Fitness Test Protocol
Before The Exam:
Q1. When a DHS fitness only exam has been scheduled with my facility, what should I
expect?
A1. A fitness only exam is given when an examinee has already received a comprehensive
medical exam and is physically testing for a specific position. The DHS fitness only exam
includes an EKG with interpretation, a blood pressure reading and the fitness test that is specific
to that job position. A fitness only exam also may include a random drug screen specimen
collection.
Q2. Will I receive the paperwork at my facility, or should I expect the examinee to bring it
with them?
A2. This depends on the type of exam the examinee is receiving:
 A comprehensive medical exam with or without a fitness test is sent to the examinee.
 A fitness only exam is sent to your facility.
Q3. What happens if I don’t receive the paperwork, or the examinee forgets to bring it with
them?
A3. Contact CHS at 866-493-0476 and speak with a representative to have the required
paperwork sent to your location.
Q4: What if I do not have the proper CD to conduct the fitness test?
A4. Contact CHS at 866-493-0476 to have a replacement CD shipped to your facility. Audio
recordings for temporary use can be found at www.chsmedical.com/dhscbpfitness
During The Test
Q1: A particular examinee came to our facility recently and had an EKG done at the time
of his/her appointment. We have a request to do another EKG with this exam? Is it
necessary?
A1: Yes, a new EKG is required.
Q2. If the examinee has an abnormal EKG or elevated blood pressure, what steps should I
take from there?
A2: Complete all authorized components except for the fitness test and clearly document on the
exam paperwork why the fitness test was not done. Please refrain from referring the applicant to
a specialist. The applicant MUST be referred back to CHS at 866-493-0476 Option 3.
Q3: The examinee did not sign the “Consent for Fitness/Performance Test” provided, but
they signed the facility consent form. Is this an okay substitute?
A3: No. It is VERY important to note that all examinees MUST sign the “Consent for
Fitness/Performance Test” form that is provided with the exam paperwork, BEFORE the fitness
exam is administered. It is understandable for the facility to provide their own form; if that is the
case, however, both forms must be signed.

Q3: If an examinee fails a portion of the fitness test, should I continue on to the other
sections?
A3: YES. If an examinee fails a section of the fitness test, please stop the specific section of the
fitness test at the point of failure and continue to the next section of the fitness test. Each failure
should be accompanied by a “Statement Regarding Fitness Test Failure”, filled out in its entirety,
signed and dated.
Q4: Are women allowed to do female modified push-ups during the fitness test?
A4: No. The protocol requires every examinee to be tested with the standard “proper-form”
push-up.
Q5. Should I fail the examinee during the step test if he cannot change his/her feet?
A5. No. The examinee is not required to alternate feet during the step test.
Q6. Can I give the examinee another chance?
A6. No. All fitness testing requires prior authorization from CHS. If there are any issues or
concerns, please call Comprehensive Health Services at 866-493-0476.
After The Test
Q1. What do I do with the paperwork once the test has been conducted?
A1: Send all fitness paperwork to CHS in the provided FedEx mailer on the day of the
appointment.
Q2. I didn’t receive a FedEx envelope to return the exam to Comprehensive Health
Services. What should I do now?
A2. Fax the exam to Comprehensive Health Services immediately following the appointment at
703-288-5454. If a FedEx envelope is not provided, the exam can be sent to Comprehensive
Health through regular mail:

Q3: What should I do if I have other questions not answered in this booklet?
A3: Please call Comprehensive Health Services at 866-493-0476 Option 3 for any additional
