Isolation According to the Isolation Precautions and Standard (Universal) Precautions policy, if there is evidence for the need of special precautions, nursing can initiate the precautions/Isolation PowerPlan. 1. Open the patient’s chart to Orders, look to see if the Isolation PowerPlan has been initiated, if not, Click Add. 2. Search for Isolation, select a. Click link to access isolation signs b. Additional references within the Isolation PowerPlan include: MRSA, VRE, C. Diff Algorithms and the Dermatome Guide. 3. Select the desired subphase, such as MRSA Contact Precautions 4. Read the instructions and select the appropriate cultures 5. According to the MRSA Algorhym nursing can initiate decolinization orders. Read the instructions and make the appropriate selections. Be sure to notify the provider of the decolinization orders being entered. 6. Select 7. Select 8. When entering orders that contain medications the provider’s name who is caring for the patient that day and POE Cosign must be used for the communication type. 9. Select 10. Select 11. Only the MRSA subphase contains medication orders that would require the provider’s name/POE CoSign. Placing Additional Orders to an Initiated PowerPlan 1. Click the Initiated phase of the Isolation PowerPlan 2. Click the View Excluded Components icon to expand/view the orders that are still available for selection. 3. Place a check in the desired order/orders 4. Click Orders for Signature 5. Click Sign and Refresh Nursing Documentation Once the PowerPlan has been initiated nursing will receive two tasks to update: 1. Patient Care Alerts-update the patient’s precautions 2. Patient Education-document patient/family education. Include the specifics of your education a. Isolation educational materials are available on the Intranet. Go to Departments and select Infection Prevention 3. Update the patient’s Plan of Care to include Infection Plan Of Care.
