The World’s First Adaptive Scanner SOMATOM Definition AS Excel Edition Answers for life. SOMATOM Definition AS Excel Edition 2 3 Siemens CT Vision Siemens CT Vision The justification for the existence of the entire medical industry is, of course, better healthcare for all patients. But the realities of clinical routine often make this simple-to-understand goal quite difficult to realize: stay within budgets, reduce hospital stays, speed up time to diagnosis, and deal with personnel issues, while maintaining high clinical standards and volume/throughput. At the same time, patients demand better and faster results. Your benefits In order to meet our share of responsibility in addressing these challenges, Siemens, from the earliest stages of research, 4 product development, and design, relies upon the advice and recommendations of external medical experts to determine our focus – and this focus has been on the needs and demands of our end users. In this way, our products have been able to make a welcome difference for you. Leading innovation In addition, Siemens has always been a visionary company, believing that even the farthest technical horizons were temporary and could be surpassed with consistent dedication to improved health care. We improved every component of our MSCT to provide the industry’s best parameters for image quality and patient access. As a result, SOMATOM Definition AS Excel Edition is the only CT to adapt to any patient and any clinical need. Pediatric. Obese. Cardiac. Trauma. Transforming into what you want at your command, enabling you to become an expert in any field. 11 Introduction Adapts to Any Patient Speed 0.33 s Resolution 0.24 mm SOMATOM Definition AS Excel Edition, with its high true temporal resolution of 165 ms in combination with the industry’s highest isotropic spatial resolution and advanced scanning techniques, such as the Adaptive Cardio Sequence, performs reliable, visually outstanding cardiac imaging at the lowest dose possible, with values of 1–3 mSv. Scan any patient with: 78 cm opening 300 kg weight limit 2 m scan range Coverage 64-slice Reliable cardiac imaging at lowest dose Power 80 kW Supported by the High Definition (HD) Volume Projector, even the smallest coronary vessels appear sharper, while common steps in cardiac imaging are entirely eliminated. Giving you the confidence to perform accurate stenosis measurements or stent planning with outstanding precision. Making obese patients a routine exam When imaging obese patients the challenge is two-fold: patient accommodation and power reserves. 12 Adapts to Any Patient Not anymore. There are no patient exclusions with its large 78 cm gantry opening and a patient weight capacity of up to 300 kg (660 lbs), everyone can be comfortably scanned. And with a powerful 80 kW generator and highly efficient UFC™ detector material, your power reserves are virtually unlimited. Additionally, the Adaptive Signal Boost amplifies low signal areas of the CT data through a comprehensive analysis of surrounding CT measurements. The Adaptive Signal Boost further reduces streaks and noise in the image and maintains correct HU values even for larger patients. Rapid and comprehensive trauma imaging It enables you to visualize patients in trauma situations – including polytraumatic cases – in one stop. Supported by a 2 m scan range, the fast volume coverage of our 64-slice detector means that you can perform an entire body scan of 2 m, from head to toe, at around 20 seconds with highest isotropic resolution at any pitch. The advantages are significant. Uncooperative or unconscious patients with short breathholds can be quickly scanned. Fractures, bleeding, and other polytraumatic injuries can be revealed and immediately shared with your trauma or vascular surgeons using syngo.via and the Acute Care Engine. Managing your daily routine Neuro patients benefit from great graywhite matter differentiation due to new detector configuration. With a simple mouse click your SOMATOM Definition AS Excel Edition can virtually change its detector design. The result? The smallest structures and their pathologies – like the inner ear – can now be easily visualized with unmatched 0.24 mm spatial isotropic resolution. Images are decomposed into high and medium/low spatial frequencies. While relevant tissue information is contained in medium and low frequencies, noise is dominated by high frequencies. Separate processing of medium and low frequency information improves the tissue contrast without amplifying image noise resulting in a better signal to noise ratio. In addition CARE Contrast II facilitates contrast enhanced clinical workflow by synchronizing CT scan and contrast media injection. The injection parameters are automatically transferred to the patient protocol, the e-Logbook and to MPPS thus completing the data for the examination therein and avoiding separate documentation. All this makes the SOMATOM Definition AS Excel Edition an expert in virtually any clinical field. Adapting to any patient, exam, and level of complexity. Every day. Power reserves, large bore, and high-capacity table for bariatric imaging. Visualization of vascular structures in angio-like resolution. Artifact-free cardiac imaging with fast 0.33 s rotation time, combined with the highest spatial resolution and our HD Volume Projector. Excellent gray-white matter differentiation for neuro examinations. Native head image quality can be significantly improved with Neuro BestContrast without increase in dose. 13 How it works Speed Anode In cardiac imaging, where the ability to freeze motion is critical, SOMATOM Definition AS Excel Edition provides you with a very fast rotation time, making non-invasive cardiac diagnosis routinely available. Vacuum Cathode Anode Heat Unique STRATON X-ray tube To increase temporal resolution, the proprietary STRATON® X-ray tube enables the fast gantry rotation time of 0.33 s, resulting in 165 ms temporal resolution for motion artifact-free imaging of the heart. SOMATOM Definition AS Excel Edition provides an exceptional solution by adapting for complete dose protection. It actively manages dose, removing clinically irrelevant radiation for every patient. 21 With SOMATOM Definition AS scanners, saving dose starts right at the point where image data is acquired – the Ultra Fast Ceramic (UFC) detectors enable up to 30 % dose reduction. Introduction Innovation Leadership in Low Dose X-ray UFC Light From the very beginning, one of the most frequent demands of our customers has been patient safety and in Computed Tomography. This translates primarily into dose reduction. For this reason, Siemens has developed many significant products and protocols that follow – even shape and redefine – the “As Low as Reasonably Achievable” (ALARA) principle to reduce radiation dose to the lowest possible level. This lies at the heart of our CARE (Combined Applications to Reduce Exposure) research and development philosophy. Over the years, Siemens has been highly successful in integrating many innovations into the Siemens scanners that significantly reduce radiation dose in comparison to other systems available on the CT market, making Siemens the undisputed leader in Low Dose. Consequently, the SOMATOM Definition AS Excel Edition delivers a unique portfolio of hardware and software features targeted explicitly towards saving dose; it adapts for complete dose protection. 22 Adapts for Complete Dose Protection CARE Dose4D Dose Saving Up to 68 % UFC Up to 30 % Fully automated dose modulation adapts dose to each individual patient in real time, reducing dose up to 68 %. The tube current is switched off within a certain range of projections, minimizing direct exposure to the hands during CT-guided interventions, and saving up to 70 % of dose. An intelligently triggered sequence that shuts off radiation in the systolic phase, thus enabling low dose cardiac imaging below 3 mSv routinely. Enables long-range dynamic scanning with up to 50 % lower dose utilizing an intelligent image noise suppression algorithm. Adaptive Cardio Sequence Adaptive Dose Shield 4D Noise Reduction 1–3 mSv Cardio Up to X-ray off X-ray on Adaptive ECGPulsing Up to 50 % HandCARE Up to 70 % Pediatric 80 kV Protocols Up to 50 % 13 % Up to 50 % IRIS Up to 60 % Dose Shield Image data recon Image correction Dose Shield The heartbeat-controlled dose modulation applies the exact dose necessary to collect all data during the diastolic phase, saving up to 50 % dose*. *Results may vary. Data on file. Enables up to 50 % dose savings in pediatric imaging by utilizing dedicated low kV child protocols that allow an individual adaptation of the patient dose to small sizes of pediatric patients. Its ability to dynamically block clinically irrelevant pre- and postspiral dose significantly reduces the possibility of over-radiation, saving up to 13 % of dose. Multiple iteration steps in the reconstruction process eliminating noise, therefore enabling up to 60 % dose reduction. 23 Introduction Adapts for Complete Dose Protection Dose Shield Dose Shield Superb image quality at lowest dose Most of today’s clinical examinations benefit from spiral acquisition techniques. That’s where the heart of our dose protection technologies lies. CARE Dose4D™, our real-time dose modulation, guarantees an unparalleled combination of maximum image quality at minimum dose for every patient in every spiral scan. Eliminating over-radiation in every spiral scan However, the continuous demand for more coverage and the corresponding increase of detector size has unveiled a new challenge for CT manufacturers – over-radiation, both pre- and post-spiral scan, has significantly grown. 24 Adapts for Complete Dose Protection Our answer: Siemens’ innovative Adaptive Dose Shield. It addresses this important safety issue by dynamically blocking clinically irrelevant over-radiation in spiral scans. This simply was not possible before. Now, we ensure that the only dose applied to the patient is dose that is clinically relevant with no compromise in image quality. Low dose cardiac imaging Additionally, the increasing demand, especially in cardiac imaging, is to reliably deliver superior image quality, even during unstable conditions, while maintaining lowest possible dose. To offer full flexibility, we developed dose saving techniques for both spiral and sequential examinations. With Adaptive ECG-Pulsing™, our innovative heartbeat-controlled dose modulation, we have designed a unique solution that reduces dose of up to 50 %* in spiral cardiac imaging. At the same time, its real-time monitoring of the ECG automatically and instantly reacts to changes and abnormalities of the heartbeat. Intelligently triggered step-and-shoot In addition, SOMATOM Definition AS Excel Edition offers an alternative acquisition technique called Adaptive Cardio Sequence. Based on an intelligently triggered step-and-shoot exam, this approach for low heart rates makes an even lower radiation exposure possible, while compensating for potential patient arrhythmia. SOMATOM Definition AS Excel Edition gives you the ability to control dose and adapt it precisely to your clinical need. It’s something all patients will appreciate. Elimination of unnecessary dose for every spiral scan with the Adaptive Dose Shield. 4D dose adaptation, reducing dose up to 68 % with CARE Dose4D. Low dose cardiac exams with intelligent spiral and sequence scan modes. Significant image quality improvement or up to 60% dose reduction with IRIS. *Results may vary. Data on file. 25 How it works Adaptive Dose Shield Conventional tube collimation SOMATOM Definition AS Excel Edition eliminates over-radiation to the patient during spiral scans (marked in red). The Adaptive Dose Shield is part of the innovative new STRATON X-ray tube design. It dynamically moves shields into place on the X-ray tube to block unnecessary dose. The Adaptive Dose Shield dynamically opens at the beginning of a spiral range and then dynamically closes at the end. Pre-Spiral Dose Post-Spiral Dose Image area Conventional technology Eliminating over-radiation in every spiral scan Now all clinically irrelevant dose is eliminated. Not only for dedicated applications, but for every single spiral acquisition. Giving you the ability to additionally save up 13 % of dose for example in cardiac or head scans. STRATON with Adaptive Dose Shield No Pre-Spiral Dose No Post-Spiral Dose Image area Adaptive Dose Shield *Results may vary. Data on file. 26 Adapts for Complete Dose Protection react Reliable low dose spiral heart scans Adaptive ECG-Pulsing, our innovative heartbeat-controlled dose modulation, applies the exact dose necessary to collect data projections of 180 degrees during the diastolic phase. Its real-time monitoring of the ECG enables a fully automated dose adjustment, instantly reacting to changes and abnormalities of the heartbeat. With a temporal resolution of only 165 ms, the time of high exposure during each heartbeat can be cut down even further, as compared to traditional single source CT scanners. The MinDose feature reduces the pulsing window to a minimum of 4 %, giving you additional dose savings. Low dose scans of 1–3 mSv in clinical routine In addition to the Adaptive ECG-Pulsing, the SOMATOM Definition AS Excel Edition offers a sequential cardiac acquisition mode. We call it Adaptive Cardio Sequence. It’s an intelligently triggered sequence that shuts off radiation in the systolic phase and dynamically reacts to irregularities during the ECG trace. The real-time ECG monitoring reacts instantly to irregularities and stops the scan until the heart rate becomes stable again. This arrhythmia compensation method allows for high dose savings and an increased robustness of the scan, thus avoiding re-scans. 4 % Minimum Dose Adaptive ECG-Pulsing in Spiral Mode scan move scan react scan Adaptive Cardio Sequence in Sequential Mode 27 How it works Iterative Reconstruction in Image Space (IRIS) Standard filtered back projection Dose reduction with CT has been limited by the currently used filtered back projection (FBP) reconstruction algorithm. When using this conventional reconstruction of acquired raw data into image data a trade-off between spatial resolution and image noise has to be considered. Higher spatial resolution increases the ability to see the smallest detail; however, it is directly correlated with increased image noise. Iterative Reconstruction in Image Space Iterative reconstruction approaches allow decoupling of spatial resolution and image noise. In an iterative reconstruction, a correction loop is introduced into the image generation process. To avoid long reconstruction times known from theoretical iterative reconstruction 28 Adapts for Complete Dose Protection Siemens has developed a new method. The new Iterative Reconstruction in Image Space first applies a raw data reconstruction only once. During this newly developed initial raw data reconstruction, a so-called master data are generated that contains the full amount of raw data information yet at the expense of significant image noise. The following iterative corrections known from theoretical iterative reconstruction are consecutively performed in the image space. They “clean up” the image and remove the image noise without degrading image sharpness. Therefore, a time-consuming repeated projection and corresponding back projection can be avoided. In addition, the noise texture of the images is comparable to standard well-established convolution kernels. The new technique results in artifact and noise reduction, increased image sharpness, and dose savings of up to 60 % for a wide range of clinical applications. Standard FBP Raw data recon • Limited dose reduction • Ultra-fast recon without iterations • Well-established image impression IRIS Image data recon Image correction • Up to 60 % dose reduction • Image quality improvement • Fast recon in image space • Well-established image impression Standard FBP IRIS at 60% lower dose IRIS with better low contrast detectability This image shows the use of standard filtered back projection reconstruction. Despite the fact that the middle image was acquired at 60% lower dose it shows the same low noise compared to the original acquisition and similar noise texture of the images maintaining a well-established image impression. This image shows an IRIS reconstruction performed on the initially acquired data. Improved image quality with better low contrast detection can be nicely visualized. 29 Clinical Images Excellent image quality with low noise, despite only 2.37 mSv for a 65 cm thorax abdomen scan with IRIS. 30 Adapts for Complete Dose Protection Excellent visualization of intracranial arteries including cerebral lesions with syngo.CT Neuro DSA.
