Background Statement In 1979 a group of local volunteers wanted to provide our community with hospice services. The work of these wonderful volunteers led to the development of Yolo Hospice. Yolo Hospice was developed on the principal that dying is a normal part of life and it affects the whole being; mind, body and spirit. It also recognizes that the care of the dying person affects the family and all those that love him/her. Yolo Hospice, a not for profit, community based organization was run by volunteers until it became Medicare and Medi-Cal certified in 1995. Since then Yolo Hospice has developed a highly trained interdisciplinary team including a physician, a nurse practitioner, an all RN nursing staff, Masters trained Social Workers, Spiritual Care Counselors, Certified Nursing Assistants, bereavement specialist and a large base of volunteers specially trained in end of life care. Yolo Hospice remains true to our mission of providing expert and compassionate hospice care, bereavement support and public education about all end-of- life issues for all people in our culturally diverse community & service area. Yolo Hospice has never denied a patient who needed care, including those without financial resources or health care coverage. Needs Statement 1.Funding for patient's and families that have no healthcare coverage. $12,000 per month covers hospice care for two patient's and their families. 2.Funding for special patient wishes at end of life - $10,000 annually. We receive very special urgent wishes from our patients at end of life that are beyond the medical scope of hospice. Recently a patient requested from one of our spiritual care counselors to marry him and his partner of thirty years. This fund will help us to continue to grant special wishes at end of life. 3.Funding for uncompensated Bereavement support for adults & children - $245,000 annually. 4.Funding for Yolo Care - Palliative Care Program. Support & resources for underserved patients who suffer from chronic illnesses; allowing people to stay in their homes & have services provided to them. $180,000, planning & staffing costs. 5.Funding for specialty Pharmacy needs at end of life - $50,000 annually. 2 CEO/Executive Director Statement As the first established hospice in Sacramento Valley, Yolo Hospice has the longest regional track record as experts in caring for people who have a life-limiting illness. Serving more than 180 communities in the five counties of Yolo, Colusa, Sacramento, Solano, and Sutter, Yolo Hospice provides care in a range of settings from private residences to nursing homes. Our most recent physician satisfaction survey shows that 100 percent of 150 referring physicians are satisfied with the care Yolo Hospice provided to their patients. The most recent Family Evaluation of Hospice Care (FEHC) survey, used by thousands of hospices across the country, shows that Yolo Hospice's bereavement program has scored among the top in the nation and 20 points higher than the national average. Yolo Hospice is dedicated to caring for the community. As a not-for-profit, 501(c)(3) organization, we provide hospice care to patients, bereavement services to patients, loved ones and education in the community about end-of-life issues, grief support, palliative and hospice care. Hospice is a philosophy, not a place or a facility. Hospice provides physical, emotional, and spiritual support. The objective of hospice services is to manage pain and distress; to optimize the quality of life for those with a life-limiting illness. Hospice is a choice about making each moment count. The care may be provided in a private home, an apartment, a residential board and care, or skilled nursing facility—wherever the patient calls home. It’s not just for the last few days of life. Families often state they wish they had known about or started hospice care sooner. Each person and their life situation is different—the care needs, the environment, the acceptance, and sometimes the fear of the unknown. Our skilled staff recognizes the importance of meeting the family and the patient where they currently are—physically, emotionally, and spiritually. We can help identify the appropriate resources for immediate needs, such as caregivers, adult day care, medical equipment, facilities and health care providers. We also offer additional unique services such as the pet therapy program whereby animals are brought to visit with patients and give unconditional love; pet loss support groups for those you have lost a beloved pet; bereavement support group with integrated yoga as another tool to help individuals heal, combining education about the grief process, individual sharing & the yoga experience and the Threshold Choir, an a capella, singing group who, when invited, bring ease and comfort at the bedside to those at the threshold of living and dying. Areas of Service Areas Sacramento County Yolo County Sutter County Solano County Service Categories Primary Organization Category Health Care / Home Health Care 3 Programs Programs Patient/Family Services Description When you or someone you love has a life limiting illness and when there is no longer the possibility of a cure, Yolo Hospice is the choice. Our highly trained specialists (MD, RNs MSWs, chaplains, aides and volunteers) help manage pain and other symptoms, including the emotional and spiritual aspects at the end of life. Our focus is on the patient, as well as their loved ones; our goal is to meet their needs by providing symptom relief, education, and anticipatory guidance with practical support and resources. Yolo Hospice pays for durable medical equipment (i.e. hospital beds, wheelchairs, oxygen etc.), as well as medications related to the terminal illness and for symptom management, and related items such as briefs and wound care supplies. RNs are available 24 hours a day, 7 days a week to answer questions and make necessary visits. Bereavement services are also available to the family for 13 months following the loss or longer if needed. There are progressive star ratings from 1-5 indicating our advances in the unique care of veteran's and our agency commitment to outreach, collaboration, and promotion of this partnership and services available to veterans. We are actively working towards our third star. We offer a unique service with the Barbara Frankel Memorial lending library, comprised of books, periodicals, support pamphlets & DVDs with information to help those seeking support, guidance & comfort with a variety of losses. Many families have taken advantage of these resources housed in one place & readily available. Community Education is a highlight of Yolo Hospice, with an R.N. & Outreach Coordinator, actively working in the community as a resource, offering education & training on hospice choices, community resources, aging, caregiving & end-of-life care. They offer one-on-one visits, in group presentations & at events. Program Success Monitored By Yolo Hospice’s Quality Assessment Performance Improvement Program monitors the quality of patient care through a wide range of internal and external evaluative methods. We participate in Mandatory reporting to Centers for Medicare and Medicaid (CMS). For over 5 years Yolo Hospice has participated in 3 national studies in which quarterly data is submitted to the National Hospice & Palliative Care Organization with subsequent reports compared to regional & national results. The Family Evaluation of Hospice Care, the Patient Outcome Measures, & the Family Evaluation of Bereavement Services studies monitor patient care both during & immediately after services provided, as well as following the 13-month Bereavement Services period. Yolo Hospice has a rigorous internal audit process that systematically reviews patient care data on routine & emergent frequencies. Analysis of the data findings dictates the determined area of improvement efforts required. Evaluation cycles are on-going. 4 Bereavement Services Description The Yolo Hospice Bereavement Services staff is available to be a source of caring support, free of charge, to those whose loved ones have been in our care, as well as to community members. Bereavement specialists & trained volunteers facilitate groups, family meetings, community presentations & provide individual support sessions, home visits, & telephone contact as needed. Grief is a natural & normal response to loss. Though there are common experiences people share as they grieve, each of us has a unique experience of grief as influenced by our personalities, life experiences, coping styles, & the meaning of the loss to us. Loss impacts our minds, bodies, and hearts. By tending to your grief, a sense of increased comfort & gradual healing comes with time. The Barbara Frankel Memorial lending library, comprised of books, periodicals, support pamphlets and DVDs has information to help those seeking support, guidance and comfort with a variety of losses. This allows for an efficient way to support these grievers who seek to be with others facing similar circumstances. Added a Spanish-speaking bereavement specialist to provide follow-up calls to Yolo Hospice families and support to community members. Allows us to provide much needed support and referrals to Spanish-speaking families. Added several 2-3 hour workshops on grief-related topics throughout the year. This provides education & support to individuals who might not want to commit to our other services. Added Pet Loss support group. Funders who have awarded grants for program. 5 Stepping Stones Description Stepping Stones is a support program specifically designed to help children & teens cope with the serious illness or death of a loved one. We offer year-round access to a richly diverse program where children direct their own healing with the guidance of caring & dedicated bereavement specialists & volunteers. The on-going, open format support groups allow them to participate as long as they may need. Art, play, writing, music, projects & group discussions, are used as a means to process grief. They make new friends, who can relate to them. The Children’s group is for age 5-11 and the middler's group is 12-14, the Teen’s group is for age 15-17. New format provides consistent, effective, proven program of support. Added Young Adult (18-25) support group-2x/month drop-in format. Fills a gap in our own service offerings and in the region. Teens can “graduate” into this group when they “age-out” of the teen group. Many group members have suffered traumatic loss and often require longer access to support to help them heal. Developed 4-week curriculum for children/teens who would benefit from a brief support/education model. Format could be used for groups facilitated in schools. Allows us to serve children/teens with an alternative brief, effective & efficient program. Added children’s grief presentations/workshops. Provides opportunities to respond to requests in the community for grief education for parents, teachers, counselors, clergy, medical professions, etc. • Evaluations are given to group participants and parents/guardians 2x/yr or upon termination of group participation. • Funders who have awarded grants for program. • Notes of gratitude and donations sent in for services provided. • Regular contact is maintained with the parents/guardians & provides an excellent opportunities to assess how the child/teen is doing while also providing support and education, as needed, to the adult. 6 We Honor Veterans Description We Honor Veterans, a program of the National Hospice and Palliative Care Organization (link is external) (NHPCO) in collaboration with the Department of Veterans Affairs (link is external) (VA), invites hospices, state hospice organizations, Hospice-Veteran Partnerships and VA facilities to join a pioneering program focused on respectful inquiry, compassionate listening and grateful acknowledgment. By recognizing the unique needs of America’s veterans and their families, community providers, in partnership with VA staff, will learn how to accompany and guide them through their life stories toward a more peaceful ending. We Honor Veterans provides educational tools and resources in advancing these goals: • Promote Veteran-centric educational activities • Increase organizational capacity to serve Veterans • Support development of strategic partnerships • Increase access and improve quality Yolo Hospice is recipient of four stars with the We Honor Veterans program. Budget $0.00 Category Health Care, General/Other Hospice Care Population Served People/Families of People with Health Conditions Program Successes The use of the Military Checklist upon admission to our service highlights for our team the service of a veteran and the challenges he/she may face with end of life. It is an indicator that this patient is not a civilian but a veteran and comes to us with a different story that requires us to tune in to a different channel in order to hear all of his/her story. It also raises the teams awareness that they are working with the family of a veteran and allows them to be fully aware of all of the possibilities that may entail. Program Success Monitored By When this program first began in 2011 we had very few veterans on service with us. Our service to veterans has grown in three years. On veterans day veterans made up a third of our census. The Military Checklist is evaluated on a quarterly basis. Our work with veterans is monitored and approved by the national We Honor Veterans program. We remain committed to our veterans locally and on a statewide level. 7 Yolo Care - Palliative Care Program Description Yolo Care, a collaborative program with Partnership HealthPlan of California, provides home-based palliative care and management for patients with a life-altering illness. Every person is unique, so the program is adapted for each patient's physical, emotional and spiritual needs. A life-altering diagnosis presents very real challenges that go beyond medical treatment. As doctors and specialists focus on treating your condition, we focus on you. Your physical, mental, social and spiritual well-being will all benefit from our whole-person supportive care. Who Qualifies for Yolo Care? Partnership HealthPlan members who are experiencing a decline in the past 30 to 60 days due to an advanced chronic illness, including: 1.Congestive Heart Failure (CHF) 2.Congestive Obstructive Pulmonary Disease (COPD) 3.Cirrhosis 4.Dementia - Advanced 5.Frailty Syndrome 6.Stage IV Cancer What's provided? Yolo Care offers an assessment of care goals and needs, and it establishes a coordinated whole-person approach to your care and/or treatment. Many clients also find great comfort in the program's 24-7 access to a palliative nurse via a special phone number. Beyond that, the program provides connection to community resources, companionship, emotional support, practical help with errands or light housekeeping, education about illness and what comes next, and respite or a little time off for caregivers. After assessing a client's needs, a case manager connects clients with appropriate resources. The case manager may also assign a professionally trained volunteer for ongoing support. What's the cost? There is no charge for Yolo Care clients. The program is funded through Partnership Healthplan of California and other philanthropic support. Budget $105,600.00 Category Health Care, General/Other Health Care Reform Population Served At-Risk Populations Program Successes Yolo Care was launched in collaboration with Partnership Healthplan of California in September 2015. Initially, it was designed to be a six month pilot program but has now received an extension of six months. We currently have seven patients on service with Yolo Care. Program Success Monitored By This is a pilot program with Partnership Healthplan of California. Results are tracked by QDACT and Advance Care Planning tools including the POLST. Monthly tracking by Partnership Healthplan of California and biannual audits.
