A.4.2. Segunda línea
Tras quimioterapia basada en derivados del platino y en pacientes con buen PS, los fármacos que han
demostrado beneficio en supervivencia y calidad de vida son docetaxel, pemetrexed (en histología no
escamosa) y erlotinib. No está clara la actitud terapéutica tras progresión a gefitinib y erlotinib en pacientes
portadores de mutaciones activadoras de EGFR.

A.4.2.1. Tratamiento de mantenimiento
Hablamos de este tipo de tratamiento cuando usamos uno o varios de los agentes utilizados de inicio
en la primera línea, con los que se continúa hasta progresión o toxicidad.
Pemetrexed es el único fármaco que ha demostrado beneficio en SLP en histología no escamosa, tras
4 ciclos de quimioterapia de inducción con pemetrexed más cisplatino.

A.4.2.2. Segunda línea precoz/ “switch”
Usamos este término cuando continuamos con otro agente distinto al utilizado en los 4 a 6 ciclos
iniciales de inducción sin que exista progresión.
Tanto pemetrexed en población no escamosa como erlotinib, tras 4 ciclos de quimioterapia de
inducción basada en dobletes de cisplatino, han demostrado beneficio en SG y SLP.
Se debe considerar el mantenimiento y el “switch” en pacientes con PS 0-1 que no han progresado ni
experimentado una excesiva toxicidad durante la primera línea y que desean continuar tratamiento. Con los
datos de que disponemos, los pacientes respondedores parecen beneficiarse más del mantenimiento y
aquellos con enfermedad estable del “switch” o segunda línea precoz.
De todos modos, otra opción a tener en cuenta es realizar un seguimiento más estrecho de estos
pacientes, y tratar a la progresión.
