Section 1: Introduction to the Ongoing Achievement Record
BSc (Hons) Nursing Programme: Mental Health Nursing
The Ongoing Achievement Record (OAR) is a record of your achievement in practice throughout your period of pre-registration nursing
education. The OAR is the assessed practice element of the Skills and Practice modules within each year of the programme. It must be
fully completed by you, your mentors and personal tutors ready to be submitted to the Assessment Award Board at the end of each
year. At the end of Year 3 it will contribute as evidence towards achievement of the NMC requirements for registration. In order to
facilitate effective communication regarding your development in practice it is imperative that you ensure its safe keeping and you
are advised to photocopy and keep a copy of the summative assessment and Personal Tutor form for each year.
The OAR for the BSc (Hons) Nursing programme enables you to demonstrate continuity and progression throughout the programme.
The OAR includes the following elements:
Mandatory Training
Placement Induction
The Programme requires all students to
undertake mandatory training in a number of
areas each year.
This will usually be provided by Plymouth
University at the start of each academic year.
Your attendance will be recorded on your POW
record. It is your responsibility to ensure you
have recorded attendance, failure to do so may

When commencing placement it is a requirement that you complete the appropriate
Placement Provider induction within the stipulated timeframe. This ensures you are
familiar with policies and procedures relevant to your placement.
Induction may take the form of an on-line package or require personal attendance
you are encouraged to undertake this activity at the earliest opportunity before you
start your placement.

Nursing and Midwifery Council (NMC) Requirements
During your placement, you will be supported and assessed by mentors. You must work a minimum of 40% of your time with a
mentor under direct or indirect supervision, and an additional protected hour a week should be timetabled for you and your sign-off
mentor to discuss your progress in your final period before assessment (final placement Year 3).

You must meet the progression point criteria to move from one part of the programme to the next (NMC 2010).

A mentor is a nurse who has completed specific preparation in assessing students and is responsible for ongoing supervision
and assessment in practice settings. Other registered professionals who have been suitably prepared can supervise and
contribute to the assessment of nursing students.

A sign-off mentor is a mentor who has met additional criteria, is registered in the field of practice that the student intends to
enter and makes a final judgement of competence and proficiency during the final placement of Year 3.

If you have not achieved all module components by the Extended Resit Board, held 12 weeks after the end of year
Award Board, you will need to repeat the outstanding modules and you will be unable to progress into Year 2 or 3.
Practice Assessment
Assessment of practice in the pre-registration nursing programmes is through continuous monitoring of progress. You should gradually
move towards a point of practising at a level of proficiency, competence and accuracy under supervision. Formative and summative
assessment by your mentor must be documented at set points in the programme.
Your assessment will be based on domains of practice on which you will be assessed in every placement. These domains are listed
below;




Professional values
Communication and interpersonal skills
Nursing practice and decision-making
Leadership, management and team working
Completion of the OAR is assessed entirely in the practice placement. The OAR comprises the assessment requirements for each
domain with suggested guidance to aid achievement of the Standards for Pre-Registration Nurses (NMC 2010) as well as Quality
Assurance Agency Nursing Benchmark statements (QAA, 2001). The statements used in the domain boxes reflect the NMC
competencies that every nursing student must acquire before applying to be registered with the NMC.

The purpose of the OAR is to enable you to demonstrate that you have met the Nursing and Midwifery Council (NMC) progression
points and Standards of Competence for Pre-Registration Nursing Education (NMC 2010). During the three years of your programme
you will work in a variety of practice placements thus ensuring a breadth of experience.

The OAR must be submitted on the stated hand-in date; therefore you should ensure that all documentation is complete. All
practice assessments must be signed by your mentor at the end of each practice placement. If this is not possible for any reason then
they must be signed within 3 weeks following completion of the placement, after which time the mentor is permitted to decline to
sign them.
E-Portfolio
The e-portfolio will be assessed by your mentor therefore, you will be expected to share relevant evidence identified in the e-portfolio
with them. You will be required to demonstrate that you are developing your skills and knowledge in relation to the assessed domain. If
your mentor is unable to share your e-portfolio electronically then you must download copies of your evidence for your mentor to review.

Guidelines for Practice Assessment

Preparation for placement – before each placement you are required to undertake preparatory work which includes evidence of
the following elements of study;
 Common service user difficulties seen in the placement
 A Biopsychosocial understanding of these difficulties’
 Key elements of nursing care related to recovery orientated collaborative management of service users in
the placement
 Assessment tools and treatment modalities commonly used in the placement
 Common medications and treatments used in the placement (refer to BNF online)
 Action to be taken when escalating concerns or reporting incidents

Development plan – this must be commenced by you and your mentor, ideally within the first week of the placement. This will
enable you to identify what you hope to achieve in that placement. The development plan should be reviewed throughout the
placement to assess your progress.

Formative assessment of practice - is an essential part of the assessment process that supports your development. The
formative assessment is an opportunity for you and your mentor to review progress, strengths and weaknesses, and to plan how
to maintain your strengths and improve your weaknesses. The formative assessment enables you to undertake a selfassessment and to review the evidence gathered to support your achievements to date.

Action plan – if there are concerns raised during the formative assessment, the action plan should be completed to help you
focus on improving key elements of your practice.

Summative assessment timing – your summative assessment cannot be undertaken until you have gained at least 20 days or
150 hours experience on your placement. It is recommended that you should be summatively assessed towards the end of your
placement. Any concerns identified after summative assessment should be raised immediately with your personal tutor.

Evidence – to demonstrate achievement of a domain you must provide evidence to your mentor. A minimum requirement for
each domain is Direct Observation and Reflection. Your reflection should be no more than 750 words, referenced and should
relate to a further form of evidence such as Questioning or Statements from Others. So, in total, your domain assessment should
be supported with 3 forms of evidence.









Summative assessment of practice – is undertaken in a planned meeting when you and your mentor identify whether you
have successfully achieved the required level of performance for all the domains. You are required to achieve ALL domains as
specified within each year. When the summative assessment of practice documentation is completed, signed by you and your
mentor, and submitted to Programme Administration by the required submission date, your personal tutor will review your OAR
and it will then be returned to you.

Non achievement - if you do not provide sufficient appropriate evidence or you have been assessed as not competent you will
not pass the module. You may then need to be reassessed following a decision by the Award Assessment Board.

It is YOUR responsibility to check that all the necessary assessed domains have been fully completed and correctly
signed by the mentor. FAILURE TO DO SO MAY RESULT IN YOUR WORK BEING REFERRED

Expectations of Students in Placement
While you are in placement you are expected to;






Have undertaken the preparatory work before you start the placement
Work 40% of your time under direct or indirect supervision by a mentor
Work the shifts that the rest of the nursing team work i.e. 24 hour, 7 day if appropriate
Not normally work any more than 75 hours per fortnight (i.e. the equivalent of 37.5 hours per week).
Attend the hand over meetings at the beginning and end of the shifts
Not normally work more than two weekends in any four week period or more than 14 night shifts during the
programme. Where this is difficult, an alternative may be negotiated between student and mentor and personal tutor
 Agree a development plan with your mentor within the first week of your placement
 Agree the type of evidence that you intend to present to your mentor for each Domain
 Identify and complete wider patient activity
 Obtain completed written reports from a service user/patient /carer during each year of your nursing programme within
some clinical placements (see Appendix Service User Report Form)
Year One – at least one report
Year Two – at least two reports
Year Three – at least two reports, one of which is undertaken in the final sign-off placement.




Agree a date for your formative assessment with your mentor

Show your mentor your OAR and any evidence that they request to see
Communicate any concerns or incidents to your PDT Lead/personal tutor as soon as possible

Expectations of Mentors in Placement
While you mentor students you are expected to;






Be aware of the Expectations of Students in Placement (see previous page)
Ensure your student is directly or indirectly supervised by a mentor for at least 40% of their time in placement
Ensure that the student is rostered to work the same shifts as the rest of the nursing team
Agree a development plan with your student within the first week of their placement
Agree the type of evidence that you expect the student to present for each Domain
Support the student in undertaking wider patient activity and if your student requires it, recommend an appropriate patient/service
user/carer who would be willing and able to provide your student with constructive written feedback related to;











appropriate communication skills
provision of caring and compassionate nursing care
demonstration of dignity and respect at all times
involvement of patient / service user / carer / advocate in decisions of care
provision of information about care
Agree a date for your student’s formative assessment with you
Following their formative assessment agree a date for their summative assessment with you
Review your student’s OAR and any evidence that they present to you
Ensure that you have recorded the type of evidence seen and the mark you wish to award in the relevant section
Ensure that you sign the student’s OAR in the relevant places
Communicate any concerns or incidents to your PDT Lead/personal tutor as soon as possible

Grading of Practice

To effectively achieve equal weighting of marks between theory and practice, practice will be graded and contribute to the overall
degree classification for the student at the end of the programme. Grading practice rewards students who demonstrate a high
standard of competence and achievement in clinical practice; and who demonstrate integration of management and leadership
skills in their clinical placement settings.

Students are assessed in the all 4 domains in each placement area. The mentor will indicate the level of achievement the
student has met for each domain. The level of achievement awarded, from 0 to 100%, should reflect the skills, knowledge and
values pertinent to the stage of the student’s programme and the student should demonstrate these both in the practice area
and with the evidence collated in their e-portfolio. Only grades identified in the banding should be used (e.g. 52% or 55% or
58%).

A minimum grade of 40% must be gained for each domain for a pass to be achieved.
If the student is given a score of less than 40% (referral grade) for any domain then an Action Plan will be negotiated between
the student, mentor with the personal tutor/Placement Lead support and guidance on how to develop this domain further for
resubmission. The student may work on any referred domains during their last practice placement in each year of the
programme. However cannot be submitted until the date advised by the Award Board and any resubmitted domains will be
capped at a grade of 40% for BSc (Hons) level. Under University Regulations the student is only allowed two summative
attempts at practice.


An optional placement can be arranged to enable nursing students studying with Plymouth University to gain practice experience in
other healthcare provider settings either inside or outside of the South West region. An optional placement is an exciting opportunity for
students to plan and organise a short period of learning in an area either not available locally or in an area where employment has been
sought. Please refer to the guidance on POPPI for information regarding optional placements.
Please ensure you meet the timelines required to ensure an optional placement is available.
