Term Functional Relationships with Purpose KEY TASKS Lead a team that ensures patients receive quality care by identifying and resolving problems that delay timely cancer treatment Produce patient centered pathways Support care coordination activities of tumour stream clinical teams • • • • • • Lead Cancer Coordinator EXPECTED OUTCOMES Work with Clinical Nurse Specialists to establish clinically appropriate and timely pathways for tumour stream groups Ensure co-ordination of care and support occurs for all patients with a cancer diagnosis Work with Maori and Pacific navigators to provide culturally sensitive and appropriate care Work with cancer tumour stream teams to ensure one point of contact for patients to simplify the pathway Establish patient information content and cancer community networks to support patients Ensure care planning approach is consistent through the tumor streams Waitemata District Health Board -JOB DESCRIPTION - Page 1 Lead Cancer Care Coordinator Position Description KEY TASKS Team leadership and role model WDHB values in all interactions Supervise data collection and reporting of FCT indicators Co-ordinate MDMs streamlining information & ensuring connectivity for team members in relation to cancer patients Implement across all major local tumour types Initiate improved ongoing care coordination processes EXPECTED OUTCOMES • Support and guide key staff in their specialty roles Ensures compliance with the requirements to measure FCT indicators High suspicion of cancer referral to FSA 14 days Confirmed cancer diagnosis to treatment within 31 days of decision to treat All patients referred urgently with a high suspicion of cancer receive their first cancer treatment within 62 days • Embed the practice of FCT indicator reporting and ensure that high quality monthly reports and accurate baseline information on FCT indicators are provided to the Ministry of Health • Work collaboratively across the cancer spectrum (including with regional partners) in the tumour stream pathways • Supports the development of automated data collection processes for the FCT indicators • Meet faster cancer treatment indicators • Work with CD and specialties relevant to Waitemata DHB clinical services to participate in regular MDMs • MDM Chairs are identified • Referral processes are timely with a specified minimum information set for each patient • Video conferencing facilities are user friendly, available and ready for use by MDMs • Administrative requirements for MDMs are in place and supported • • Together with clinical services responsible for the care of patients with cancer, design improvements to care coordination processes within each tumor group so to ensure that Waitemata meets any current and future treatment time targets, and addresses ethnic and other inequalities in cancer outcomes • Develop a scorecard and report on service development, progress made and KPI’s • Actively liaises with cancer care coordinators in other DHBs (most notably ADHB) who provide cancer care for Waitemata’s population. • Review performance and PD in line with development of new role and service.
