SUBJECT: MEDICATION ADMINISTRTION VERIFICATION- BAR CODE SCANNING DEPARTMENT: MEDICAL SURGICAL UNIT REFERENCE PAGE: 1 OF: 3 EFFECTIVE: 2/15/2012 APPROVED BY: MEDICAL STAFF REVISED POLICY: To ensure patient safety during medication administration, a barcode scanning system will be used for patient identification and medication verification. PROCEDURE: At the patient bedside, the nurse will scan the patient’s armband along with all the medications treatments ordered for the patient. For ISOLATION patients, the patient armband will be placed on the inner door frame to the patient room using a metal magnetic clip. Once you have visualized the patient, scan the armband on the clip and proceed with med verify. Bulk items (inhalers, ointments, eye drops, etc) will be labeled with a printed patient identification label. Injectable medications except insulin will be scanned at the patient’s bedside. Sliding scale insulin, and accuchecks will be documented through the Diabetic Record. Scheduled insulin will be scanned and documented through the eMAR PATIENT MED VERIFICTION SCREEN Medical Surgical Unit Medication Administration Verification SUBJECT: MEDICATION ADMINISTRTION VERIFICATION- BAR CODE SCANNING DEPARTMENT: MEDICAL SURGICAL UNIT REFERENCE PAGE: 2 OF: 3 EFFECTIVE: 2/15/2012 APPROVED BY: MEDICAL STAFF REVISED MEDICATION LIST- Allows access to the patient’s pharmacy profile BAD NDC message is displayed in the medication name- save the med container and record the information on the Med Verify Problem Log for Pharmacy. MEDICATION NOT FOR THIS PATIENT appears in the medication name, the NDC Number scanned does not match the NDC number on the patient’s profile. The 24 hour MAR should be reviewed before attempting to administer the medication. MEDICATION HAS BEEN DISCONTINUED appears in the medication name, the medication has been discontinued on the patient’s pharmacy profile. The 24 hour MAR should be reviewed before attempting to administer the medication. MEDICATION COUNT – The number of medication that have been selected for administration. Give/Omit Select Give to document that the medication has been administered. Select Omit to document that an administration has not been administered or if the medication is to be discontinued. Dose Dose includes fields for medication quantity and units Site: Select the drop-down arrow if the site needs to be documented Reason: Select this option to document the reason for a medication administration. Comment: This is a free text field that can be accessed on all administrations excluding PRN medications. Results: Select this option to display a list of resulted ancillary orders. Select an order to display the result report in PDF format. Clear All: When selected, all patient information is removed, and the system is ready for the next patient to be scanned. Select Exit to return to the previous screen Unverified medication- all medications must be verified prior to administration. Nursing will receive the following prompt when an unverified medication is scanned. Each month the nursing staff will receive their medication administration compliance percentage along with the overall nursing staff medication administration compliance percentage.
