If a person experiences pain in the abdominal region it is necessary to record as
accurately as possible the location so that the source of the pain ascertained and a
diagnosis made.
Abdominopelvic Quadrants
A simple way of dividing up the abdominopelvic region of the body is by means of four
quadrants. These are obtained by drawing, on the surface of the body, two horizontal
lines and two vertical lines that intersect at the umbilicus or navel (see figure A). This
gives rise to four quadrants as follows:
RIGHT UPPER QUADRANT (RUQ): LEFT UPPER QUADRANT (LUQ)
RIGHT LOWER QUADRANT (RLQ): LEFT LOWER QUADRANT (RLQ)

Abdominopelvic Regions
The abdomen overlies many surgically important organs, e.g. the appendix and so for
more precise localization of pain is further sub-divided into nine regions. These are
obtained by drawing four imaginary lines as follows (see diagram below).
• 2 horizontal lines are drawn; a superior line connecting the inferior margin of the ribs
and an inferior line connecting the iliac crests (uppermost parts of hipbones).
• 2 vertical lines are drawn connecting the diaphragm to points approximately midway
between the pubis and the sides of the body (medial to the nipples).
These lines may be drawn on the surface of the body to mark the internal boundaries of
the following NINE ABDOMINOPELVIC REGIONS. In this way the location of
abdominopelvic organs can be pinpointed before surgery is undertaken. The nine
regions are in 3 zones as follows:
a)
A superior zone directly beneath the ribs containing:
- a central EPIGASTRIC REGION.
- two lateral HYPOCHONDRIAC REGIONS (right and left).
b)
A middle zone between the two horizontal lines containing:
- a central UMBILICAL REGION which includes the umbilicus or navel.
- two lateral LUMBAR REGIONS (right and left).
c)
A zone inferior to the umbilicus containing:
- a central HYPOGASTRIC or PUBIC region.
- two lateral ILIAC or INGUINAL regions (right and left).
