Acceptance and Discharge Protocol
Over-riding Principle – Kirklees PCT
NHS dental care in Kirklees will primarily be delivered by general dental contractors under the
terms of General Dental Services (GDS) contracts or Personal Dental Services (PDS)
agreements entered into between practices and Kirklees PCT. NHS Dental care provided by
CKCDC for Kirklees residents will be provided strictly in accordance with this agreed acceptance
and discharge protocol.
Categories of patients accepted:

Full routine care for children and adults with special needs, who cannot receive
appropriate care in the GDS – usually by referral from GDP / GMP / healthcare
professional. These special needs may include medical conditions or medication
impacting on dental treatment, learning difficulties, behavioural management problems
and mental health conditions.

Hospital-based dental extraction under general anaesthesia for children (up to their 16th
birthday)

Hospital-based dental treatment service for patients with special needs, who require their
care under general anaesthesia

Dental treatment service using sedation for patients who have a dental phobia – usually
referred from GDP / GMP / CKCDC dentist

Emergency-care for children without a regular dentist (up to their 16th birthday) who are in
pain/ trauma, where no other alternative exists e.g. daytime emergency service.
In addition, on receiving a request from the Kirklees PCT PALS Dental team, wherever possible
to provide temporary routine care for children where there is a high level of untreated dental
caries and insufficient provision within the GDS.
However, wherever possible children should be encouraged to attend a local GDP with the
others within the family. This will include local initiatives to address waiting lists identified by the
local PCTs. Details of all such acceptances will be reported back to Kirklees PCT every quarter.
CKCDC will provide regular care for patients who live within the geographical areas of
Calderdale and Kirklees PCTs.
Wherever possible, patients will be required to attend health centre premises to ensure that high
quality care can be provided in the most appropriate setting for the patient.
In certain circumstances, a salaried dentist working for CKCDC may assess a patient as needing
care on a domiciliary basis. This assessment will be reviewed on each occasion that the dental
team sees the patient, taking account of the patient’s needs, but also considering the limited
treatment that can be provided in such settings.

How to Refer

Referral from General Dental Practitioner or Other Health/Social Care Professional

Referrals must be made by letter or on an appropriate referral form, providing full
details of the patient’s medical history.

If required, further investigation regarding the referral will be made by the CKCDC
dentist with the health professional concerned.

Inappropriate referrals will be returned with an explanation letter.

Patients remain under the care of referring dentist/health professional until a dentist
within CKCDC has examined the patient.

On completion of treatment, a letter will be sent detailing the dental treatment and
proposed continuing care of the patient.
Referral for dental extraction under general anaesthesia

Referring GDP’s should complete DenGA forms 1, 2 and 3.

Any restorative work should be completed prior to referral

Where blood tests are necessary, referrers must ensure that test results are enclosed
with the referral forms
Patients making direct contact with CKCDC

Children in pain/ trauma will be asked to attend any CKCDC clinic across Calderdale
and Kirklees for assessment/treatment on a ‘sit and wait’ basis.

All other patients will be directed to their GDP, Local Care Direct, or Kirklees PCT
PALS as appropriate.
Retention/Discharge of Patients
 Patients originally referred to CKCDC by their General Dental Practitioner will be
referred back, wherever possible, to their original General Dental Practitioner.

Patients retained by CKCDC for continuing care, can be discharged at the discretion
of the dentist as long as there are definite alternative arrangements in place.

All patients discharged from CKCDC must be given full instructions on how to access
continuing dental care, i.e. by making contact with the Kirklees PCT PALS team on
0800 587 2417.
Patients who Fail To Attend
 For patients referred by their General Dental Practitioner who fail to attend, a letter
will be sent returning the patient back to their care and informing them that no
subsequent appointments will be made unless requested by them by re-referral.

For patients referred by other health professionals who fail to attend, a letter will be
sent informing them of the situation and explaining that no subsequent appointments
will be made unless requested by the health professional

For CKCDC continuing care patients who fail to attend a “DNA” letter will be sent
asking the patient/parent/carer to contact the surgery to make a further appointment.
If no contact is made within one month the patient will be classified as discharged.
