Mission Statement
Welcome to our practice website. We aim to provide the highest quality, patient centred,
personal and continuing medical care specifically for our local population.
To achieve this mission, our Health Centre is equipped with minor surgery, resuscitation,
research, counselling, treatment and ultrasound facilities, together with administrative,
conference, education facilities and offices. This enables us to support secondary care within the
Health Centre thus enabling us to commission services successfully for our local population.
The Friends of West Byfleet is a registered charity, through which we are able to buy medical
equipment not normally available through the NHS.

When We Are Closed
The Surrey Primary Care Trust provides emergency out of hours services through Thamesdoc
on 0300 130 1305. Patients may be offered advice by telephone, or asked to visit a Primary Care
Centre. For the deaf or hard of hearing, Thamesdoc can be contacted by text on 0800 500 888 or
by using the Type Talk service.
The following walk-in cenres are also available:


Woking Walk-In Centre - Woking Community Hospital, Heathside Road,
Woking. Telephone 01483 846209. Open - Monday to Friday 07:00 - 22:00 and
Saturday, Sunday and bank holidays 09:00 - 22:00.
Weybridge Walk-In Centre - Weybridge Hospital, Church Street, Weybridge.
Telephone 01932 826013. Open - Monday to Friday 07:00 - 22:00 and Saturday, Sunday
and bank holidays 09:00 - 22:00.
Please note that walk-in centres do not treat children under two years old.
NHS Direct
In the case of urgent need when the practice is closed you can also call NHS Direct
(www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and
advice offered or arrangements made for you to see a doctor.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an
emergency.

Appointments
To make an appointment telephone us on 01932 336880 or visit the reception desk during
opening hours. The practice offers an automated telephone system for booking, checking or
cancelling appointments. Routine appointments with the GPs may be booked up to a maximum
of twelve weeks ahead.




50% are available up to 8 -12 weeks ahead.
25% are held until 18:30 the previous day and can be booked via the automated booking
service from 18:30 - 22:00 and from 06:00.
25% are available from 08:30 on the day via reception.
Special appointments are available for medicals, minor operations, cryotherapy and
gynaecological examinations.
You may state the doctor of your choice, although this may depend upon availability. If you feel
it is necessary for you to see a health professional within 48 hours, please advise the receptionist.
You will be offered an appointment with the duty doctor or the practice nurse.
Telephone appointments are also available for consultations where you do not need to see a GP
face to face.
Practice Nurses
In a number of cases it might be worth considering an appointment with a practice nurse rather
than a doctor. Practice nurses are qualified to deal with many ailments and you may be seen
more quickly.
Cancellations
If you cannot attend an appointment for any reason please inform us as soon as possible in order
for us to give the slot to someone else.
Home Visits
If possible please try to telephone reception before 10:00 if you require a home visit.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your
GP will only visit you at home if they think that your medical condition requires it and will also
decide how urgently a visit is needed.
You can be visited at home by a community nurse if you are referred by your GP. You should
also be visited at home by a health visitor if you have recently had a baby or if you are newly
registered with a GP and have a child under five years.
Teaching & Training
This practice has been accredited as being suitable for the training of those doctors intending to
become general practitioners.
Points for your information:





Doctors training to become GPs are called GP Registrars. They are employed by the
practice and trained by a practice GP.
F2 doctors are qualified doctors gaining experience in the practice.
Sometimes other doctors are also trained and supported in the practice.
Please consult them with confidence - they have extensive hospital experience.
Please ask any member of the reception staff for further information if you need it.
Sickness Certificates
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your
employer may however require you to complete a self-certification form (SC2) which is
available from your employer or on the HMRC website (www.hmrc.gov.uk/forms/sc2.pdf) .
Evidence that you are sick
If you are sick for more than seven days, your employer can ask you to give them some form of
medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate,
now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence
that you are sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist.
Your employer will decide whether or not this evidence is acceptable. If your employer has any
doubts, they may still ask for a medical certificate from your GP.
Statement of Fitness for Work - ’Fit Note'
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help
you return to work sooner by providing more information about the effects of your illness or
injury.

Repeat Prescriptions
Patients on long-term medication and eligible for repeat prescriptions can order them in a
number of ways:



In Person - bring the tear off slip attached to your last prescription to the surgery, ticking
the item(s) you require. Written requests are also accepted if you have mislaid the slip.
By Post - send it in to us remembering to enclose a stamped addressed envelope if you
want your prescription posted back to you.
By Fax - to 01932 355681 making sure to include your doctor's name on the request.


Online - follow the link at the top of this page and carry out a simple registration.
Prescription Collection Service - ask your local pharmacy and they will advise you
what to do. The local chemists in West Byfleet, New Haw, Pyrford and Sheerwater
operate repeat prescription services.
Medication Reviews
Patients on repeat medication will be asked to see either a doctor, nurse practitioner or practice
nurse at least once a year to review these regular medications.
Please allow two full working days between requesting and collecting your
medication. Remember to take weekends and bank holidays into account and allow longer at
these times.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
These charges apply in England only. In Northern Ireland, Scotland and Wales
prescriptions are free of charge.




12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 15 items
in 12 months, you may find it cheaper to buy a PPC.



Clinics & Services
Clinics & Services
In addition to general medical consultations we are pleased to be able to provide specialist clinics
and services covering the following areas:
Antenatal/Postnatal - Our GPs run antenatal clinics in conjunction with the community midwife
to ensure the best possible outcome for mother and baby. Our midwife and health visitors hold
parent-craft classes at the health centre.
Anticoagulation - ongoing surveillance of Warfarin therapy after initial stabilisation by the
hospital.
Asthma / COPD - run by the practice nurse and doctor with a special interest in respiratory
disease. Your doctor or nurse will advise which patients should attend.
Audiology Clinic - provided by the Royal Surrey County Hospital here at the health centre two
days a week. The clinic sees patients for hearing assessments, hearing aid fittings, tinnitus and
balance problems.
Back Clinic - provided by Mr Schofield, consultant orthopaedic surgeon one afternoon a week
on alternate weeks. The clinic sees patients with surgical back problems, suspected disc prolapse
and acute sciatica.
Blood Pressure - 24 hour monitoring.
Blood Tests - In addition to the blood test service provided in our treatment room we provide
specialist blood tests for certain conditions.
Breast Clinic - provided by Professor Layer, consultant breast surgeon, one afternoon a week on
every 3rd week. The clinic sees follow-up patients with breast disease, breast cancer, melanoma,
sarcoma and general surgery.
Cervical Smears - Tests are carried out by appointment by the practice nurse. Chlamydia
screening is available for 18-25 year old males and females.
Child Health - run by the health visitors with 6 week postnatal checks carried out by GPs. You
may attend any time the clinic is open. A health visitor is available to weigh your baby and
discuss baby's progress and any problems you may have. Clinics are held as follows:



2nd & 4th Wednesday 13:15 - 14:30, Byfleet Library, High Road, Byfleet.
Tuesday 11:00 - 12:30, Woking Children's Centre, Blackmore Crescent, Sheerwater.
Thursday 09:30 - 11:30, Health Centre, Madeira Road, West Byfleet

The Postnatal Group is held on Wednesday 13:00 - 15:30 at Woking Children's Centre,
Blackmore Crescent, Sheerwater.
Immunisations - please ask your health visitor for details or speak to a practice nurse.
Contraceptive Services - including Mirena and copper coils, Implanon and contraceptive caps
during normal surgery appointments. Emergency contraception is available after discussion with
a GP or nurse or through the local walk-in centres.
Counselling - via a doctors referral.
Cryotherapy - treatment for superficial skin lesions including benign skin lesions.
Diabetes - we work closely with the specialist diabetic clinic at St Peter's Hospital. All patients
with diabetes on our register are invited for annual checks.
Eye Clinic - provided by Mr Tappin, consultant ophthalmologist, once a month. The clinic sees
patients with suspected glaucoma, retinol lesions, sore eyes, flashes and floaters.
Gynaecology Clinic - a team comprising GPs, nursing staff, sonographer and physiotherapist
specialise in female health disorders including menstrual/PMT problems, menopause/HRT,
polycystic ovaries, prolapse/bladder problems, fertility, uro-gynae continence.
Heart Failure Clinic - two mornings a week with Dr Cummin.
Minor Surgery - for non-cosmetic skin lesions that require removal under local anaesthetic.
Podiatry - this service is via self referral (telephone 01932 570607). Appointments are sent
directly to the patient by the Podiatry Service at Chertsey Health Centre. Clinics are held at West
Byfleet Health Centre.
Sexual Health - If you are concerned about sexually transmitted infections you may wish to
contact the Genito-Urinary Clinic at the Blanche Heriot Unit on 01932 722669 (Office),
or 01932 722390 (Advice).
Smoking Cessation - by appointment with a practice nurse or enquire at reception, where
information about support groups is available.
Travel Health - The practice nurses provide full and comprehensive travel advice and
vaccination service.
Non-NHS Services
Non-NHS Services
Some services provided fall outside the scope of the NHS and therefore attract charges.
Examples include the following:





Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
Insurance claim forms
Prescriptions for taking medication abroad
Private sick notes
Vaccination certificates
Our reception staff will be happy to advise you about appointment availability and applicable
charges.
Test Results
Test Results
You are welcome to call after 14:00 to enquire about your test results as our staff will have more
time to help you at this time.
You will be told how long you should expect to wait for the results at the time of your test so
please bear this in mind before calling. As a guide blood test results usually take 5-7 days and Xrays 10-14 days. It is then your responsibility to check them and make any necessary follow-up
appointment with the doctor.
Please note that we do have a strict policy regarding confidentiality and data protection. In this
respect we will only give out results to the person they relate to unless that person has given prior
permission for their release or if they are not capable of understanding them.
Blood Tests
Blood Tests
A blood test is when a sample of blood is taken for testing in a laboratory. Blood tests have a
wide range of uses and are one of the most common types of medical test. For example, a blood
test can be used to:



assess your general state of health
confirm the presence of a bacterial or viral infection
see how well certain organs, such as the liver and kidneys, are functioning
A blood test usually involves the phlebotomist taking a blood sample from a blood vessel in your
arm. and the usual place for a sample is the inside of the elbow or wrist, where the veins are
relatively close to the surface. Blood samples from children are most commonly taken from the
back of the hand. The child's hand will be anaesthetised (numbed) with a special cream before
the sample is taken.

X-Ray
X-Ray
An X-ray is a widely used diagnostic test to examine the inside of the body. X-rays are a very
effective way of detecting problems with bones, such as fractures. They can also often identify
problems with soft tissue, such as pneumonia or breast cancer.
If you have a X-ray, you will be asked to lie on a table or stand against a surface so that the part
of your body being X-rayed is between the X-ray tube and the photographic plate.
An X-ray is usually carried out by a radiographer, a healthcare professional who specialises in
using imaging technology, such as X-rays and ultrasound scanners.

Registration
New Patient Registration
Patients living within the practice area are welcome to register and our reception staff can
confirm your eligibility from your postcode. In this respect please bring along proof of address
such as a recent utility bill.
You will also need to complete a registration form (GMS1) and a health questionnaire which will
provide useful information whilst we wait for your medical records to arrive from your previous
doctor.
All new patients are offered a health check with a member of the healthcare team.
Medical treatment is available from the date of registration. Please contact reception for further
information.

Registering Online
Online 'Pre-Registration' With The Practice
If you wish to pre-register click on the link below to open the form. When you have completed
all of the details, click on the "Send" button to mail your form to us. When you visit the surgery
for the first time you will be asked to sign the form to confirm that the details are correct.
Pre-registration Form
When you register you will also be asked to fill out a medical questionnaire. This is because it
can take a considerable time for us to receive your medical records. There is an online version of
this file too, which you may fill out and send to us. When you come to the surgery you will be
asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the
Internet and although every effort is made to keep this information secure, no guarantee can be
offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your
first visit to the practice.

Temporary Residents
Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one
you can receive emergency treatment from the local GP practice for 14 days. After 14 days you
will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on
the local practice list and still remain a patient of your permanent GP. After three months you
will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do
not have to accept you as a temporary patient although they do have an obligation to offer
emergency treatment. You cannot register as a temporary patient at a practice in the town or area
where you are already registered.

We are introducing a new service called Vision Online Services. This will let you interact with
our practice using the Internet at your convenience, even when we are closed.
You will need to register with the practice to access this service, and once registered you will be
given information that will enable you to create your username and password.

Practice Policies
Confidentiality & Medical Records
The practice complies with data protection and access to medical records legislation. Identifiable
information about you will be shared with others in the following circumstances:



To provide further medical treatment for you e.g. from district nurses and hospital
services.
To help you get other services e.g. from the social work department. This requires your
consent.
When we have a duty to others e.g. in child protection cases anonymised patient
information will also be used at local and national level to help the Health Board and
Government plan services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us
know.
Reception and administration staff require access to your medical records in order to do their
jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.
Freedom of Information
Information about the General Practioners and the practice required for disclosure under this act
can be made available to the public. All requests for such information should be made to the
practice manager.
Access to Records
In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients
may request to see their medical records. Such requests should be made through the practice
manager and may be subject to an administration charge. No information will be released
without the patient consent unless we are legally obliged to do so.
Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a
genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly,
and as amicably, as possible.
To pursue a complaint please contact the practice manager who will deal with your concerns
appropriately. Further written information is available regarding the complaints procedure from
reception.
Violence Policy
The NHS operate a zero tolerance policy with regard to violence and abuse and the practice has
the right to remove violent patients from the list with immediate effect in order to safeguard
practice staff, patients and other persons. Violence in this context includes actual or threatened
physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we
will notify the patient in writing of their removal from the list and record in the patient’s medical
records the fact of the removal and the circumstances leading to it.

Practice nurses are qualified and registered nurses. They can help with
health issues such as family planning, healthy living advice, blood pressure
checks and dressings. The practice nurses run clinics for long-term health
conditions such as asthma or diabetes, minor ailment clinics and carry out
cervical smears.

Healthcare assistants support practice nurses with their daily work and carry out
tasks such as phlebotomy (drawing blood), blood pressure measurement and new
patient checks. They may act as a chaperone when a patient or doctor requests one.
Practice Management

The practice manager is involved in managing all of the business aspects of the
practice such as making sure that the right systems are in place to provide a high
quality of patient care, human resources, finance, patient safety, premises and
equipment and information technology. The practice manager supports the GPs and
other medical professionals with delivering patient services and also helps to
develop extended services to enhance patient care.

Receptionists provide an important link for patients with the practice and are your
initial contact point for general enquiries. They can provide basic information on
services and results and direct you to the right person depending on your health
issue or query. Receptionists make most of the patient appointments with the GPs
and nurses. They also perform other important tasks such as issuing repeat
prescriptions and dealing with prescription enquiries, dealing with financial claims
and dealing with patient records.
Community Nurses
The District Nursing service provided by Surrey Primary Care Trust is responsible
for nursing care to patients in their homes or residential homes in the community.
The district nursing sister is responsible for the assessment and review of the needs
of each individual patient and carer and the provision of an appropriate care plan.
Referrals to the service may be made either by your doctor, hospital or self referral.
District nurses also run clinics at New Haw Day Centre and St Mary's Day Centre.
Leg ulcer and well leg clinics are also available.
District nurse on-call - available 09:00 - 17:00 on 07966 236125 every day
including weekends and bank holidays.

Health Visitors
The team, which comes under Surrey Primary Care Trust, takes care of children
under the age of 5 and the elderly. All team members are qualified nurses with
specialist training in family health, child development and the elderly. Their
primary responsibility is for the promotion of good health and prevention of ill
health. They may be contacted on 01932 340411. A health visitor is usually in the
office between 09:00 and 10:00 and 16:00 and 17:00; otherwise a message may be
left with the health visitor's secretary or receptionist on 01932 356812.
Our health visitors work closely with our midwife providing parenthood classes.
Their prime function is to promote good health through education and to give
parents an understanding of their child's development. Apart from child health
clinics, they also carry out routine developmental checks on pre-school children.

A Macmillan Cancer Nurse is attached to the practice.
