UniversityofCapeTown • Thevegetationshave occurredatasiteofminor trauma,wherethevalve cuspscomeintocontact eachtimethevalvecloses i.e.onthecontactline(not thefreeedgeofthevalve). UniversityofCapeTown • Therheumaticvegetationsorverrucaearisedueto inflammatoryerosionofthevalvesurface,followedbyplatelet andfibrinprecipitation. • Theyarethecardinalfeatureofrheumaticendocarditis. • Butendocarditisisjustonecomponentofthepancarditisthat occursinacuterheumaticfever,i.e.thereareinflammatory changesinallthreelayersoftheheart,endocarditis, myocarditisandpericarditis.
