services, and drug information services to patients and health care providers to deliver high quality care. Provides education and training to medical and pharmacy students and residents. Ensures accuracy of work completed by support personnel, in compliance with established policies and procedures. Clinical Pharmacist Job Roles and Responsibilities Reviews prescriptions as written by authorized prescribers to determine that order is safe and appropriate for the specific patient; contacts authorized prescriber as necessary to review vague or illegible prescriptions. Fills prescriptions as ordered by authorized prescriber by dispensing or compounding medication according to established procedures. Interviews patients for previous medication history and provides counseling on current drug therapy. Identifies patients who require teaching, and provides educational material. Also works collaboratively with other health care providers to assure coordination of the provision of patient education. Assigns, educates, and supervises the work of support personnel (technicians, interns, clerks and students) in the preparation of medications for dispensing and in related duties. Ensures maintenance of an inventory of medication and pharmacy supplies and related records. Ensures that storage of medications and supplies is in compliance with environmental and regulatory agency procedures. Responsible for the provision of formal patient and staff educational programs. Conducts patient care area inspection to ensure proper drug availability, storage and dating. Provides clinical pharmacy services to patients and health care providers in various areas, including: therapeutic drug monitoring, anticoagulation service, nutritional support, investigational drug services, hematology/oncology pharmacy, medication use evaluation, pain management service, and adverse drug reaction reporting, etc. Adheres to system, hospital and departmental policies, procedures, and objectives; maintains a professional appearance and a clean, safe environment. Participates in clinical research, oversees and/or participates in the preparation and dispensing of medications. Provides education and training to medical and pharmacy students and residents. Clinical Pharmacist Specialist Job Roles and Responsibilities Provides primary care and consultative services to patients and health care providers in assigned specialty areas. Reviews medication orders to ensure accurate, rational and costeffective drug therapy. This includes choice of appropriate drug, indication, route and rate of administration. Assesses patient’s drug therapy; performs pharmacokinetic analysis with appropriate adjustments in dosing, and identifies monitoring parameters and therapeutic outcomes in consultation with the patients. Participates in patient care rounds and consults with health care providers to ensure optimal drug therapy; makes recommendations regarding selection, dosing and administration of medications. Oversees and coordinates the activities of technical staff/pharmacists/interns assigned to specialty area. Assists in the orientation and training of other departmental personnel. Precepts residents, graduate and undergraduate students from accredited Colleges of Pharmacy in clinical clerkship experiences. Obtains medication histories, maintains and monitors medication profiles, and provides medication consultation for patients in the ambulatory clinics when appropriate. Plans, provides and evaluates educational classes for patients and/or health care professional, as appropriate. Participates in the development and production of in-service lectures, clinical case conferences, newsletter articles and other staff development programs for pharmacy or other health care providers. Participates in on-going clinical research projects and other scholarly activities as appropriate; prepares reports or materials for publication. Participates in MUE and ADR activities, and in the planning, implementation and evaluation of other programs designated by the Medication Use Committee and the Pharmacy and Therapeutics Committee. Participates in appropriate committees. Adheres to system, hospital and departmental policies, procedures, and objectives; maintains a professional appearance and a clean, safe environment. Performs other duties as assigned.
