an Influenza
Vaccination

Data shows that “Influenza is a seasonal illness; estimates attribute
influenza to approximately 200,000 hospitalizations and 41,000
deaths in the United Stated annually” (Anderson et al., 2016).
Outline of 5 Simple Steps:
1. Educate yourself on flu vaccinations
2. Decide on a date & time
3. Get vaccination
4. Print immunization sheet
5. Receive verification of completion
Step One: Educate Yourself on Flu
Vaccinations
• Locate brochures, flyers or
pamphlets
• Conduct personal research
Step Two: Decide on a Date & Time
• Call your local hospital,
pharmacy or clinic to set and
appointment
Step Three: Get Vaccination
• Report to correct location
• Bring your health insurance,
drivers license and extra
money.
• Receive the vaccination
Step Four: Print Immunization Sheet
• Ask for a printed copy of
your immunization sheet
• Keep it in a safe place
Step Five: Receive Verification of
Completion
• Report to your supervisor/boss
and turn in your immunization
form
• Receive a badge/sticker on name
