UNIVERSITY OF CENTRAL MISSOURI Welch-Schmidt Center for Communication Disorders Audiologic Evaluation Report CLIENT: Tracy Blueberry DATE OF BIRTH: 12-1-80 CLINICIANS: Amy Arsenault and Honora Heili DATE OF EVALUATION: 1-29-01 HISTORY: Tracy Blueberry, a 20 year-old female, was seen at the Welch-Schmidt Center on January 29, 2001 for a hearing evaluation. The purpose of the evaluation was for extra credit in Dr. Wilson's class. Tracy reported that the only time she experiences difficulty hearing is in background noise. She denies tinnitus, vertigo, family history of hearing loss, and ear infections. Tracy reported that she has been experiencing cold symptoms for about two days, but she is not currently taking any medication for it. RESULTS: During today's evaluation, both tympanic membranes were visualized and were unremarkable. Conventional pure tone audiometry revealed pure tone averages (PTA) of 10 dB HL in the right ear and 5 dB HL in the left ear. An Auditory Brainstem Response (ABR) evaluation utilizing alternating clicks (19.1 clicks per second) presented at an intensity of 75 dB nHL revealed a wave I latency of 1.85 milliseconds (ms), wave III at 3.88 ms, and wave V at 5.60 ms, in the left ear. Due to time constraints, right ear wave latencies were not obtained. The latencies in the left ear appear to be within normal limits, indicating normal retrocochlear function in that ear. IMPRESSIONS: The results indicate Tracy is hearing within normal limits, with normal retrocochlear function in the left ear. Further testing should be done to complete the ABR on the right side.
