Transcripts Official transcripts from all colleges and universities attended are required. 4 Graduation Goal Masters of Science in Nursing (MSN) 2 track options: • Nurse Practitioner in Adult Gerontology, Primary Care • Clinical Nurse Specialist, Adult Gerontology Health Application Deadlines Priority application deadline for the MSN program (either CNS or NP track) cohort beginning in Winter 2016, is March 2, 2015. Applications accepted after this date, space permitting. 5 GPA • 3.0 cumulative undergraduate GPA (Applicants with a 2.75 undergraduate GPA may seek conditional admission) • Applicants with a master’s degree may use that GPA vs. undergraduate GPA. Writing Sample 4-5 pages demonstrating your writing skills, ability to support ideas with literature, and properly cite literature using APA format (6th Edition). Personal goal statement 2-3 page narrative statement demonstrating your writing skills in which you discuss your choice of CNS or NP track, in relation to your personal goals and skills. Goal statement does not require APA format. 6 Past Education BSN from an accredited school *Completion of a basic statistics course (descriptive and inferential statistics) with a grade of B or better within the last 10 years. Interview May be required. 8 RN’s with a bachelor’s degree in another discipline may seek conditional admissionto the CNS track, contingent upon successful completion of the following bridge courses: 1. Completion of a basic health assessment course 2. Completion of a community health nursing course and clinical from an approved university or completion of the ATI Community Health Examination 3. Introductory Statistics (Descriptive and Inferential) EMU’s MA 170 or SOC 250 will satisfy this requirement. These requirements must be satisfied prior to the start of the MSN coursework. Note, that applicants to the NP track, must possess a BSN by the start of coursework. 9 1. 2. If you are offered and accept admission into the MSN Program, you will need to do the following: Send in a $100 non-refundable deposit to hold your seat by the due date noted on your admission letter. The three spheres are overlapping and interrelated but each sphere possesses a distinctive focus. In each of the spheres of influence, the primary goal of the CNS is continuous improvement of patient outcomes and nursing care. 26 Key elements of CNS practice are to create environments through mentoring and system changes that empower nurses to develop caring, evidence-based practices to alleviate patient distress, facilitate ethical decision-making, and respond to diversity. The CNS is responsible and accountable for diagnosis and treatment of health/illness states, disease management, health promotion, and prevention of illness and risk behaviors among individuals, families, groups, and communities. 27 The NP program at EMU is a primary care NP program. The primary care role is often best suited for positions in the community, such as medical care offices, urgent care centers, etc. Primary care NP’s are generally not found in settings such as the ED or ICU’s. NP’s in these settings usually need preparation in acute care. 28 Nurse Practitioner For the certified nurse practitioner (CNP) care along the wellness-illness continuum is a dynamic process in which direct primary and acute care is provided across settings. CNPs are members of the health delivery system, practicing autonomously in areas as diverse as family practice, pediatrics, internal medicine, geriatrics, and women’s health care. CNPs are prepared to diagnose and treat patients with undifferentiated symptoms as well as those with established diagnoses. 29 Both primary and acute care CNPs provide initial, ongoing, and comprehensive care, includes taking comprehensive histories, providing physical examinations and other health assessment and screening activities, and diagnosing, treating, and managing patients with acute and chronic illnesses and diseases. This includes ordering, performing, supervising, and interpreting laboratory and imaging studies; prescribing medication and durable medical equipment; and making appropriate referrals for patients and families. 30 Clinical CNP care includes health promotion, disease prevention, health education, and counseling as well as the diagnosis and management of acute and chronic diseases. 31 Course Level Resident Tuition NonResident Tuition General Fee Technology Fee Student Center Fee 500-699 $480.75 $947.70 $27.25 $12.85 $3.75 **These rates apply to 2014-15 rates. They total $43.85 per one credit hour. **The Registration Fee is non-refundable. **Additional Program fees 32 To be eligible to qualify for the NFLP, through HRSA, students must be admitted to the MSN program, commit to serving as a nurse educator in a School of Nursing within one year of graduation, complete the Certificate for Teaching in Healthcare Systems (CTHC) program while admitted as a student in the MSN program, and: 33 Be a U.S. citizen, a non-citizen national, or a foreign national having in his/her possession, a visa permitting permanent residences in the U.S. Be a student in good standing (GPA 3.0) in the MSN-Teaching Concentration program at the school. Have no judgment liens entered against him/her based on the default on a Federal debt. 28 USC 3201 (e) 34 Service Requirements and Cancellation: The borrower has 9 months after graduating to begin working full-time as faculty member in a U.S. School of Nursing. At that time, the school will cancel an amount up to 85% of the principle and interest of an NFLP loan as follows: ◦ 1. Upon completion by the borrower of each of the first, second, and third year of full-time employment as a faculty member in a School of Nursing, the school will cancel 20% of the principle of and interest on the NFLP loan, as determined on the first day of employment. ◦ 2. Upon completion by the borrower of the fourth year of employment as a faculty member in a School of Nursing, cancel 25% of the principle of and interest on the NFLP loan, as determined on the first day of employment. Varicella Zoster (*History of any of the above diseases is not sufficient proof of immunity) 3. Proof of tetanus vaccination within the past five years. 4. Proof of having at least the first of three Hepatitis-B injections or a positive titer result or submission of a signed declination form (enclosed). 5. Annual documentation that the student is free from Tuberculosis. Freedom by TB is evidenced by: a. Negative TB skin test b. Current negative Chest X-ray c.
