Dentists and staff are encouraged to take advantage of
online registration. Staff will not pay a registration fee
if they register online and print & bring their badge
(limited attendance course fees still apply).

Dentists must be members of the ADA. 2016 dues
must be paid at time of advance registration or ADA
members may register on-site upon presentation of
a 2016 ADA membership card. Dentists may not
register under any other category.
The following dentists will be exempt from paying the
registration fee: Table Clinicians, Speakers, Graduate
Students, Active Life Members (only for the year in
which they are inducted into life membership), and
Retired Life Members.
Non-member dentists............................... $1097
Non-members of the ADA will be admitted upon payment of the non-member admittance fee. This is applicable to the current fee of national and state dues
plus the $200 registration fee. By request, this fee (not
including registration fee) may be applied to the dentist’s account for the current year dues upon payment
of district dues within 30 days of the Annual Session.
Non-members will receive a guest badge.
Military Personnel must be a member of ADA to be
admitted or may pay the non-member admittance fee.
Dental Students will be admitted upon presentation of
a 2016 ASDA membership, 2016 ADA membership
card, or Student ID.
Spouse and family members who are dental assistants

or dental hygienists should register as staff.* Dentists
may not register as a spouse or family member.

All staff, including IDAA and IDHA members, must
register with the IDA to attend exhibits, table clinics,
and courses at the Iowa Events Center.
Dental Lab Technicians will be admitted to exhibits;
however, those not registered with an exhibit booth are
prohibited from promoting their services on the exhibit
floor. A Dental Lab Technician who is working at an
exhibit booth should register as an exhibitor.
Dentists who are exhibiting must also be registered under the dentist category.
Cancellations
A $25 administrative processing fee will be charged on
all cancellations per paid registrant. The cancellation
fee applies to registration fees and course fees. The
cancellation fee will be deducted from all refunds.

Dentists: Bring your confirmation with you. Show
your confirmation to pick up your packet of materials
in the Exhibit Hall. Dentist, spouse and family badges
will be in your packet. Course passes and event tickets
will be included in the dentist packet.
Staff: Bring your confirmation with you. Staff registering online must print and bring their badge. Badge
holders will be available to pick up in the Exhibit Hall.
Staff registering by mail or fax may pick up their badge
in the registration area.

Continuing Education Credits for Dentists, Dental
Hygienists and Dental Assistants
General Attendance
The Iowa Dental Board will award three (3) CEUs for
general attendance. DANB will award one (1) CEU
for attending exhibits. The verification code for general attendance will be posted in the registration area.
Table Clinics
The Iowa Dental Board will award two (2) CEUs
for attending table clinics. DANB will award one
(1) CEU for attending table clinics. The verification
code for attending table clinics will be provided in the
exhibit hall.
Courses
The courses sponsored by the Iowa Dental Association are open to dentists, dental hygienists, and dental
assistants. All courses sponsored by the Iowa Dental
Association have been submitted for approval to the
Iowa Dental Board for CEU credit for dentists, dental
hygienists, and dental assistants. The number of approved CEU credits is listed with the course description.
Continuing Education Recording Form
All attendees will receive a Continuing Education
Credit Recording Form. To receive credits, record the
verification code announced at the end of each course.
Completed forms should be deposited in the boxes in
the registration area or in the foyer at the end of the
lecture room hallway.
AGD Members: Sign course roster at each lecture
you attend to receive AGD credit.

Dental Assistant Registration Renewal
Requirements
All dental assistants must renew their registration every two years. Renewal will require completion of 20
Iowa Dental Association 2016 Annual Session
hours of continuing education every two years to include:
• Two hours in the subject area of infection control
• Evidence of current certification in CPR (up to three
hours may be claimed)
• Two hours in the subject area of dental radiography
(required only if certified to take x-rays)
CEU Requirements for Dental Hygienists and
Dental Assistants Courses - Staff Courses
You must complete a separate CEU card to receive
credit for courses. You will receive the card when you
enter the lecture room. You must be on time for the
lecture or you will not receive the card and, therefore,
will not receive credit. Record the verification code
announced at the end of the lecture on the card and return the card to the course monitor at the end of the lecture to receive credit. In exchange for the completed
CEU card, you will receive a proof of attendance card.
You must print your course pass when
registering online. You will need the course
pass to be admitted to the lectures.
Limited Attendance Courses
See program schedule for the listing of limited attendance courses and course fees for dentists, dental hygienists and dental assistants.
Course Handouts Online
Handouts will be available online before the meeting.
You may print the handouts for the courses you plan to
attend and bring them with you to the Annual Session.
Handouts will not be distributed on-site. Download
handouts from the IDA website (www.iowadental.org)
after April 15.
Course Passes
Course passes for limited attendance courses will be
placed in dentist registration packets. Dentists must
show course pass to be admitted to the lecture.
Staff must print course passes when registering
online.

Parking
Available north of Veterans Memorial with entrances
off Third and Fifth. Parking also available on streets
and in city garages. Parking fees may apply.
Transportation to Lectures and Exhibit Hall
Shuttle bus transportation will be provided from all
designated overflow hotels to the Iowa Events Center.

The Iowa Dental Association needs support from dentists to make our table clinics program a success. As
a table clinics presenter, the $125 registration fee will
be waived and you will receive 4 CEU credits. Remember that one of the easiest ways to present the information to your colleagues is by doing a Powerpoint
presentation. An application to present a table clinic
is included in this brochure. If you have questions

Socket grafting without a doubt is a value added service. It obviously facilitates good implant placement
but it can also be used for ridge augmentation for fixed
or removable prosthetics. Socket grafting is an intricate

Description:
Biofilm infections that cause periodontal disease present
significant treatment challenges. Not only do biofilms
have defensive capabilities that make treatment less effective, but they are also difficult to address between
office visits. Toothbrush, rinse, and floss just don’t get
deep enough. One approach that offers better biofilm
management is the prescription tray delivery of oxidizing and oxygenating agents deep into periodontal pockets. Research shows statistically significant reductions
in bleeding and pocketing when prescription tray delivery is combined with scaling. Case studies explain how
doctors incorporate the special delivery into treatment
plans. You’ll walk away from this course with practical
solutions that you can implement tomorrow.

The traditional clinician’s head and neck examination is
best at detecting visible changes on the tissues surface.
Unfortunately, the tissue changes that lead to oral cancer
start below the surface of the epithelium at the basement
membrane. These changes are not normally visible to

the unaided eye until the disease progresses to the epithelial surface.
Enhanced screening is at the core of the new approach
of prevention. Technology now exists that allows the
dental clinician to easily detect pre-malignant conditions and much more at the earliest stages. Now with
frequent exposure in the media, increased legal claims,
and links to the HPV virus, patients are beginning to understand, differentiate, and expect adjunctive technologies to be used for enhanced screening and prevention.

Know Your Patients’ Medications and Their Impact
on Dental Treatment!
Your patients are living longer thanks to their medications but many of the physician-prescribed medications
used by your patients have dental implications and side
effects affecting your treatment plan. When is it safe to
have the patient stop taking their anticoagulants for dental treatment? Some of these medications are the drugs
of choice of “doctor shoppers”. Some medications are
more likely to cause violent behavior and others can
cause a “burning mouth syndrome” and, in fact, others
may cause sexual dysfunction. Should I use caution
when prescribing NSAIDs with antihypertensives? Is
antibiotic prophylaxis required for patients with orthopedic prostheses?
This presentation includes the indications, contraindications, and side effects of the most commonly prescribed
medications. Many of these medications were not approved when you took your pharmacology course in
dental school. These medications represent 30% of all
prescriptions taken by your dental patients and familiarity with these drugs will provide the dental practitioner
with a better appreciation for the health profile of your
dental patient.
At the conclusion of this course, the attendees

adversely interact with dental drugs.

The objectives of this course are to explain the CDC’s
guidelines and recommendations and discuss how dental
practices in Iowa can maintain compliance with the law
to provide a safe environment for patients and employees. Specific topics that will be covered include standard precautions, use of personal protective equipment,
hand hygiene, latex sensitivities and allergy, operatory
preparation, instrument processing and sterilization,
and management of sharps. You will also understand
the OSHA regulations and how they apply to the dental
practice.

2016 ANNUAL SESSION
Dr. Trishul Allareddy, is an Associate Professor at
The University of Iowa. He joined The University
of Iowa, College of Dentistry and Dental Clinics in
2008 as a faculty member after completing his residency there and having spent a little more than a year
as a consultant to a private imaging company. He is
currently the Program Director for the Advanced
Education Program in Oral and Maxillofacial Radiology (AAOMR) and the Director of Radiology at the
College of Dentistry. He serves on several collegiate,
university, national, and international committees.
He is currently the American Dental Association and
American Academy of Oral and Maxillofacial Radiology representative to DICOM. He is the co-chair for
Working Group 22 (Dentistry) of the Digital Imaging
and Communication in Medicine (DICOM) Standards
Committee, where he also serves on the Leadership,
Outreach, and Educations Committee and is the cochair of the Standards and Codes Committee for the
AAOMR. He is a reviewer for eleven peer-reviewed
Journals in dentistry and radiology. His main expertise
and interest is in the use of technology in dentistry. His
research areas are related to reduction of the radiation
dose to the patient, healthcare outcomes, dental education, big data share, and the role of advanced imaging
in the oral and maxillofacial regions as it applies to
dentistry.

trainer, clinical consultant for The Dental Advisor and
member of Catapult Elite. Dr. Bizga gives back to the
community via dental missions around the world.
Dr. Timothy M. Bizga, is a general dentist practicing in Cleveland, Ohio. His practice focuses on comprehensive care with Dr. Bizga’s special interests in
implants, cosmetics and facial aesthetics. His background in dentistry is lengthy and diverse. Once a
former chairside assistant, he also worked as a dental
lab-technician making his perspective unique among
others in the field of dentistry. He graduated from
Saint Louis University with a BA in Biology before
receiving his DDS from the University of Michigan
School of Dentistry, at which time he was the recipient of the Francis B. Vedder award for excellence in
prosthodontics. He is currently a member of the American Dental Association and a Pre-fellow in the Academy of General Dentistry. He is a graduating member
of the Whitecap Institute where he received advanced
Dr. Crossley has co-authored a number of articles and
four books dealing with a variety of topics within the
field of pharmacology. Other areas of expertise include the pharmacology of street drugs and chemical
dependency. He serves on the Maryland State Dental Association’s Well-Being Committee, is an active
member of Phi Kappa Phi, Omicron Kappa Upsilon
Honorary Dental Society, the American College of
Dentists, International College of Dentists, and an
honorary member of the Thomas B. Hinman Dental
Society. He was the recipient of the 2008 Gordon
Christensen Lecturer Recognition award presented by
the Chicago Dental Society and the recipient of the
2012 Award of Distinction presented by the Academy
of Dentistry International for his efforts in Continuing Dental Education. He has been a consultant for the

Dr. Bruce Cochrane, has been practicing periodontics in Northwest Iowa for forty years. He is a veteran
of blade, periosteal and osseo integrated implants. He
has a wealth of experience with osseo integrated implants. Dr. Cochrane is a graduate of the University of
Nebraska. He has specialty degrees in Fixed Prosthodontics and Periodontics from the University of Missouri in Kansas City.
Dr. Harold L. Crossley, is Professor Emeritus at
the University of Maryland Dental School. A native
of Rhode Island, Dr. Crossley received a Bachelor of
Science degree in Pharmacy from the University of
Rhode Island in 1964. He later was awarded the Master of Science (1970) and Doctorate degrees (1972)
in Pharmacology. The University of Maryland Dental
School in Baltimore awarded Dr. Crossley the D.D.S.
degree in 1980. The liaison between the classroom
and his part-time dental practice produced a practical
approach to understanding the pharmacology of drugs
used in the dental office.

other law enforcement agencies since 1974. Drawing
on this unique background, Dr. Crossley has become
nationally and internationally recognized as an expert
on street drugs and chemical dependency as well as
the clinical pharmacology of dental drugs.
Tanya Dunlap, PhD, is the Managing Director for
Perio Protect, LLC. She coordinates clinical research
involving prescription tray delivery and offers educational content on the important implications of research results to dental professionals and patients.
Dr. Keith V. Krell, has been in full-time private practice with Endodontics PC in West Des Moines, Iowa,
for 27 years. He is an adjunct professor in the Department of Endodontics at the University of Iowa College
of Dentistry where he also served as graduate program
director. In 1993, he retired from the United States
Army National Guard as a lieutenant colonel after 22
years of service.
and faculty at the Mayo Clinic where he served as Program Director in Orthodontics and received several
“Teacher of the Year” awards. In 1996, Dr. Larson left
Mayo for full-time private practice in Rochester, MN,
where he continues to practice 1 day a week. In 2004
he returned to full-time academics as Associate Professor and Director of Orthodontics at the University
of Minnesota. He has written many scientific articles,
contributed book chapters, and given many lectures in
the US and abroad. He recently received the Distinguished Dental Alumnus Award from the University
of Minnesota.
Dr. Larson is active in organized dentistry both locally
and nationally. He is a Past-President of the Minnesota
Association of Orthodontists, the Midwestern Society
of Orthodontists (MSO) and currently represents the
MSO on the AAO Board of Trustees which will lead
to becoming AAO President in 2018.
He received his D.D.S. and M.S. degrees from the
University of Iowa in 1981 and 1983, respectively. He
earned his M.A. degree in sociology-anthropology in
1975 from the United States International University
in San Diego, Calif.
Dr. Harvey Levy, practices general and hospital dentistry in Frederick, Maryland. He emphasizes comprehensive dental care for individuals with anxiety or special needs. He graduated from Tufts University Dental
School in 1974, Residency at Eastman Dental Center
in 1976, and was a clinical residency director at University of Penn Hospital.
Dr. Krell is a Diplomate of the American Board of
Endodontics as well as a past president and past director of the ABE. He also served as a director on the
AAE Board of Directors and is the Past President for
the AAE Foundation.
Dr. Levy has earned Mastership plus four Lifelong
Learning Service Recognitions by the Academy of
General Dentistry along with eight fellowships, five
diplomate certificates, and board certification in Integrative Medicine.
Dr. Krell is a member of the Scientific Advisory Board
of the Journal of Endodontics and is a reviewer for
both the JOE and JADA.
His work with patients with anxiety or special-needs
has earned him the AGD Humanitarian Award, the
American Dental Association Access to Care Award,
the Maryland Governor’s Doctor of the Year Award,
Morgan State University’s Public Oral Health Care
Award, the Maryland State Dental Association’s First
Humanitarian Award, Special Care Dentistry’s Saul
Kamen Award, and the honor of running the 2002
Olympic Torch.

Dr. Brent Larson, received his dental degree from
the University of Minnesota in 1981, completed a
GPR in the Air Force in 1982 and practiced general
dentistry until 1985. He received his Certificate and
Master’s degree in Orthodontics from the University
of North Carolina in 1987. He spent 3 years as an orthodontist in the U.S. Air Force before joining the staff

Dr. Levy also holds nine black belts and was inducted
into the US Martial Arts Hall of Fame.
Dr. Donnie Luper, has lectured on dentistry and
endodontics internationally in countries including the

USA, Philippines, Malaysia, Thailand and Kenya. He
is a member of the ADA, AAE, Tar Heel Endo Society, Southern Endo, and the North Carolina Dental
Society.
Dr. Luper received his certificate in Endodontics from
the National Naval Dental Center, Bethesda, MD, in
1990 and his dental degree from the University of
North Carolina School of Dentistry in 1982. He earned
his undergraduate degree from North Carolina State
University in 1979.
Dr. Luper practiced general dentistry in the US Navy
for 16 years and found his interest in endodontics while
completing a three month rotation in the Endodontics
Department at the US Navel Air Station in Jacksonville, FL. He is presently in a group private practice
with offices in New Bern, Kinston and Greenville, NC.
Dr. Michael Moffitt, received his DDS from Baylor University and, after Naval service, completed his
certificate in Prosthodontics from UMKC where he
taught briefly. His practice is geared toward creating
esthetic solutions and solving complex dental problems. He has been an author and speaker but his true
love is completing exquisite dentistry. Dr. Moffitt restored his first implant working with Dr. Don Callan in
1975 and has since restored thousands. He has always
enjoyed removable prosthetics and began using locators when they first were introduced.
Dr. Moffitt is a member of the American College of
Prosthodontists, The Academy of Osseointegration,
The American Academy of Implant Dentistry and is
the prosthetic advisor for the North Iowa Dental Seminars. He also enjoys mentoring dentists in the art of
comprehensive dentistry and in restoring complex
cases.
Dr. James Q. Swift, A native of Manchester, a graduate of Cornell College and the University of Iowa
College of Dentistry, Jim Swift has strong Iowa ties.
Dr. Swift has been on the OMS faculty at the University of Minnesota for 27 years. During that time, over
60 OMS have graduated from the residency program.
He is the 2011 recipient of the Gies Award for Oral and
Maxillofacial Surgery. He was Century Club Professor of the Year in 2001 at the University of Minnesota

and received the American Dental Education Association’s Distinguished Service Award in 2009. He is a
former President of the American Board of Oral and
Maxillofacial Surgery. He is currently Chair of the
Board of OMSNIC. He research has been sponsored
by the NIDCR at the R01 level. He had authored many
scientific articles an chapters. In September 2015, Dr.
Swift was given the RV Walker Distinguished Service
Award from the American Association of Oral and
Maxillofacial Surgeons.

Dr. Timmons became a faculty member in the Department of Oral Pathology, Radiology and Medicine
(OPRM) in 2002. Dr. Timmons teaches pre-doctoral
and post-doctoral students; treats general dentistry and
oral medicine/head and neck pain patients within the
faculty practice; and is the Program Director of the
Advanced Education Program in Oral and Maxillofacial Pathology. Dr. Timmons is also the director of the
Sterilization Monitoring Program and the Health and
Safety Officer at The University of Iowa College of
Dentistry.
Ebon Turner, B.S. Neuroscience, has spent the
last sixteen years working for several major players
in the medical and dental industry covering a spectrum of sales management assignments. As a former
principle at Med Tech Systems (a medical consultant
firm), Ebon gained experience bringing new and innovative technologies to the market place. In 2006,
Mr. Turner joined LED Medical Inc. as the Eastern
Regional Manager. The company goal is to bring the
brilliance of tissue fluorescence to dentistry, as developed by the British Columbia Cancer Agency. The
intention is to enhance the detection of oral abnormalities and promote and advocate for early detection

of oral SCC. After several years spent establishing a
reputation as a go to professional for information on
oral cancer detection technologies, Ebon now lectures
throughout North America on the topic of oral cancer,
the importance of early detection, and the role of VELscope within the dental health landscape.
Dr. Juan F. Yepes is an associate professor in the
department of pediatric dentistry at Indiana University
School of Dentistry and an attending at Riley Children
Hospital in Indianapolis, Indiana. Juan F. is a dentist
(DDS) and a physician (MD) from Javeriana University at Bogotá, Colombia. In 1999, Juan F. moved to
the USA and attended the University of Iowa and the
University of Pennsylvania where he completed a fellowship and residency in Radiology and Oral Medicine respectively in 2002 and 2004. In 2006, Juan
F. completed a Master in Public Health (MPH), and
in 2011 a Doctoral Degree in Public Health (DrPH)
both with emphasis in Epidemiology at the University
of Kentucky College of Public Health. In 2008, Juan
F. completed a residency program in Dental Public
Health at University of Texas, Baylor College of Den-
tistry. Finally, Juan F. completed a residency program
and a masters in pediatric dentistry at the University of
Kentucky in 2012.
Juan F. is board-certified by the American Boards of
Pediatric Dentistry, Oral Medicine, and Dental Public Health. He is an active member of the American
Academy of Pediatric Dentistry, American Academy
of Oral Medicine, American Medical Association,
Hispanic Dental Association, American Academy of
Oral and Maxillofacial Radiology, American Association of Public Health Dentistry, International Association of Dental Research, International Association of
Dento-Maxillo-Facial Radiology, and American Dental Association. Juan F. is a fellow in dental surgery
from the Royal College of Surgeons at Edinburgh. He
is a member of the editorial board of the journals Pediatric Dentistry and Oral Surgery, Oral Medicine, Oral
Pathology, and Oral Radiology. He has many years of
experience in medicine, medically compromised children and dental protocols, diagnosis and radiology in
children and shares these experiences in outstanding
lectures.

Table Clinics are an important aspect of our state dental meeting. This is your opportunity to share
knowledge at a one-on-one level. Your registration fee for the Annual Session will be waived. Table
clinics will be assigned on a first come, first served basis, with priority given to dentists.
Table clinic applications will be accepted on availability of space capacity. To qualify to
present a table clinic, dentists must be members in good standing with the American Dental
Association.
Clinic set-up time: 7:30 – 8:30 a.m. Judging starts at 8:30 a.m.
Clinicians should limit presentations to 3-5 minutes for judges.

Table Clinics Procedures and Regulations for Clinicians
1.) Table Clinic applications must be submitted to the IDA by April 14. Applications will be accepted on availability of space capacity.
2.) Table clinics present basic clinical and research information related to the practice of dentistry using photographs, charts, diagrams, narrative materials, Powerpoint presentations, and poster boards.
3.) All table clinic applications are reviewed by the Table Clinic Chairperson for content, quality of work and
relevancy to the general membership. The chairperson accepts those applications which, in his/her judgment,
best relate to the improvement of the health of the public and the science and art of dentistry. The IDA Table
Clinics chairperson reserves the right to reject or cancel the appearance of any person or to reject the material
submitted for exhibition.
4.) Clinicians must be at their table during the assigned times. Set up for the table clinics will be one hour prior to
judging start time on assigned date.
5.) The IDA will provide a 30” wide x 6’ or 8’ long x 42” high table, stools, electrical outlet, and small screen at
no cost. A sign with the clinic number, title of presentation, and names of clinicians will also be provided. All
A-V equipment must be brought by the clinician.
6.) While commercial organizations may sponsor table clinics, these must be purely scientific and devoid of commercialism. Advertising, sales or solicitation are not permitted in the table clinic presentations. Violation of
this policy will result in immediate closure and removal of the table clinic from the area.
7.) Trade Names: Promotion of any product or service by trade name is absolutely prohibited. Drugs and products
mentioned in the presentation generally should be identified by chemical formula or by generic name. Trade
names may be used for identification and classification purposes only. Should the use of trade names be advantageous to the attendees’ understanding of the presentation, the rationale should be submitted in writing to
the table clinics chair (30) days in advance of the clinic requesting approval.
8.) Film/Video Production: Daylight projection of filmed materials is permissible, but sound devices may not be
used unless accompanied by individual earphones.
9.) Patient Demonstration: Demonstrations involving procedures on patients or live models generally will not be
permitted unless given prior written approval by the Table Clinics chair. Only if, in the opinion of the chair
after consultation with representatives of the appropriate scientific and legal agencies of the Iowa Dental Association, such demonstrations do not violate local law and would contribute significantly to the advancement
of the art and science of dentistry will such demonstrations be allowed.
10.) Handouts are the responsibility of the clinician. The IDA will not pay to duplicate handouts.
11.) The IDA is not responsible for any loss or damage to the clinician’s material or equipment.
12.) Table clinics will be judged by a panel of two judges selected by the Table Clinics Chair. Standard evaluation
forms provided by the IDA will be used to judge the clinics. Judging will start one-half hour prior to clinics
start time. Clinicians are requested to limit their presentations to 3-5 minutes for the judges.
13.) Certificates and Awards: Each clinician will receive a participating certificate. Certificates of recognition will
be presented to the clinicians chosen as honor clinicians. Returning Honor clinicians will receive plaques.
Dentists, dental hygienists, and dental assistants will receive (4) CEU’s and a continuing education card.
