Decide which club/clubs you wish to apply for.
Fill in the attached booking form.
Please make sure that you tick the clubs/activities you are applying for.
Return the booking form together with a cheque for the total amount. All the information is on the
booking form.
5. Make sure that you write your child’s name and clubs on the reverse of the cheque.
6. Make a note of the clubs that you have applied for below and retain for your information.
7. All applications must be returned to the office by FRIDAY, 13 MARCH 2015.
8. All clubs are booked on a ‘first come, first served’ basis. Children who already attend clubs must
reapply for the new term.
9. If any clubs are over- subscribed you will be informed and your child’s name will be put on a waiting list.
If you do not receive your cheque back or a call, please assume that you have been given a place.
10. All clubs will start w/c 27 APRIL 2015 and end w/c 29 June 2015 (9 sessions for all clubs) Club leaders
will inform you of any club date cancellations or changes throughout the term.
11. Refunds for any clubs will only be given in exceptional circumstances and will have £20.00 nonrefundable deposit deducted; please see the club teacher if you want to request a refund.
