PerfectSmile TM Aligners by Aligning process from start to finish including cephalometric tracing for treatment planning PerfectSmile is a smoothly sequenced therapy for the correction of misaligned teeth with a series of clear, almost invisible aligners processed with the latest digital technology in Toronto, Canada. Starting with the patient’s present condition, a progression of set-ups is created aligning the teeth step by step and registered in 3D printed models. Clear aligners are fabricated based in those set ups in a special thermoformed polyurethane material engineered specifically for orthodontic movement. These aligners are cosmetic, thin, comfortable and removable. pg. 1 How does PerfectSmile work? This sequence of almost undetectable devices, will act without interfering with speech, work or other social activities. By seating them on the dental arches, the physical properties of the material they are made of, provide slight forces on the teeth aligning them progressively. Patients are requested to use each set of aligners for 2 to 4 weeks for most part of the day, being removed only for eating and brushing. According to the severity of the case, the number of treatment phases, number of aligners to be used and the time of treatment may vary. After Dr.’s approves the treatment plan, a PerfectSmile kit containing the aligners will be delivered. Included in the package, you will also find: pg. 3 1 Free set of PerfectSmile initial aligners to allow patients to get used easily to the clear aligners therapy and to confirm the accuracy of the scans or impressions. 1 Free set of PerfectSmile engagers builder trays. Designed to build up in office little composite bumps (engagers) in specific geometric shapes on the teeth whenever it is necessary, to provide better retention and anchorage for dental movement. After treatment is completed we will send a Free set of PerfectSmile clear retainers. pg. 4 General procedure First, Dr. completes the case submission form (online or hardcopy) and sends it with the scans or /impressions or/ models from the case, bite registration, along with available photos and x-rays. Usually a personal phone conversation will follow to assure a good understanding of the prescription. PerfectSmile will support your clinical evaluation, providing a free cephalometric tracing with your submission of a lateral ceph X ray. pg. 5 After approximately 5 business days, Dr. will receive in digital format the Virtual Treatment Preview (VTP) for his/her assessment. Based in his/her indications from the Case Submission form, Dr. will find animation files illustrating the evolution of the planned treatment step by step, images of the final expected outcome, composite attachments and Interproximal reduction guides in case any of these procedures were necessary. After Dr. reviews the VTP there will be 3 options to choose: a) to approve it, b) to hold it waiting for a further communication with PerfectSmile or c) to introduce changes in the treatment plan before approving it. Each set of aligners is numbered on the labial surface of the right last molar and they should be worn for at least 2 weeks as a default procedure. The first set to be given to the patient to be used as a first step is the PerfectSmile initial aligners set marked as U 0/ L 0 for upper and lower arches. This set provides the patient with a comfortable therapeutic start for better adaptation to the treatment. If composite attachments on the teeth are planned, do not place them yet, wait until the next appointment. Composite attachments should be placed on the teeth with a special set of trays, PerfectSmile engagers’ builder trays, made specifically for this purpose in a softer and accurate material that does not bond to the composite. There is no need to use a release agent, no hassle! Aligners should be used day and night, only removed for eating and cleaning and brushing teeth. pg. 6 Case selection Clear aligners therapy can improve most types of malocclusion. Misaligned teeth cases are of diverse complexity. Some may need more treatment time and a more extensive series of aligners than others, to achieve the planned treatment goals. Interproximal reduction, arch expansion and/or pro inclination of the teeth may be considered as resources to obtain the necessary space. Some others, due to factors like amount of dental movement required, dental anatomy or patient’s compliance, may need the utilization of traditional braces or different techniques combined to achieve the optimum success. For this reason, it is important to do a good case selection. The ones that are ideal for aligners treatment are those in which the alignment and rotation required is moderate and the space available is not too compromised. ROTATIONS OPENBITE DIASTEMAS DEEP BITE CROWDING ANTERIOR CROSSBITE More complex cases are suitable to be treated too, depending on different variables including Dr.’s experience with clear aligners therapy. pg. 7 As a general guideline, we can say that the possible corrections with clear aligners are within the following parameters. Crowding 1.0- 5.0 mm per arch Spacing 1.0- 5.0 mm per arch Midline correction 3 mm in total Distalization 2 a 4 mm per quadrant Expansion 2 a 4 mm per quadrant. Default amount of tooth movement per tray approximately: Rotation 3 degrees Tipping 1 degree Inclination 1 degree Mesial- distal 0.20 mm Labial- lingual 0.20 mm Intrusion/ extrusion 0.12 mm PerfectSmile phases The treatment is staged in phases and the aligners are delivered in sets of six per dental arch according to the complexity of the crowdedness or the difficulty of achieving the final planned outcome. By doing so and setting partial treatment goals to achieve in each phase the results are more accurate and predictable. After a 3 phases’ treatment, a few more steps or a fourth phase may be needed for refinement before retention.
