DETAILS OF THE EXCHANGE Name of visitior Carmen Muñoz-Almagro Position Head of Pediatric Infectious Diseases Group-Sant Joan de Déu Foundation From 15-07-2011 to 30-07-2011 Workplan Please provide a short description (~300 words) of the purpose of the visit and an outline of the work plan Collaboration between laboratory of molecular microbiology of human pathogens unit, Instituto de tecnología química e biologíca (ITQB), Universidades nova de Lisboa (UNL), Oeiras, Portugal and Pediatric infectious diseases research group (PIDRG-HSJD), Hospital Sant Joan de Déu, University of Barcelona. Dr. Raquel Sá-Leão from the Laboratory of Molecular Microbiology of ITQB and Prof. Hermínia de Lencastre head of the Molecular Microbiology of Human Pathogens Unit are worldwide recognized authorities in the field of pneumococcal infections. They have important experience about genetic studies related to the diversity of pneumococcal populations. PIDRG-HSJD is an emergent research group in the field of pneumococci in Spain. This group has a well characterized collection of invasive and carriage pneumococcal isolates among children from Catalonia, during the period 2004-2009. In addition, the molecular microbiology department of HSJD has wide experience in the use of molecular microbiology techniques such as Real-Time PCR and sequence-based methods for the diagnosis and characterization of main pediatric infections. In the field of pneumococci, Real-Time PCR is applied for diagnosis of pneumococci in direct biological samples (plasma, cerebrospinal fluid or pleural fluid among others) molecular capsular type detection of pneumococci and pneumococcal bacterial load quantification. Automatic sequencing is applied for clonal type detection of pneumococci by MLST. The objective of this collaboration is to exchange information and know-how through handon experiments about: 1) Real-Time PCR applications for pneumococcal detection and molecular serotyping 2) Serotyping methods based on bacterial culture 3) Detection and characterization of atypical pneumococci 4) Application of Simpson’s diversity index and other coefficients to compare pneumococcal collections To develop this collaboration, Dra. Carmen Muñoz-Almagro, Head of PIDRG-HSJD and responsible of the Molecular Microbiology Department of HSJD, will make a formative internship next July 2011 in ITQB in Oeiras, Portugal. This stage will serve to complete the study “ Invasiveness of pneumococcal serotypes and clones circulating in Catalonia during heptavalent conjugate vaccine period” performed by Carmen Muñoz-Almagro and the PIDRG-HSJD under the supervision and collaboration of Dr Raquel Sá-Leão and Prof. Hermínia de Lencastre. Budget estimation Please provide an estimation of the expenses you may incur. Remember that you will have to justify all expenses with the corresponding invoices. Curr Opin Pulm Med. 2010 Jul;16(4):394-8 This paper is a review about the influence of pneumococcal vaccine on the increase of empyema observed in the vaccine period Selva L, Esteva C, Gené A, de Sevilla MF, Hernandez-Bou S, Muñoz-Almagro Direct detection of Streptococcus pneumoniae in positive blood cultures by real-time polymerase chain reaction. C.Diagn Microbiol Infect Dis. 2010 Feb;66(2):204-6 This is an article about the usefulness of Real Time for rapid detection of S.pneumoniae. This technique will be use for direct detection of pneumococci from dried spot in the present study Muñoz-Almagro C, Esteva C, de Sevilla MF, Selva L, Gene A, Pallares R Emergence of invasive pneumococcal disease caused by multidrug-resistant serotype 19A among children in Barcelona..J Infect. 2009 Aug;59(2):75-82 The study compares the incidence of serotype 19A before and after the introduction of conjugate pneumocccal vaccine. The conclusion of the study reported that in our geographical area there was an increase of IPD caused by S. pneumoniae serotype 19A which was mainly related with the emergence of preexisting clones several of them closely related with international multidrug-resistant clones. One of the conclusions was that The use of Real-Time PCR to detect S. pneumoniae substantially increases etiologic diagnosis. Pediatric parapneumonic empyema, Spain. Emerg Infect Dis. 2008; 14(9):1390-7. This is a collaborative study which report the emergence of empyema in Spain is mainly due to an epidemic clone of serotype 1. Serotype 1 is not included in the 7-valent conjugate vaccine (PCV7) Muñoz-Almagro C, Jordan I, Gene A, Latorre C, Garcia-Garcia JJ, Pallares R. Emergence of invasive pneumococcal disease caused by nonvaccine serotypes in the era of 7-valent conjugate vaccine. Clin Infect Dis.2008; 46(2):174-82. This is a major article with more than 80 citations. It is one of the first studies worldwide about replacement phenomenon. We reported that since the introduction of PCV7 for children, there has been an emergence of IPD caused by virulent clones of non-PCV7 serotypes that has been associated with significant clinical changes and a decrease in antibiotic resistance in our geographical area.
