Surveillance Summary Highlights for City Council Budget Meeting 2.10.16

HIV/AIDS
• In 2014, Cuyahoga County saw a 6% increase in HIV/AIDS cases from 2013.
•
In 2014, City of Cleveland saw a 28% increase in cases from 2013.
•
In 2014, 67.9% of Cuyahoga County cases were African-American males; 47% of county cases
were men who reported having sex with another male (MSM).
•
Preliminary results for 2015 Jan-Jun show that Cleveland accounted for 66.3% of all Cuyahoga
County cases.
Syphilis
• In 2014, Cuyahoga County saw an 88.4% increase in cases from 2013.

In 2014, City of Cleveland saw a 65.8% increase in cases from 2013.
•
In 2014, 67.5% of Cuyahoga County cases were African-American males; 56.4% of county cases
were men who reported having sex with another male (MSM).

Preliminary results for 2015 Jan-Jun show that Cleveland accounted for 67.7% of all Cuyahoga
County cases.

2014 Incidence Trends across Cuyahoga County and Cleveland
In 2014, there were 212 new cases of HIV/AIDS in Cuyahoga County. This is a 6% percent increase
from 2013, which had 200 cases.
• 88.2% of cases were male.
• 67.9% of cases were African-American males.
Of the 212 county cases, 164 of the cases were in Cleveland (77.4%). This is a 28% increase from 2013,
which had 128 cases.
• 71.3% of the Cleveland cases were African American males.

Youth/Young Adults Ages 13-24:
• There were 72 cases (34%) that were age 24 and younger.
• Of the 72 cases, 53 cases (73.6%) were in Cleveland.
• There were two cases that were below the age of 18; one case was 16yo in the county, and one
case was 17 yo in Cleveland.
Risk Factors for Cuyahoga County:
• 47% of cases were males who reported having sex with another male in the previous 12 months
(MSM).
• Intravenous Drug Use (IDU) levels were low across the county, at less than 2% among the cases.
• 9% of cases had a history of a sexually transmitted disease (STD).
• Almost 30% of cases reported having had sex with an anonymous partner in the previous 12
months.
• 20% of cases reported having met sex partners through the internet in the previous 12 months.
• 7.5% of cases reported having had sex with a known HIV-infected person.
HIV/AIDS with Syphilis for Cuyahoga County:
• 9 cases (4%) had a history of a syphilis diagnosis.
• At the time of diagnosis, when also tested for syphilis, 24 cases (11%) had reactive (positive) test
results for syphilis.

2015 Incidence Trends across Cuyahoga County and Cleveland
According to preliminary results, in the first six months of 2015, Cuyahoga County had 92 new cases of
HIV/AIDS, with 61 of these cases being in Cleveland (66.3%).

71.7% of all cases were African-American.
88% of total cases were Male.
64% of total cases were African-American male.
Youth/Young Adults Age 24 and Below:
• 34.8% of cases were age 24 and younger (32 cases).
•
21 out of the county’s 32 cases were in Cleveland (65.6%).
•
Of the 32 cases, 5 cases were below the age of 18. Three of the five cases were in Cleveland;
two were in county. The age ranges of the five cases were from 12yo-17yo.
Risk Factors for Cuyahoga County:
• 51% of cases were males who reported having sex with another male in the previous 12 months
(MSM).
• Intravenous Drug Use (IDU) levels were low across the county, at only 3% of cases.
• 22.8% of cases reported having met sex partners through the internet in the previous 12 months.
• 45.6% of cases reported having some form of sexual contact with an anonymous partner in the
previous 12 months.
• 16.3% of cases had a history of a sexually transmitted disease (STD).
HIV/AIDS with Syphilis for Cuyahoga County:
• 7 cases (7.6%) had a history of a syphilis diagnosis.
• At the time of diagnosis, when also tested for syphilis, 13 cases (14%) came back reactive for
syphilis.

Cuyahoga County had a total of 179 cases, with 126 of those cases being in Cleveland (70.4%).
Cuyahoga County saw an 88.4% increase in cases from 2013 to 2014.

Secondary
Primary and Secondary (P&S) Cases:
The primary and secondary stages of syphilis are the most infectious stages of the illness. There were 72
P&S cases in the county in 2014. 40.2% (29 cases) of the county’s P&S cases were in Cleveland.

Risk Factors for Cuyahoga County:
• 91.6% of total cases were male; 67.5% of cases were African-American males
• 56.4% of cases were males who reported having sexual contact with a male in the past 12
months (MSM).
• 18.4% of cases reported having met a sex partner on the internet in the past 12 months.
• 29% of cases reported that they had sex with an anonymous partner in the past 12 months.
• 29.6% of cases had a history of a sexually transmitted disease (STD).
Youth/Young Adults, Age 24 and Younger in Cuyahoga County:
• Cuyahoga County had five cases that were below the age of 18. The youngest age was 15yo.
• 42.5% of county cases were age 24 and younger.
Syphilis with HIV for Cuyahoga County:
19.6% of cases had a co-infection of HIV (35 cases). Of the 35 co-infections in the county, 24 cases
were in Cleveland, which is 68.5% of the cases. Of the 35 cases, 23 were P&S cases (65.7%).
Action Plan
We will continue to encourage the sharing of these findings with health systems, local clinical practices,
and community prevention partners to promote risk reduction strategies for those at highest risk.

Incidence Trends across Cuyahoga County and City of Cleveland
Preliminary results show that in the first six months of 2015, Cuyahoga County had 96 cases of syphilis,
with 65 of those cases being in the City of Cleveland (67.7%).
In the county, 96.8% of cases were male.
62.5% of total cases were African-American male.
Youth/Young Adults, Age 24 and Younger in Cuyahoga County:
• 16.7% of Cuyahoga County cases were age 24 and younger.
• There were no cases below the age of 18.
Primary and Secondary (P&S) Syphilis:
Primary and secondary stages of syphilis are the most infectious stages of the illness.
The county had 26 P&S cases; Cleveland accounted for 18 of the county’s 26 cases (69.2%).
Risk Factors for Cuyahoga County:
• 70.8% of cases were male who reported having sexual contact with another male in the previous
12 months (MSM).
• 27.1% of cases reported having met sex partners through the internet within the past 12 months.
• 42.7% of cases reported having sexual contact with an anonymous partner within the past 12
months.
• Almost half of the cases had a history of a sexually transmitted disease (STD).
Condom Use Among Syphilis Case in Cuyahoga County:
• 52.4% of cases reported “Sometimes” using condoms during anal sex.
• 75% of cases reported “Never” using condoms during oral sex.
Coinfection with HIV in Cuyahoga County:
• About 1/3 of these syphilis cases also had a coinfection of HIV.
This preliminary data will be used to inform stakeholders and community health workers about risk factors
and at-risk populations within Cuyahoga County and the City of Cleveland.
